RE: ADDENDUM NO. 1 TO REQUEST FOR PROPOSALS (RFP) NO. 01-08109
FOR COMMUNITY SATISFACTION SURVEY SERVICES.
In response to questions and requests for additional information received by prospective
Proposers and the Administration's additional review of the requirements set forth in the RFP,
the RFP is hereby amended as follow:
Q#1: I did notice in the proposal documentation (on page 12) that a minimum requirement
was the conducting of such programs for local government entities. My question is
whether not meeting that requirement would automatically disqualify our firm from
consideration.
A#1: Yes. The proposer shall have at least three years of experience conducting
statistically valid surveys of Community satisfaction for local government.
Proposers will be deemed non-responsive if their proposal fails to include
documentation that demonstrates they meet the minimum requirements.
Q#2: Can you share the proposed budget for this project?
A#2: There is not a project specific budget at this time.
Q#3: Please clarify. On page 2 of the RFP, you state that: Sealed proposals will be
received until 3:00 PM on November 20, 2008. On the same page you also state:
Any proposal received after 3:00 PM on September 30, 2008 will be returned to the
proposer unopened.
A#3: Sealed proposals will be received until 3:00 PM on November 20, 2008 at the
following address:

Any proposal received after 3:00 PM on November 20, 2008 will be returned to
the proposer unopened. The responsibility for submitting proposals before the stated
time and date is solely the responsibility of the proposer.

Please be reminded that aPre-Proposal Submission Meeting is scheduled for October 30,
2008, at 10:00 A.M. at the following address:

Attendance (in person or via telephone) to this Pre-Proposal submission meeting is encouraged
and recommended as a source of information, but is not mandatory. Proposers interested in
participating in the pre-Proposal submission meeting via telephone must follow these steps:
(1) Dial the TELEPHONE NUMBER: 1-800-915-8704 (Toll-free North America)
(2) Enter the MEETING NUMBER: "2659980* (note that number is preceded and followed by
the star (*) key).
Proposers who are interested in participating via telephone, please send an a-mail to
mestevez@miamibeachfl.gov expressing your intent to participate via telephone at least one
business day in advance of the meeting.
Proposers are reminded to please acknowledge receipt of this addendum as part of your RFP
submission. Potential proposers that have elected not to submit a response to the RFP are
requested to complete and return the "Notice to Prospective Bidders" questionnaire with the
reason(s) for not submitting a proposal.
