Dear Mr. Ciraldo:
Please be advised that the Miami Beach Office of the City Clerk received the following
paperwork, relating to the Political Committee known as "Save Miami Beach 2016," on March 9,
2016:
• Form DS-DE 5 - Statement of Organization of Political Committee;
• Form DS-DE 6 — Appointment of Campaign Treasurer & Designation of Campaign
Depository for Political Committees;
• Form DS-DE 41 — Registered Agent Statement of Appointment.
For your records, we have included a copy of said forms showing proof of receipt by the Office
of the City Clerk.
As the area, scope and jurisdiction of the Political Committee is "All Miami Beach," you must file
all paperwork with the City of Miami Beach Office of the City Clerk, located at 1700 Convention
Center Drive, Miami Beach, Florida, 33139.
As your Political Committee was registered on March 9, 2016, you are due to submit Campaign
Treasurer's Report G3, which covers from the day of registration, March 9, 2016 through March
10, 2016. Please file this immediately, as it was due on March 11, 2016.
The next Campaign Treasurer's Report 2016-M03 will be due on April 11, 2016, and will cover
the period from March 11, 2016 through March 31, 2016.
Included herein is a form entitled "Important Dates and Deadlines," listing the dates the
Campaign Treasurer's Reports are due, Report Codes/Types, periods covered and other
relevant dates and information for the City of Miami Beach. Dates are subject to change.
Please contact the Office of the City Clerk for possible updates.
I have also attached hereto the Political Committee Handbook, dated January 27, 2016,
prepared by the Florida Department of State.
