EXECUTIVE PHYSICAL EXAMINATIONS
Date: January 22, 2002
We are pleased to let you know that we have arranged with Sunshine Medical Center to
provide you with access to an annual executive physical examination at no cost to you.
This examination includes a medical history, comprehensive physical examination, blood
tests that include a full lab panel, urinalysis, colon-rectal cancer screen, vision test, and
audiogram. An EKG and PSA test will be given if you are over age 40.
This physical may be done at any of the three Sunshine Medical Center locations:
907 S. American Way (Port of Miami)
6341 Sunset Drive, Miami
1750 SE 32 Street, Ft. Lauderdale
The results of the examination are confidential and are not provided to the City. If you are
interested in this physical, you may contact Pat Hippie, Human Resources Administrator, at
(305) 673-7000, ext. 6542 to arrange an appointment.
