006-1994 DM ,..- r- CITY OF MIAMI BEACH Department Memo # 6-1994 DATE: January 21, 1993 TO: All Department/Division Heads FROM: Roger M. city Manage SUBJECT: Hazard Notification and Correction Program Hazardous conditions on city property lead to accidents, liability claims and lawsuits against the city which cost a substantial amount of self-insured funds each year. By prompt reporting and correction of those conditions, many claims can be prevented. With all employees reporting hazardous conditions immediately and those reported hazards promptly corrected, we will realize a substantial reduction in our claim and litigation costs. To motivate departmental and employee response, we are implementing a hazard notification and correction program containing incentive awards for the employees that report the most hazards and the department/divisions that most promptly correct hazards in their areas of responsibility. Please inform all employees in your department/division of this program and their responsibility to report to their immediate supervisor any hazards they find while working throughout the city. Each supervisor is to report the hazards to the appropriate city department responsible for that particular area or condition via the attached multi-part carbonless form (with distribution as noted). The department/division responsible for correction of the hazard must complete correction of the hazard within 72 hours of their being notified or explain why the hazard has not been corrected. Documentation of their corrective action is to be submitted to Risk Management via the same form (bottom portion) . Risk Management will maintain records on the number of hazards reported by each employee, supervisor and department/division for ~ the purpose of determining the recipients of the incentive awards. Risk Management will also maintain records on each r r r Page 2 department's correction activities. consist of: The incentive awards will the employee reporting the most hazards per quarter will receive $100. the supervisor forwarding the most notices for corrective action per quarter will receive $100. the department/division reporting the most hazards per quarter will receive $200 to be used for an employee luncheon. the department/division correcting the percentage of hazards reported to them for will receive $500 for an employee luncheon.
