AMENDMENT N0.3
TO THE PROFESSIONAL SERVICES AGREEMENT
BETWEEN
THE CITY OF MIAMI BEACH, FLORIDA
AND
STA ARCHITECTURAL GROUP,
DATED MAY 10, 2006
FOR THE PROVISION OF ADDITIONAL PROFESSIONAL SERVICES IN AN
AMOUNT NOT-TO-EXCEED 623,682.50, FOR THE 10T" STREET AUDITORIUM /
BEACH PATROL HEADQUARTERS PROJECT.
This Ame ment No. 3 to the Agreement made and entered this ~v~ day of
L' , 2009, by and between the CITY OF MIAMI BEACH, a Municipal
Corporation existing under the laws of the State of Florida (hereinafter r~efen'ed to as CITI~,
having its principal offices at 1700 Convention Center Drive, Miami Beach, Florida 33139,
and STA ARCHITECTURAL GROUP, INC., a Florida corporation, having its offices at 3526
North Miami Avenue, Miami Florida, 33127 (hereinafter referred to as Consultant).

WHEREAS, pursuant to Request for Qualifications (RFQ) No. 41-99/00, the Mayor
and City Commission adopted Resolution No. 2006-26194 on May 10, 2006, approving
and authorizing the Mayor and City Clerk to execute an Agreement with the Consultant for
Professional Architecture and Engineering Services for the 10~' Street Auditorium /Beach
Patrol Headquarters Project (the Agreement); and
WHEREAS, the Agreement provides for the detailed planning, design, permitting,
bidding /award and cons#ruction administration services for the Project; and
WHEREAS, the Agreement was executed #or anot-to-exceed amount of $775,925;
and
WHEREAS, on November 20, 2008, the City approved Amendment No. 1 to the
Agreement, in the amount of $23,800, for the provision of a threshold inspector #o conduct
structural inspections, and augercast pile testing observations; and
WHEREAS, on December 17, 2008, the City approved Amendment No. 2 to the
Agreement, in the amount of $23,692.50, for design revisions associated with the
expansion of the Ocean Drive sidewalk, Information Technology (tT) requirements and
Value Engineering (VE) recommendations provided by the Construction Management at
Risk Firm; and
WHEREAS, the following Amendment No. 3 is for design revisions associated with
Structural and Civil design revisions as a result of unforeseen conditions, DEP permit
applica#ion fee reimbursement, and additional reimbursables allowance; and
WHEREAS, the aforestated work, as set forth in the following Amendment No. 3, is
for a not to exceed amount of $23,445.00.
NOW. THEREFORE, the parties hereto, and in consideration of the mutual
promises, covenants, agreements, terms, and conditions herein contained, and other
good and valuable consideration, the respect and adequacy are hereby acknowledged,
do agree as follows:
1. ABOVE RECITALS
The above recitals are true and correct and are incorporated as a part of this
Amendment No. 3.
2. MODIFICATIONS
The Agreement is amended to include and incorporate the additional work, as set
forth in Schedule "A-3", attached hereto.
3. OTHER PROVISIONS.
All other provisions of the Agreement, as amended, are unchanged.
4. RATIFICATION.
The City and Consultant ratify the temps of the Agreemen#, as amended by this
Amendment No. 3.
IN WITNESS WHEREOF, the parties hereto have caused this Amendment No. 3, to
be executed in their names by their duly authorized officials as of the date first set forth
above.

SCHEDULE A-3
1. SCOPE OF SERVICES
Schedule "A" entitled, Scope of Services", is amended as follows:
ADD the following under Task 6 -ADDITIONAL SERVICES
The CONSULTANT shall furnish additional services associated with the 10~' Street
Auditorium /Beach Patrol Headquarters Project (Project) as noted below.
Task 6.4.1-Structural Design Revisions
The CONSULTANT shall revise the Structural drawings as required to incorporate the
following:
1. Design of new concrete stab at ground level, connection details to existing pile
caps and grade beams.
2. Redesign of structural mat slab and footing, due to underground utility conflict.
3. Design of round windows reinforcement detail due to unforeseen condition.
The City negotiated an additional service fee of $3,370 for this additional work.
Task 6.4.2 -Underground Utility Conflict
The CONSULTANT shall revise the architectural, civil and landscape drawings as
required to reroute the existing lines in conflict with new design.
The City negotiated an additional service fee of $4,525 for this additional work.
Task 6.4.3 -Electrical Design for Lummus Park
The CONSULTANT shalt revise the electrical drawings to inGude new equipment for
Lummus Park lights.
The City negotiated an additional service fee of $3,750 for this additional work.
Task 6.4.4 -DEP Permit Application Fees
The total amount paid to DEP on April O6, 2007 and May 16, 2008 was $4,300.
Task 6.4.5 -Additional Reproduction Services and Travel & Subsistence
Reimbursables
The City negotiated an additional service fee of $7,500 for the remainder of the
Project. Unused amounts in this allowance shall be credited back to the City at the
completion of the Project.
2. TIME OF COMPLETION
The additional services contemplated by this Amendment No. 3 are anticipated to
be completed by September 2009.
3. PAYMENT AND COMPENSATION
The total fee proposed for the scope of work pursuant to this Amendment No. 3
shall be on anot-to-exceed basis, in the amount of $23,445.
