A RESOLUTION AUTHORIZING EXECUTION OF EXTENSION
OF AGREEMENT BETWEEN CITY OF MIAMI BEACH AND
THE DADE COUNTY DEPARTMENT OF PUBLIC HEALTH AND
THE FLORIDA DEPARTMENT OF HEALTH AND REHABILITA-
TIVE SERVICES, DISTRICT 11, FOR CONTINUATION OF
HEALTH CARE CLINIC.
WHEREAS, on June 1, 1977 , the City Commission of the City
of Miami Beach adopted Resolution No. 77-15345, which authorized
the execution of an agreement between the City and the Dade County
Department of Public Health and the Florida Department of Health
and Rehabilitative Services, District 11, for the creation and
operation of a primary health care clinic, and
WHEREAS, pursuant to said Resolution and Agreement, the
' Miami Beach Health Care Unit was created and has been in operation
since September 26, 1977, and will terminate its operation on
September 25, 1978 , by the terms and conditions of said Agreement,
and
WHEREAS, in order to prevent an interruption in the pro-
vision of said primary health care services to this segment of the
population, it is necessary that the said Agreement be extended
up through and including November 30, 1978, and
WHEREAS, the extension of said agreement will not result
in additional costs to the City as funds for the extension will
be generated by the City's reimbursements from the Medicare and
Medicaid programs, and
WHEREAS, the entry into of said Extension of Agreement
has been recommended by the- City Manager, and has been approved
as to legal form and sufficiency by the City Attorney, and
WHEREAS, the City Commission of the City of Miami Beach
has found and determined that it is in the best interests of the
citizens and residents of Miami Beach that said extension be. entered 6.

NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION
OF THE CITY OF MIAMI BEACH, FLORIDA, that the Mayor and City Clerk
be and they are hereby authorized and directed to execute and
deliver said Extension of Agreement for and on behalf of the City
of Miami Beach.

EXTENSION OF AGREEMENT
THIS AGREEMENT made and entered into this 6th day of
September , 1978 , by and between the DADE COUNTY DEPART-
MENT OF PUBLIC HEALTH, a local health unit created under and
pursuant to Chapter 154 , Florida Statutes, and the FLORIDA
DEPARTMENT OF HEALTH AND REHABILITATIVE SERVICES , DISTRICT 11 ,
an agency and instrumentality of the State of Florida, hereinafter
collectively referred to as the party of the first part and the
CITY OF MIAMI BEACH, a Florida municipal corporation, hereinafter
referred to as the party of the second part,
WITN SETH:
That the agreement now exis ' g between the party of the first part
and the party of the secop t en ed into June 1, 1977 and
expiring September 25, 1 8 in t same terms and provisions is
hereby extended up throw and iluding November 30 , 1978 .
