SUBJECT: (RFP NO. 42- 7/02) COMPREHENSIVE MEDICAL EVALUATIONS, MEDICAL
RECORDS RETENTION, PSYCHIATRIC SERVICES, AN IMMUNIZATION
PROGRAM, AN EMPLOYEE ASSISTANCE PROGRAM, AND WORKER'S
COMPENSATION CLAIMS EVALUATION COMMITTEE RECOMMENDATION
I intend to appoint the individuals listed below to serve on the Medical Services Evaluation
Committee. These services are provided to all City employees. Pre-employment medical
examinations are administered to hired applicants.
1. Florence Cohan, Training and Development Coordinator
-Supervises the drug and alcohol random and reasonable cause testing program,
employee assistance program, wellness program, and training.
2. Linda Gonzalez, Human Resources Administrator
-Supervises employment and promotional hiring, pre-employment medical evaluations,
and fitness for duty evaluations.
3. Clifton Leonard, Risk Manager
-Supervises all worker's compensation and safety issues.
4 1. C. Adder/y, Human Resources Director - City of Miami Beach
-Supervises all functions of the Human Resources Department
5. Gilda Delgado, Human Resources Director - City of Hialeah
-Supervises all functions of the Human Resources Department
Ed Beecher, Risk Manager - City of Hialeah
(Back-up member for Gilda Delgado)
6. John Mallah Esq., Attorney and resident of Miami Beach
-Attorney specializing in Civil Law on Miami Beach.
7. John Bremer, Personnel Board Member and resident of Miami Beach
-Designed and implemented systems for community health centers. Designed an
information system for health care program administration in a state human services
department.
8. Barbara Zubkoff, Chief Operating Officer - South Shore Hospital
9. Gail Poe-Liu, Human Resources Administrator
-Supervises police and fire employment and promotional hiring processes.
The evaluation committee is scheduled to meet on June 27,1998.
