director of a major medical academic program has provided me within an opportunity to acquire a variety of skills
that would be helpful for each of the committee's to which i have applied.

Acknowledgement of fines/suspension for Board/Committee Members for failure to comply with

I understand that no later than July 1, of each year all members of Boards and Committees of the City of Miami
Beach, including those of a purely advisory nature, are required to comply with Miami-Dade County Financial
Disclosure Requirements.
One of the following forms must be filled with the City Clerk of Miami Beach, 1700 Convention Center Drive,
Miami Beach, Florida, no later than 12:00 noon of July 1, of each year:

Medical Biosketch
Dr. Slipman is one of the world’s foremost authorities on medical, interventional and
rehabilitation approaches to treating back and neck pain and has been recognized
internationally as a leading researcher and educator on the subject. He authored the
definitive textbook on interventional spine techniques; written more than 130 articles
and book chapters on a wide range of subjects ranging from the diagnosis and
treatment of spine pain to treating pain caused by sports injuries; and has been
repeatedly recognized as one of the “Best Doctors in America” a distinction awarded by
one’s peers to less than 1% of all physicians in a particular specialty.
Dr. Slipman graduated from Baylor College of Medicine in Houston Texas and did his
post graduate training at Columbia Presbyterian Medical Center in New York. Following
6 years of private practice in Mississippi and academic appointments at the University of
Mississippi Medical Center he was jointly recruited, by the Departments of
Rehabilitation Medicine and Orthopedic Surgery at the University of Pennsylvania to
initiate and develop the first academic spine center to be led by a physiatrist. One year
later he established the first academic interventional physiatry fellowship.
Dr. Slipman went on to serve as Director of the University of Pennsylvania Spine Center
for nearly 20 years; as assistant Professor in Penn’s Department of Orthopedics and
Associate Professor in the Department of Rehabilitation Medicine; and Director of Penn
Medical Center’s Interventional Physiatry Fellowship Program. Dr. Slipman has also
served as a Team consultant for the University of Pennsylvania’s sports teams for more
than 15 years.
Dr. Slipman has been the invited speaker at more than 200 conferences and physician
meetings including lecturing internationally including China, Italy, Germany, Korea,
Spain, Portugal, The Czech Republic, Canada, France, Turkey, and Japan. Dr. Slipman
has been an editor/reviewer of more than10 medical journals and served as editor in
chief of Pain Physician 2002-2004. Among his accomplishments is receipt of the 1999
Rosenthal award for for notable innovations in clinical achievement in the treatment of
lower back disorders from The American Academy of Physical Medicine and
Rehabilitation, the Justus Lehman Lectureship from the University of Washington, the
William Baer Lectureship from Ohio State University Dr. Slipman’s diverse body of work
includes producing videos on back pain for the Department of Labor; Time Life; and
WebMD.
