activate the voting equipment an •
g e q p d again n at 5.30 am on Tuesday,August 26, 2014, to begin
Election Day preparations.
To confirm the availability of your facility on these dates, and to update any pertinent information
we currently have on file, please complete, sign and detach the authorization form below.
Please return it in the enclosed envelope no later than Friday, June 20, 2014. You may also fax it to us
at 305-468-2504. If for any reason your facility is not available on the above listed dates, please call
the Polling Places Section at 305-499-8460 as soon as possible.
