Morales, City Manager a� F f " C--) , 8 DATE: December 5, 2013 SUBJECT City of Miami Beach Nurse Initiative istory and Status _X M T The purpose of this Letter to Commission is to update the Mayor and Commission_one status of the City's Nurse Initiative, and the most recent additional commitment by the Children's Trust of approximately $98,767 to continue to sustain and enhance the City's Nurse Initiative in the 2014-15 school year to provide a complete Health Connect in our Schools (HCIOS) suite to North Beach Elementary, Treasure Island Elementary, and Ruth K. Broad Bay Harbor K-8 Center. At the December 2012 Finance and Citywide Projects Committee meeting, the funding of the City's proportionate share for a full-time nurse, approximately $30,500, for the 2013-14 school year for underserved public schools in the feeder pattern was accepted. In May 2013, each neighboring municipality committed proportionate funding for the full- time registered nurse as follows: Town of Surfside $7,222, Town of Bay Harbor Islands $7,222, Bal Harbour Village $7,222, and North Bay Village $12,999. Additionally, the Miami Beach Chamber of Commerce committed funds to support this effort through an award of $30,000 from AETNA and a South Florida Health Foundation grant match based on municipal commitments of $65,000 and .the,,-Children's Trust expressed an interest in joining the effort and committed an additional match of $98,767 for the 2013- 14 school year. Resolution 2013-28258 approved in July 2013 (Attachment A) authorized the Mayor to enter into an agreement with the Miami Beach Chamber of Commerce, The Children's Trust, North Bay Village, Town of Bay Harbor Islands, Town of Surfside, and Bal Harbour Village to implement the Nurse Initiative for the 2013-14 school year. The City's Nurse Initiative provides a complete Health Connect in Our Schools suite; each participating school has a medical assistant on-site full-time and the municipal funded shared registered nurse rotates to each of the three schools. In August 2013, a Memorandum of Understanding (Attachment B) between the City of Miami Beach, North Bay Village, The Town of Bay Harbor"Islands, Town of Surfside, Bal Harbour Village, and the Miami Beach Chamber Education Foundation was executed, which provides for funding and implementation of the Nurse Initiative, with the Miami Beach Chamber Education Foundation serving as the fiscal agent. On August 19 2013 each of the three schools began receiving services from the 9 9 9 HCIOS. The Utilization Summary and Success Stories (Attachment C) provide information on the total number of unduplicated services for 582 children between August 19, 2013 and September 30, 2013. Of the children receiving services at the HCIOS, approximately 63 percent returned to class. Additional information on the Miami Beach Nurse Initiative (Attachment D) includes program goals, future expectations, and anticipated results. Dr. Peter Gorski, Chief Health and Child Development Officer, has indicated that the Children's Trust expects to continue the present form of Health Connect for another year in 2014-15. As such, the Children's Trust would continue to provide services to the three Miami Beach feeder schools contingent on continuing the current funding partnership per the Memorandum of Understanding and pending board approval at the Children's Trust March board meeting. As a result of the City's Education Compact with Miami-Dade County Public Schools, established in January 2008, the 2012 Community Satisfaction Survey indicated that 91% of residents who have children in public schools are very satisfied or satisfied with Miami Beach public schools compared to 51% of residents with children in public schools in 2007. The City of Miami Beach has made a significant impact throughout the entire county as the Compact between the City and the School Board is a model of how a City can strengthen the relationship between schools and community. Through our Education Compact, we will continue to support our strategic outcome to Enhance Learning Opportunities for Youth. WHEREAS, in January 2008, the City of Miami Beach entered into an Education Compact with the Miami-Dade County Public Schools to Enhance Learning Opportunities for Youth; and WHEREAS, the Education Compact reflects the desire of the Miami Beach community to support excellence in the City's public schools and enhance the health and well-being of our City's youth; and WHEREAS, as a result of the City's Education Compact with Miami-Dade County Public Schools, all public schools in Miami Beach offer the International Baccalaureate program which is internationally recognized 'as a benchmark for education excellence and also fosters student participation in community service; and WHEREAS, five of the eight school in our educational feeder pattern already have Health Connect in Our Schools (HCIOS) suites which are supported by The Children's Trust and which provides a full-time Medical Assistant and a shared Registered Nurse on campus; and WHEREAS, for students who do not require routine medications, the HCIOS suites are used for nursing assessments, administration of first aid, and other purposes such as consulting with parents and caregivers; and WHEREAS, the schools participating in the Nurse Initiative will have an HCIOS team of professionals including a shared Registered Nurse, a full-time Medical Assistant, and a social worker which is already funded by Miami-Dade County Public Schools; and WHEREAS, the benefits of HCIOS health teams include: a coordinated level of school- based healthcare, expanded health screenings, and assessments with access to follow-up care, mental and behavioral health services to identify and solve student health and educational issues, nutrition and health counseling, heath services for uninsured students with no other option for access to health care, emergency first aid/CPR, and chronic disease management; and WHEREAS, the disposition of every HCIOS visit will be tracked to assess the outcome for the students who utilize the services and the measure of whether or not the intervention the student received was effective is the rate at which the students are returned to class; it is expected pursuant to the Nurse Initiative agreement that 85% of the students who receive HCIOS services will return to class; and WHEREAS, during its December 2012 meeting, the City's Finance and Citywide Projects Committee met to discuss motions made by the Quality of Education Committee regarding a Nurse Initiative for the funding of a Registered Nurse at underserved public schools in the feeder pattern; and WHEREAS, the Finance and Citywide Projects Committee recommended funding the City's proportionate share of the estimated $62,000 for a full-time Registered Nurse with benefits, subject to the other neighboring municipalities also contributing their proportionate share of the Nurse's Initiative for the three underserved schools in the feeder pattern, specifically North Beach Elementary, Treasure Island Elementary, and Ruth K. BACKGROUND On October 24, 2012, Commissioner Libbin requested the motion made by the Committee for Quality Education regarding health care in feeder pattern public schools be referred to the Finance and Citywide Projects Committee to discuss consideration of the request to strengthen Education Compact activities as they relate to health and well-being. The Committee for Quality Education made the following motion at the September 24, 2012, meeting regarding health care in our public schools: The health and well-being of our children are of paramount importance and should be considered a community priority and responsibility. Therefore, the Committee for Quality Education recommends the Mayor and City Commission review the Health and Well-being activities in the Education Compact to strengthen them and consider funding support. The Education Compact with Miami-Dade County Public Schools (M-DCPS), established on January 16, 2008, includes the outcome to increase the health and well-being of students in Miami Beach public schools. The activities included in the Compact agreement to support this outcome include the following: • The school District will maintain health clinics(nurse practitioners and/or registered nurses) at Fienberg-Fisher, Biscayne, Nautilus, and Miami Beach Senior High, subject to continued available funding,and will work with all Miami Beach schools to coordinate visits from the Health Connect on Wheels service(as available)or similar service to-schools and/or Miami Beach recreation centers; • The District and the City will collaborate to deliver information regarding health and wellness education, prevention and intervention strategies regarding nutrition, physical activity levels, and health maintenance in accordance with the District's Wellness Initiative; • The City will explore the development of a middle school-level after-school program. The District will collaborate with the City by assisting with the recruitment of students, including providing space within District-owned facilities as needed, and exploring expanded transportation options for participating students; and, • District will-explore the expansion of full-time nurse practitioners and/or registered nurses at schools without health clinics, subject to available funding. Of the eight Miami Dade County Public Schools that fall within the City of Miami Beach feeder pattern, there is Health Connect in Our Schools (HCIOS) Suites in five of them: Biscayne Elementary, South Point Elementary, Fienberg-Fisher K-8 Center, Nautilus Middle School, and Miami Beach Senior High School. For students that do not require routine medications,the HCIOS is used for nursing assessments, administration of first aide,consulting with parents and caregivers, etc. These are considered Non-Routine Medication visits and they account for over 90%of all the services provided amongst the HCIOS health suites. Each of the schools above with health services has one medical assistant(health aide)on site full- time to provide the day-to-day services in the health suites. The equivalent of two full time higher- level nurses provides services to all five schools. Additionally,a social worker serves each school; however, M-DCPS provides this portion at Biscayne Elementary, South Pointe Elementary, and Miami Beach Senior High School. The remaining schools have HCIOS social workers. The Children's Trust contracts with nine providers to deliver health services via school based health suites'to approximately 5,793 students at five schools in Miami Beach currently receiving services. The three remaining schools in the Miami Beach feeder pattern that do not have full-time onsite services are North Beach Elementary(1,024 students),Treasure Island(683 students),and Ruth K. Broad Bay Harbor Elementary(1,265 students), of which approximately 1,297 reside within City of Miami Beach limits and attend either North Beach Elementary or Treasure Island Elementary. Currently, the three schools above are considered "Satellite" schools. Satellite (Basic) Schools Health Services was implemented to ensure that some level of basic health services are provided in all Miami-Dade County Public Schools by a registered professional nurse. Nurses are available to address on-call requests and consultations in Satellite(Basic)schools. These schools also receive the following health services from the Miami Dade County Health Department(per Pamela Toomer, Assistant Community Health Nursing Director): • School site visits throughout the year • School needs assessments with principal input • Tracking of students opting out of health services • Monitor immunization compliance • Monitor documented Temporary Medical Exemptions/Immunization Records • Consultation with school staff as needed • Consultation with parent/guardians as needed 2 • Individual (Student) Health Care Plans • Provide child specific trainings Additionally,per Wilma Steiner, Director Comprehensive Health Service MDCPS, mandated school health services for every-public school include preventive dental programs, vision screenings, hearing screenings, scoliosis screenings, growth and development screening/BMI, health counseling, referral and follow up of suspected or confirmed health problems, medication administration, prevention of communicable diseases,and health education curriculum. Complete optometric exams and lenses are offered to low income students through the Miami- Lighthouse, Children's Vision Program, available at the school site. ("MBCEF") (collectively, the "Parties"), to provide for funding and for the implementation of a Nurse Initiative as provided herein, and as to be provided in an agreement between MBCEF and The Children's Trust ("TCT"). WITNESSETH WHEREAS, the Parties have determined that a program to provide healthcare services to students attending underserviced public schools is in the best interests of the health and public welfare of the City of Miami Beach, North Bay Village, the Town of Bay Harbor Islands, the Town of Surfside, and Bal Harbour Village (the "Participating Municipalities"); and WHEREAS, the Parties have determined that students at North Beach Elementary, Treasure Island Elementary, and Ruth K. Broad Bay Harbor K-8 Center (the "Participating Schools") do not have full-time, on-site healthcare services and are, thus, underserved as compared to other public schools in the feeder pattern that falls within the City of Miami Beach; and WHEREAS, a Nurse Initiative program should be established at the Participating Schools as herein provided and pursuant to a separate agreement to be entered into between MBCEF and TCT. NOW, THEREFORE, in consideration of the mutual terms, conditions, promises, covenants, and payments hereinafter set forth, the Parties agree to provide for a Nurse Initiative program as follows: 1. MBCEF shall enter into an agreement with TCT whereby TCT shall provide funding in the amount of $98,767.00 for the Nurse Initiative and whereby TCT shall provide all salaries, fringe benefits, and costs of operations for a Health Connect in Our Schools ("HCIOS") Suite which will provide on-site healthcare services during the 2013-2014 school year at the Participating Schools ("Nurse Initiative") and shall provide certain schedules and reports as follows: 1of5 a) A full-time shared Registered Nurse who shall rotate between the three Participating Schools so that accessibility is equal amongst them pursuant to a detailed schedule that shall be provided to all of the Participating Municipalities on a weekly basis during the 2013-2014 school year; b) A full-time on-site Medical Assistant at each of the Participating Schools; c) A Social Worker already funded by Miami-Dade County Public Schools at each of the Participating Schools; d) A monthly report of services provided at each of the Participating Schools shall be provided to the Participating Schools and to MBCEF; and e) A monthly and an annual report shall be provided to the Participating Municipalities and to MBCEF documenting the percent of students returning to class after all nurse visits at the Participating Schools. 2. On or before August 15, 2013, MBCEF shall provide funding for the Nurse Initiative from the Health Foundation of South Florida Grant (School Healthcare Initiative) ($62,000.00) and the Aetna Voices of Health Award ($30,000.00). 3. On or before August 15, 2013, the Participating Municipalities shall provide their proportionate share of funds for the Nurse Initiative to MBCEF, which funds MBCEF shall hold in escrow pending the execution of its agreement with TCT referenced in paragraph 1. The Participating Municipalities' proportionate share of funding shall be in the following amounts: i) CMB shall provide funding in the amount of$30,500.00; ii) NBV shall provide funding in the amount of$12,999.00; iii) TBHI shall provide funding in the amount of$7,222.00; iv) TS shall provide funding in the amount of$7,222.00; and v) BHV shall provide funding in the amount of$7,222.00; except that NBV shall be permitted to provide its payment of $12,999.00 as follows: $1,749.86 before August 15, 2013 and $11,249.14 on October 2, 2013; and BHV shall be permitted to provide its payment of $7,222.00 as follows: $972.19 before August 15, 2013 and $6,249.81 on October 2, 2013. The funding provided by the Participating Municipalities and MBCEF shall be made by checks made payable to Miami Beach Chamber Education Foundation, Inc. 2of5 4. MBCEF shall serve as the fiscal agent to collect the funds required to be paid in paragraph 3 above and shall disperse such funds to TCT immediately upon MBCEF entering into the agreement with TCT referenced in paragraph 1. 5. MBCEF shall indemnify and hold harmless the Participating Municipalities and their respective officers employees, agents and instrumentalities from any and all liability, losses or damages including attorneys' fees and costs of defense, which the Participating Municipalities or their officers, employees, agents or instrumentalities may incur as a result of claims, demands, suits, causes of actions or proceedings of any kind or nature arising out of, relating to or resulting from the performance of this MOU by MBCEF or its employees, agents servants, partners, principals or subcontractors. MBCEF shall pay claims and losses in connection therewith and shall investigate and defend all claims suits or actions of any kind or nature in the name of the Participating Municipalities where applicable, including appellate proceedings, and shall pay all costs, judgments, and attorney's fees which may issue thereon. 6. In the event that any of the provisions in this MOU are not performed, or if the Nurse Initiative is terminated after funds have been dispersed to MBCEF as provided in paragraphs 3 and 4, then MBCEF shall promptly reimburse each of the Participating Municipalities their proportionate share of unused funds. O M � Q�W - O v a> >, ca (U a, ca au O Z W;R m = H D W H D o w C7 U C7 Z a F- HCIOS- MIAMI BEACH NURSE INITIATIVE SUCCESS STORIES TREASURE ISLAND ELEMENTARY On October 11 , a third grade student was sent to the clinic for redness, swelling and pain of the left inner elbow after sustaining a dog bite the night before. The mother was called immediately and she expressed concern about receiving medical treatment and possibly having to relinquish her dog. We consulted with Miami Dade County Public Health Department-Rabies specialist, and we were advised since the dog had required vaccinations she could simply obtain treatment via a Pediatrician. When we offered referral to Borinquen Medical Center, the mother reported she had gone to that clinic in the past. We immediately facilitated an appointment for her child/students the same day and mother took student to Borinquen's clinic on 36th St. On Monday October 14, the student was called to the health suite for follow up and the area was healing very well without complications. The student reported that the dog had bitten others in the past. Her parents had surrendered the dog to No Kill Animal Shelter to avoid any further injuries in the home. We continue to follow the student closely for any medical complications and she also had individual Safety /Accident Prevention educational sessions by our health suite team of Treasure Island Elementary. NORTH BEACH ELEMENTARY One day a student visited the Health Connect Clinic complaining of coughing and congestion in the chest. Staff took his vitals sign and listened to his lungs. There was slight wheezing in auscultation. Immediately the clinic staff contacted the mother of the child and explained why her son was in the clinic. She responded that her son suffers from Asthma. We requested if she could bring the Albuterol treatment to the school for her son. We also suggested that with proper documentation we can administer Albuterol treatment as needed. Upon receiving the medication and proper documentation, the students' symptoms subsided and he started feeling better immediately. His mother was thrilled that the Health Connect team was part of the North Beach Elementary school staff. RUTH K BROAD K-8 On October 7th a student came to the school clinic complaining of pain in the right middle arm after falling down while playing basketball in the school athletic field. Health suite staff assessed the arm and it was not swollen. Staff asked the student to try to move the arm and he did, but he was complaining of feeling pain. Staff applied a cold compress to avoid any swelling that could arise, called the student's mother and explained that the students' arm was not swelling or bruised, and that the student was able to move his arm. Staff also advised the mother to take the student to the doctor and have an X-Ray done to make sure that students' arm was had no complications. The mother arrived to the school clinic a half hour later and observed the students' arm, and she stated that the arm looked okay to her, but the health suite staff kindly suggested that the mother take the student to the doctor and have an X-Ray done, and she agreed. The next day, October 8th, 2013 the students' mother came back to the school clinic and she expressed the staff how happy and grateful she was with the advice she received, because she took her son to the doctor and the caregiver diagnosed a fracture of the forearm, specifically the radius, and the student has to wear a cast for four weeks.
