Acknowledgement of fines/suspension for Board/Committee Members for failure to comply with

I understand that no later than July 1, of each year all members of Boards and Committees of the City of Miami
Beach, including those of a purely advisory nature, are required to comply with Miami-Dade County Financial
Disclosure Requirements.
One of the following forms must be filled with the City Clerk of Miami Beach, 1700 Convention Center Drive,
Miami Beach, Florida, no later than 12:00 noon of July 1, of each year:
1. A "Source of Income Statement;" or
2. A "Statement of Financial Interests (Form 1)¹ ;" or
3. A Copy of your latest Federal Income Tax Return.
Failure to file one of these forms, pursuant to the Miami-Dade County Code, may subject the person to a fine of no

Betsy Pérez was born in Philadelphia Pennsylvania on February 4th, 1963.
Being the youngest of five made her very competitive as she excelled in sports.
She was a gymnast, played tennis, golf, and was a varsity field hockey player.
After working as a secretary at a law firm in Philadelphia, her love for travel
brought her to Miami Beach in 1982 where she received top honors at her
tourism & travel school. From there she was immediately hired to work for
Norwegian Cruise Lines as there Air Sea Agent and cruise passenger greeter.
Betsy married her husband Rudy Pérez in 1984 and began their family. They
made their roots in Miami Beach eventually raising five children who all went to
Miami Beach schools. Betsy became very involved in the Miami Beach School
system volunteering in the PTA as treasurer, vice president and running the
supply store as well as working at Temple Beth Shalom as a Teachers Aid. She
also became very active in the Friends of North Beach organization and after
school programs at North Beach Elementary, Nautilus and Miami Beach Senior
High School.
The Pérez’ built their Entertainment companies together; Bullseye Productions,
Rubet Music Publishing, and Rudy Pérez Entertainment in Miami Beach. Their
companies became multi award winning production and publishing companies
receiving many accolades throughout the years such as Grammys, Billboard
awards, ASCAP awards as well as many others with Betsy as Vice President and
Production coordinator on all of the multi platinum selling albums that her
husband produces. They organize entertainment and cultural events, notable
ones such as the Miami Beach Centennial Concert, The Latin Songwriters Hall of
Fame La Musa Awards, North Beach Nights, as well as many others. She
negotiates all of his production contracts and publishing deals.
Betsy is currently the Executive Director of the Latin Songwriters Hall of Fame,
an organization that was cofounded by her husband. She worked with her
husband Rudy to found LARAS, Latin Recording Academy of Arts and Sciences
and was active in opening the Florida Chapter of NARAS, National Academy of
Recording Arts and Sciences. Betsy serves on the Board of Directors of the
Animal Welfare Society of South Florida and is an active member of the North
Beach Development Corporation.
