FIRE STATION NO. 4 - PERMIT PROCESS STATUS

This is to advise you that the permit walk-through process for the Fire Station No. 4 New
Building Project has been completed as of Monday, June 14, 2004. This process includes
the approvals from the Miami-Dade County Water and Sewer Department (WASD) and
Department of Environmental Resources Management (DERM), including the payment of
the corresponding impact fees for water allocation, tree, grease discharge operation, and
plan review fees; and from all City of Miami Beach Building, Planning, and Public Works
Departments. The next step is to complete the assignment of the Building Permit
B0400493 to Carivon, Inc. (Carivon), the JOC Program Contractor selected to perform the
construction, and submit payment for the Code Compliance impact fees at the City's
Building Department.
In addition, to undergo the total demolition of the existing structure, the CIP Office has
already completed the assignment of the Demolition Permit BD040025 to Carivon at the
City's Building Department and has obtained the approval for demolition from the City's
Planning Department. Additionally, the CIP Office worked with Carivon to complete the
process of gathering the documents necessary to have the demolition permit issued.
These documents included Owner's affidavit, insurance certificates, workers compensation
and liability certificates, boundary survey, asbestos report, signed Vermin letter, and utility
disconnect confirmation. The scope required to secure these documents has been
completed and now that the Building Department has completed the plan approval, the
Contractor can return to DERM and secure an Asbestos Removal Authorization letter
needed to begin wet demolition; consequently, the issuance of the demolition permit will be
within a matter of days.
A meeting with the Fire Department representatives is being scheduled for next week, to
perform a walk-through of the existing premises to identify those items that need to be
saved and/or preserved. Under this proposed schedule, demolition should begin within
three weeks, or by the first week of July 2004, with reconstruction of the seawall and
construction of the new building scheduled to start in August 2004.
