Please be advised that the Miami Beach Office of the City Clerk received the following
paperwork, relating to the Political Committee known as "Prosperity with Transparency for
Miami Beach" on October 23, 2018:
• Form DS-DE 5 - Statement of Organization of Political Committee
• 2 Forms DS-DE 6 - Appointment of Campaign Treasurer & Designation of Campaign
Depository for Political Committees (Original Appointment of Treasurer)
• Form DS-DE 41 - Registered Agent Statement of Appointment
For your records, we have included copies of said forms showing proof of receipt by the Office
of the City Clerk.
As the area, scope and jurisdiction of the Political Committee are "issues PC in Miami Beach,"
you must file all necessary reports with the City, by means of the City's electronic filing system.
Included herein is the VR VoterFocus Candidate and Committee Handbook that will assist you
in how to upload financial reports online, as well as your candidate ID number, password, PIN,
and Treasurer's PIN. Please be advised that your first Campaign Treasurer's Report will be due
on November 13, 2018.
Please review the enclosed list of important dates and deadlines, the Political Committee
Handbook (latest edition, revised March 15, 2018), and the Florida Election Code of the Florida
Department of State Division of Elections (latest edition, revised June 2018).
