A RESOLUTION OF THE MAYOR AND CITY COMMISSION
OF THE CITY OF MIAMI BEACH, FLORIDA,
AUTHORIZING THE MAYOR AND CITY CLERK TO
EXECUTE A FIRST AMENDMENT TO THE CONTRACT
BETWEEN THE CITY OF MIAMI BEACH AND MIAMI
HEART INSTITUTE TO PROVIDE COMPREHENSIVE
MEDICAL EVALUATIONS, EVALUATION AND
TREATMENT OF WORKERS' COMPENSATION CLAIMS,
RETENTION OF MEDICAL RECORDS, PSYCHIATRIC
SERVICES, IMMUNIZATION PROGRAM, DRUG &
ALCOHOL TESTING PROGRAM, WELLNESS PROGRAM,
AND AN EMPLOYEE ASSISTANCE PROGRAM FOR CITY
OF MIAMI BEACH EMPLOYEES; SAID AMENDMENT
EXTENDING THE CONTRACT UNTIL SEPTEMBER 30,
1997.
WHEREAS, the City solicited proposals, pursuant to Request for Proposals No. 33 - 94/97
(RFP), for a service provider to supply Comprehensive Medical Evaluations, Evaluation and
Treatment of Workers' Compensation Claims, Retention of Medical Records, Psychiatric Services,
Immunization Program, Drug & Alcohol Testing Program, Wellness Program, and an Employee
Assistance Program for City of Miami Beach Employees; and
WHEREAS, Miami Heart Institute (Miami Heart) agreed to provide the specified services
pursuant to the its response to the RFP; and
WHEREAS, a Selection Committee and the Group Insurance Board recommended that the
City award the RFP to Miami Heart as the top ranked proposer; and
WHEREAS, on September 13, 1995, the Mayor and the Commission authorized the
execution of a Professional Services Agreement between the City and Miami Heart to provide
Comprehensive Medical Evaluations, Evaluation and Treatment of Workers' Compensation Claims,
Retention of Medical Records, Psychiatric Services, Immunization Program, Drug & Alcohol
Testing Program, WeUness Program, and an Employee Assistance Program for City of Miami Beach
Employees (Contract); and
WHEREAS, the Contract was awarded for an initial one (1) year term with an option for a
second and third year term, at the City's discretion; and
WHEREAS, Miami Heart has continued to perform these services since the Contract was
awarded; and

WHEREAS, the primary users of this Contract, the City's Human Resources Department,
Police Department, and Risk Management, have been pleased with the quality of service; and
WHEREAS, the Mayor and City Commission deem it to be in the best interest of the City
to exercise the option to extend the Contract until September 30, 1997; and
WHEREAS, funding for the extension of the Contract has been requested in the Fiscal Year
1996/97 budget of the Human Resources Department.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE MAYOR AND CITY
COMMISSION OF THE CITY OF MIAMI BEACH, FLORIDA, that the Mayor and City
Commission hereby authorize the Mayor and City Clerk to execute the attached First Amendment
to the Contract between the City and Miami Heart Institute; said Amendment extending the Contract
through September 30, 1997.

COMPENSATION CLAIMS, RETENTION OF MEDICAL RECORDS,
PSYCIDATRIC SERVICES, IMMUNIZATION PROGRAM, DRUG &
ALCOHOL TESTING PROGRAM, WELLNESS PROGRAM, AND AN
EMPLOYEE ASSISTANCE PROGRAM (EAP), EXTENDING CONTRACT
THROUGH SEPTEMBER 30, 1997.
ADMINISTRATION RECOMMENDATION:
It is recommended that the Mayor and City Commission authorize the First Amendment to the
October 1, 1995, Contract with Miami Heart Institute for Comprehensive Medical Evaluations,
Evaluation and Treatment of Workers' Compensation Claims, Retention of Medical Records,
Psychiatric Services, Immunization Program, Drug & Alcohol Testing Program, Wellness Program,
and an Employee Assistance Program (EAP), said amendment extending Contract through
September 30, 1997.
CONTRACT COST OF AND FUNDING:
$98,7000 (Annual Cost). Funds are requested in the Fiscal Year 1996/97 Human Resources
Departm~nt budget.
BACKGROUND:
The City issued Request For Proposals (RFP) No. 33 - 94/97 on January 5, 1995, for a service
provider for Comprehensive Medical Evaluations, Evaluation and Treatment of Workers'
Compensation Claims, Retention of Medical Records, Psychiatric Services, Immunization Program,
Drug & Alcohol Testing Program, Wellness Program, and an Employee Assistance Program (EAP).
The Human Resources Department evaluated all proposals received and presented a Preliminary
Report to a Selection Committee comprised of: the Human Resources Director, the Training
Development Coordinator, a Personnel Specialist, the Risk Manager, the Employee Benefits

Coordinator, the C.W.A. president, and the A.F.S.M.E. president. Proposals were received from:
Mercy Hospital, Miami Heart Institute, Mt. Sinai Medical Center, Ornda Health Corp., and South
Miami Hospital.
On June 30th, 1995, the Group Insurance Board voted to recommend the award of the Contract to
Miami Heart Institute, commencing October 1, 1995, with options to renew for a second and third
year. This decision was based upon the savings to the City and the ability to perform the functions
specified in the Request for Proposal No. 33 - 94/97.
ANALYSIS:
There were only two (2) proposers who provided a flat contract price, Miami Heart (the current
contractor) and Mount Sinai (the previous contractor). Mt. Sinai Medical Center had additional costs
on top of the contract price. The proposals were evaluated by the Human Resources Department,
a Selection Committee, and the Group Insurance Board, who all recommended that the contract be
awarded to Miami Heart. During the evaluations, most factors were rated approximately equivalent,
except the area labeled General Concern (which included cost), in which Miami Heart was rated
more highly.
The previous Contract with Mount Sinai, file reference RFP No. 125 - 89/92, awarded July 25, 1990,
included a base bid of$139,781 annually; however, with the additional costs of specialized tests, the
annual amount exceeded $200,000. The Miami Heart proposal was for a total of $98,700, including
specialized tests, which is over $100,000 (or 50%) less than Mount Sinai.
The transition to the Miami Heart Institute (Miami Heart) for the City's employee health services has
been a good one. The staff of the Miami Heart cooperated fully with the City to make the transition
as seamless as possible, including renovating their drug testing facility to accommodate Police
Department needs. Employee reaction to Miami Heart has been positive.
As an extra benefit, Miami Heart and the Employee Assistance Plus program offered City employees
a series of Stress and Wellness workshops which focused on handling job related stress.
The monthly evaluations compiled the opinions of the various City employees who worked with
Miami Heart during the transition period, including staff from the Human Resources Department,
Police Backgrounds, and Risk Management. The monthly efficiency ratings ranged from 88.3% in
October, 1995, to 96% in February, 1996.
CONCLUSION:
Based upon the above, it is recommended that the Mayor and City Commission authorize the First
Amendment to the Contract with Miami Heart Institute, said amendment extending the Contract
through September 30, 1997.

AMENDMENT TO AGREEMENT
This Amendment to the Agreement, dated as of October 1, 1995, between the CITY OF MIAMI
BEACH, a Florida municipal corporation, having its principal office at 1700 Convenyion Center
Drive, Miami Beach, Florida, hereinafter referred to as the "City" and MIAMI HEART
INSTITUTE, hereinafter referred to as "Miami Heart", is entered into this ~ day of
~' 1996.
WITNESSETH:
WHEREAS, on October 1, 1995, the City entered into the Agreement with Miami Heart,
for professional services pertaining to the provision of Comprehensive Medical Evaluations,
Evaluation and Treatment of Workers' Compensation Claims, Retention of Medical Records,
Psychiatric Services, Immunization Program, Drug & Alcohol Testing Program, Wellness Program,
and an Employee Assistance Program for City of Miami Beach Employees; and
WHEREAS, persuant to the Agreement, the awarded Contract will be for a one (1) year term
with an option for a second and third year, at the City's discretion; and
WHEREAS, the City wishes to exercise its option to extend said Contract through
September 30, 1997.
NOW THEREFORE, the Agreement is hereby amended as follows:
1. The contract will be for a two (2) year term, extending through September 30, 1997, with an
option for a third year, at the City's discretion.
2. All other terms and conditions of the Agreement between the City and Miami Heart shall
remain in full force and effect.

IN WITNESS WHEREOF, the parties hereto have caused this Amendment to the
Agreement to be executed by their duly authorized officials on the day first above indicated.
