A RESOLUTION PROVIDING FOR THE FORM OF
BALLOT TO BE USED AT THE GENERAL MUNICIPAL
ELECTION JULIE 2, 1953.
WHEREAS, oaths or affirmations in writing, were taken, signed,
subscribed and filed with the City Clerk by the following named
persons not later than noon, April 15, 1953, for the purpose of
qualifying as Candidates for the office of City Councilman in
the General Municipal Election to be held on June 2, 1953, and
WHEREAS, qualifying fees in the amount of $90.00 in cash,
were paid by each of said persons, to the City Clerk simultaneously
with the filing of said oaths or affirmations; all as prescribed
in Ordinance No. 1035, passed and adopted by the City Council
of the City of Miami Beach, Florida, on December 24, 1952;

WHEREAS, each of the above named candidates is qualified to
participate as an elector in the said General Municipal Election;
and
WHEREAS, the following question is to be submitted at the
election:
"SHALL THE FOLLOWING RESOLUTION BE ADOPTED?
"RESOLUTION NO.
"WHEREAS, there are two hospitals in the City of Miami
Beach, Florida, neither of which has, or operates emergency
ambulances, and
"WHEREAS, Emergency Ambulances are operated in the City
of Miami Beach by various Funeral Homes without supervision or
requirements for trained, medical personnel, and
"WHEREAS, accidents and sudden illnesses have become more
frequent, in the City of Miami Beach, with the growth of the
local and transient population, and
"WHEREAS, the City of Miami Beach attracts many elderly
people who enjoy its warm weather and sunshine, and who suffer
from serious ailments, and particularly from heart ailments, and
"WHEREAS, the City Council of Miami Beach recognizes the
need for providing facilities to protect the health, safety and
welfare of its citizens.

"NOW, THEREFORE, BE IT RESOLVED that the City Council of
the City of Miami Beach, Florida, shall initiate an Ordinance
directing the City Manager and the Hospital Commissioner of the
City of Miami Beach, Florida, to cause to be maintained by the
City of Miami Beach, Florida, within 90 days from the passage
of this Ordinance, an EMERGENCY AMBULANCE SERVICE; each ambulance
to be manned by a chauffeur and an attendant, at least one of
whom shall be trained in First Aid; that each ambulance shall
be equipped with a two-way radio, oxygen, blood plasma and
splints to immobilize fractures at the scene of the accident.
"BE IT FURTHER RESOLVED that an ambulance shall be dis-
patched AT THE REQUEST OF ANY RESIDENT who gives his name and
address, and states the nature of the emergency.
"BE IT FURTHER RESOLVED that a duly licensed physician,
or interne shall accompany the ambulance on all calls involving:
1. Heart Case 2. Maternity Case 3. Serious Accident
4. Unconscious Person 5. Apparent Death 6. Any case
where, in the opinion of the Emergency Ambulance Dis-
patcher, the presence of a physician is deemed necessary.
"BE IT FURTHER RESOLVED that the City Manager and the
Hospital Commissioner shall formulate such additional rules and
regulations as shall be deemed necessary to execute the spirit
and intent of this Resolution, and said rules and regulations
may provide for payment to the City of Miami Beach for such
services rendered. If any portion of this Resolution be held
invalid, the remainder thereof shall not be affected thereby. "
NOW, THEREFORE, BE IT RESOLVED by the City Council of the
City of Miami Beach, Florida, that the City Clerk be and he
is hereby directed to cause to be prepared a sufficient number
of ballots for use by absentee voters participating in said
general election, pursuant to the provisions of the law, which
ballot shall be in substantially the following form:

hereby directed to cause to be prepared two sets of official ballots
for each voting machine at each polling place for each election
district for use in and upon the voting machines, of which one set
shall be inserted or placed in or upon the machine and the other
retained in the custody of the Election Board unless it shall be-
come necessary during the election to make use of same upon or in
said machine, and to provide two sample ballots as provided by law
fol° instruction of voters and shall furnish to the election offi-
cials at each polling place a sufficient number of sample ballots
so that one may be given to each voter desiring same.
