RIAK ASSESSMENT PLAN1 " RISK ASSESSMENT PLAN ~~~ RISK #1: ,.ate. Geotechnical subsurface explorations performed in the South Pointe Neighborhood indicate the majority of the underlying soils to be of heavy organic composition. Condition As per Division 2, Section 02222, Part 3 of the Technical Specifications, the CONTRACTOR, shall cut two (2) feet deeper in all pipe/structure excavations and backfill with clean suitable fill. All backfill for pipe/structures shall be clean fill free from organics. Action Plan Once awarded the project, we shall immediately obtain an approved staging area large enough to help keep clean and expedite the completion of the project. Once all staging area paperwork is approved we shall begin to import clean suitable fill. The stockpiling of clean fill is essential for the timely and successful completion of the project. All spoils from the excavations must be stockpiled and allowed to dry out before hauling. RISK #2: Encroachments on City owned right-of--way were observed in various areas of the neighborhood. These encroachments in many cases are in conflict with the proposed improvements. These encroachments can be classified as hard or soft and have been assigned an encroachment classification. Condition Due to the sensitivity of encroachment removals, residents should be notified with ample time in order to give the resident an opportunity for self removal. In addition, due to the lapsed time since the final encroachment log was generated there may be additional encroachments that have yet to be identified. Action Plan Prior to commencement of construction, we shall conduct a preliminary right-of- way survey that will help identify all encroachments. A detailed list of encroachments will be submitted to the City along with a time scale in which removal of these encroachments will become imminent. RISK #3: Considering the volume of residents in the South Point Neighborhood, communication with residents is imperative during construction. In order to reduce the number of calls and letters to City Officials, residents need to be well informed of lane closures and day to day construction activities in there vicinity. Condition Residents that are not well informed will overwhelm the City with phone calls and emails. This will cause an inconvenience to the City and a burden to the residents. Action Plan Although Add Alternate No. 5 gives the City an opportunity to exercise the services of a Public Information Liaison, Horizon Contractors, Inc. will provide part time personnel in order to filter complaints from residents business owners to the City and to alleviate any unnecessary burdens to neighboring constituents. RISK #4: The South Pointe Neighborhood is densely populated. Most, if not all properties adjacent to the proposed improvements are multifamily homes. Condition Parking is extremely scarce in the neighborhood. Parallel parking must be minimally affected in order to minimally affect the residents and business owners within the project limits. Action Plan 1 of 2 Careful planning and utmost cooperation by the CONTRACTOR is essential in order to minimize inconveniences to the residents business owners of the South Pointe ~"' Neighborhood. In our experience, if the residents are well informed, inconveniences are kept to a minimum. This project will take extra personnel just to keep residents business owners informed. RISK #5: South Pointe Elementary School, located on 1050 4`~' Street, should be a major focal point for the CONTRACTOR. Condition Pedestrian and vehicle traffic significantly increases during posted school zone hours. Action Plan Production rates near South Pointe Elementary School shouldn't be the main focus of the CONTRACTOR. Keeping the adjacent streets open during posted school zone hours, is essential in therefore allowing for safe pedestrian and vehicle traffic flow. Construction should start once school zone hours have expired. Additionally, all trenches shall be backfilled properly and any hazards shall be barricaded before school zone hours begin for the afternoon. Production around the school will be slow, but safety must come first. RISK #6: The project plans call for the CONTRACTOR to retrofit some of the existing luminaires. Condition After performing various field observations, the supplier has determined that retrofitting the existing luminaires will not come at a cost saving to the City. Action Plan Existing luminaires shall be replaced at no additional cost to the City. All existing °'^ luminaires shall be turned over to the City's Public Works Department for spare parts. RISK #7 The project plans call for the installation of twenty-one (21) 24" Gravity Wells. Condition The installation of the wells is a timely process which can take between 10 - 15 days. Because of the location of the wells, the contractor must perform a series of lane closures which will further inconvenience neighboring business, adjacent residents and the traveling public. Action Plan At no additional charge to the City, state of the art equipment will be used in order to cut down installation times from 15 to 5 days max. VALUE ADDED SUBMITTAL Value Added Submittal #1 After performing various field visits, 6" and 8" fire lines with their respective double detector check valves were observed outside almost all condominiums in the alleyways. Neither the plans nor the specifications indicate what shall be done in these cases. At this point in time, it is extremely difficult to predict what will be done. Connections of these fire lines to the proposed main have been omitted from the base bid. Value Added Submittal #2 There is a possible discrepancy in the amount of services shown in the plans. Our bid includes all services shown on the plans. The unit price for the 1" services is $85.00 per linear foot. This price includes the tap at the main and all assemblies needed for the proper installation of a 1" service. Please note that the additional restoration is not included in this price. The additional restoration price is $1225.00 per service. This restoration price includes the lime rock base, asphalt, sod, curb and sidewalk needed to restore the area. 2 of 2
