Commissioner Good Morning;
We need the Form 9, being a City Commissioner you only have to file the Form 9 with the State. The
county form we received I will still mail this morning to the County and send you a copy of the
tracking when I input it in the system. Did you file the Form 9? This is required for the State. Please
send us a copy and if you need me to file it I will send out. We would like to keep a copy on file in
case we ever need for the future.
