3. ALLEGED VIOLATION(S):
Please list the provisions of The Florida Election Code that you believe the person named above may have
violated. The Commission has jurisdiction only to investigation the following provisions: Chapter 104,
Chapter 106,and Section 105.071,Florida Statutes. Also,please include:

In violation of F.S.106.07,Stephen Cody,Treasurer of Save Miami Beach,has failed to provide the Miami Beach Office of the City Clerk with
the following Campaign Treasurer Reports:2015-R1(due November 6,2015);2015-R2(due November 13,2015);2015-M11(due December 10,2015);
2015-M12(due January 11,2016);2016-M1 (due February 10,2016);2016-G1 (due February 19,2016);2016-G2(due March 4,2016);
2016-G3(due March 11,2016);and the latest report 2016-M3(due April 11,2016).We have tried calling Mr.Cody,and sent him
two certified letters notifying him that these Campaign Treasurer Reports were not received.These letters were delivered,as evidenced in the attached
USPS tracking numbers and delivery status.Pursuant to F.S.106.07(8)(d)and F.S.106.07(8)(9),we are notifying the Florida Elections Commission of the
failure of a Political Committee to file a report after notice.We are asking the Commission to investigate this matter. In order to assist you

in the investigation,we have included herein copies of the notification letters sent to Mr. Cody with print-outs of the
USPS tracking numbers stating delivery. Please let us know of how we may further assist you in your investigation.
