Amendment 1 - Kerr & Downs Research AMENDMENT NO. 1
TO
AGREEMENT BETWEEN THE CITY OF MIAMI BEACH, FLORIDA (CITY)
AND
KERR &'.DOWNS RESEARCH, LLC (CONTRACTOR)
FOR COMMUNITY SATISFACTION SURVEY SERVICES
This Amendment No. 1 to the above subject Agreement is made and entered into this
30th day of January 2012, by and between the CITY OF MIAMI :BEACH, FLORIDA, and
KERR &DOWNS RESEARCH, LLC and hereby amends the Agreement as follows:
1. The Contractor will review the Customer User Survey Methodology and Survey
Instrument developed by CAP Government, Inc. for the City's Building
Department and provide input to the City on the content of the questions and
answers and Survey Methodology. 'The rate of pay for this work shall be at the
contracted rate of $150.00 per hour with a not to exceed maximum hours- of.30
hours for a total amount of $4,500.
2. The City and the Contractor agree that all other provisions of the Agreement shall
_remain unchanged and in full force and effect. This Amendment supersedes and
governs .any prior negotiations, correspondence, or conversations applicable to
the matters contained herein, and there are no commitments, agreements, or
understandings concerning the subject matter of this Amendment that are not
contained in this document.

executed by their appropriate. officials, as of the date first entered above.
