AMENDMENT NO. 2
TO
AGREEMENT BETWEEN THE CITY OF MIAMI. BEACH, FLORIDA (CITY)-
AND
SFM SERVICES, INC. (CONTRACTOR)
PURSUANT TO INVITATION TO BID (ITB) 34 -08/09 TO PROVIDE GROUNDS
MAINTENANCE SERVICE FOR THE CITY OF MIAMI BEACH CAUSEWAYS,
ISLANDS AND OTHER LOCATIONS,.AND MOWING SERVICES
This Amendment No. 2 to the above subject Agreement is made and entered into this 91h
day of May, 2011, by and between the CITY OF MIAMI BEACH, FLORIDA, and SFM'
SERVICES, INC.; and hereby amends the Agreement as follows:
1. The Agreement shall be amended to - .add the following additional locations to
Contractor's Scope of Work, as set forth in, Section 3.1. of the ITB:
a. Fifth Street: From Alton Road east to Ocean Drive including all medians
and right of ways. Approximately 5.12 acres Thirty (30) full services visits
--and- {One — Hundred - -and =-four -- (1-04)- additional - litter- - services - annually, --
annual cost of 28,917.90.
b. Palm Island: Includes landscaping on medians `and along right of ways
south and north of the entrance/ gate house area and the medians
running west to east from east end of the island of the west end.
Approximately 2.76 acres or 211,689 sq. ft. Thirty (30) full services visits
annually and One Hundred and four . (104) additional litter services
annually, annual cost of $25,350.
2. The City and the Contractor agree that all other provisions of Agreement shall
remain unchanged and in full force and effect. This Amendment supersedes and
governs any prior negotiations, correspondence, or conversations applicable to
the matters contained herein, and there are no commitments, agreements, or
understandings concerning the subject matter, of this Amendment that are not_
contained in this document.
IN WITNESS WHEREOF, the parties hereto have caused this Amendment to be
executed by their appropriate officials, as of the date first entered above.
