Ordinance 81-2278ORDINANCE NO, 81-2278 AN ORDINANCE AMENDING CHAPTER 2 OF "THE CODE OF THE CITY OF MIAMI BEACH, FLORIDA" TO PROVIDE TO EACH REGULAR EMPLOYEE THE RIGHT TO ELECT PARTICIPATION IN THE CITY OF MIAMI BEACH EMPLOYEES' BENEFIT PLAN OR A HEALTH MAINTENANCE ORGANIZATION; REPEALING ALL ORDINANCES OR PARTS THEREOF IN CONFLICT THEREWITH AND ESTABLISHING AN EFFECTIVE DATE. BE IT ORDAINED BY THE CITY COMMISSION OF THE CITY OF MIAMI BEACH, FLORIDA: SECTION 1. That Section 2.52 of "The Code of the City of Miami Beach, Florida," be amended as follows: It is the intent of this plan to provide regular employees of the city protection against serious financial reverses arising out of non -service connected physical disablities of the employees and their immediate families, which are not intentionally self-inflicted or the result of their own misconduct; and this plan is intended to supplement benefits provided for service connected injuries by the workmen's compensation law in offering regular employees an opportunity for protection against other physical exigencies. The general scope of benefits shall as nearly as practicable conform to the following: and shall afford to each regular employee the right to elect participation in one of two plans. I. Group Health Insurance (a) HOSPITALIZATION: Hospitalization shall be provided for all cases involving surgery, and for illnesses under such limitations as the board shall determine. Rates may be separately established, and optional, for private, semiprivate or ward accommodations. (b) MEDICAL SERVICE: Medical service shall be provided for all illnesses requiring hospitalization, and for convalescence following hospitalization when required, not to exceed one call per day. This medical service may be made available in cases of illnesses which would normally involve hospitalization, but which because of special circumstances are treated in a home. (c) SURGICAL SERVICE: Surgical service shall be provided for all illnesses and accidents requiring the use of a medical surgeon for purposes of operating. -1- I. Group Health Insurance (continued) (c-1)DENTAL SERVICE. Dental service shall be provided for care and treatment of teeth as provided by a registered dentist. (d) Any employee, retirant, pensioner or dependent who is a member of the city employees' benefit plan and who is age sixty-five or over on or before July 1, 1966, and who is eligible for the benefits of that certain health and welfare plan of the United States Government commonly known as Medicare, Parts A and B, or who, had he or she registered for such Medicare benefits on or before March 31, 1966, would have been eligible for such benefits effective July 1, 1966, shall continue to receive the benefits of "The City of Miami Beach Employees' Benefit Plan," in the following manner: (1) Eighty percent of the remaining balance of costs for hospitalization, medical and surgical service after Medicare payments, as prescribed for all regular employees participating in the city employees' benefit plan, shall continue to be furnished to such employees, retirants, pensioners and dependents. II. Health Maintenance Organization (HMO) (a) HOSPITALIZATION: Hospitalization shall be provided for all cases involving illness and accident. (b) MEDICAL SERVICE: Medical service shall be provided for all illnesses and accidents. (c) SURGICAL SERVICE: Surgical service shall be provided for all illnesses and accidents requiring the use of a medical surgeon for purposes of operating. (d) Benefits for retirants, or same as under or pensioners benefits paid duplicated by over age 65 employees, pensioners shall be the 65 employees, retirents, with the exception that by medicare will not be the HMO. SECTION 2. That all ordinances or parts of ordinances in conflict herewith be and the same are hereby repealed. -2- • SECTION 3 This ordinance shall become effective as of November 1, 1981.
