The system was comprii ed of approximately 1,015 linear feet of exfiltration trenches that were to be constructed to a depth of approximately 18 feet below existing grade. This work was contemplated to cost approximately $125,000. Another more cost effective solution that incorporated the collected surface waters from the No mandy Shores Golf Clubhouse and adjacent surface parking area was explored. This op. ion provided water quality pretreatment and then discharged waters through the Normandy Shores right-oFway storm water management system. In order to make this change, it was necessary for the right-of-way consultant to re-design portions of the Normandy Shores right-of-way storm water management system to allow for the additional discharge, as well as, modify the existing Miami-Dade County Department of Environmental Resources Management Class ii permit, State of Florida Department of Enviro mental Protection Well permit and the South Florida Water Management District Environmental Resources permit. The tot~l fee for these services amounted to $24,376 for a total fee of $1,265,669. The work has been completed, permitted and constructed with respect to the Normandy Shores right-of-way storm water management system. RECITALS WHEREAS, pursuant to Request for Qualifications (RFQ) No. 134-99/00, the City adopted Resolution No. 2001-24386, on May 16, 2001, approving and authorizing the Mayor and City Clerk to execute an Agreement with the Consultant, for professional serviceis for the Neighborhood No.3 - Normandy Shores Right-of-Way (ROW) Infrastructure Improvement Project (the Agreement); and (WHEREAS, the Agreement was executed fora not-to-exceed amount of $620,800; and WHEREAS, the scope of the Right-of--Way Infrastructure Improvement Program Neigh orhood No.3 -Normandy Shores Project (the Project) is comprised of improviements to the following area: Normandy Shores, a residential island with approximately 200 single family homes and 43 apartment buildings surrounding the Normanndy Shores Golf Course and Fairway Park, which is linked to Miami Beach at 71St Street; and WHEREAS, the Project scope of work includes: enhanced landscaping and street lighting consistent with community preferences; repair, extension or widening of sidewalks to comply with ADA -Title III; street resurfacing/pavement markings and improvement of dead end streets with street lighting, landscaping, fencing, and/or parking, where appropriate; bicycle paths and pedestrian access ways; incorporation of traffic calming features, consistent with the community preferences; improved storm water drainage collection and disposal infrastructure; and provision of the necessary infrast ~ucture to accommodate the burial of overhead facilities, in the event the residents consider the creation of a special assessment district to underground electric, cable, and telephone services; and -1- WHEREAS, on May 13, 2005, Amendment No. 1 to the Agreement was executed, to include the provision of additional services necessary to prepare and attend a second pre-community design workshop, and community design workshop (CDW) for the Project, for a no-to-exceed amount of $12,811; and VHEREAS, on October 5, 2004, Amendment No. 2 to the Agreement was i, to include the provision of additional professional services necessary to undergrounding utility verification for the Project, in an amount not-to-exceed and WHEREAS, on March 18, 2005, Amendment No.3 to the Agreement was executed, to include the provision of additional professional services necessary to install a drainage test well for the Project, in an amount not-to-exceed $12,750; and EREAS, on March 18, 2005, Amendment No.4 to the Agreement was to include provisions for additional professional services necessary to concrete valley gutters to the Construction Documents for the Project, for a ed amount of $3,500; and WHEREAS, on July 14, 2005, Amendment No.5 to the Agreement was executed, to include provisions for additional services necessary to install an additional drainage test well for the Project, in anot-to-exceed amount of $500; and EREAS, on December 6, 2006 Amendment No.6 to the Agreement was to include the provision of resident project representation and additional n management services, in snot-to-exceed amount of $550,000; and WHEREAS, Amendment No. $7addresses additional services required for the >n for additional permitting services during construction, in anot-to-exceed t of $24,607; and ' WHEREAS at this time, the City, has negotiated Amendment No. 98for the provision for permitting assistance to incorporate additional stormwater drainage from the Normandy Golf Course Club House and parking lot; and WHEREAS the Consultant met with the City and Hazen and Sawyer, the City's Program Manager, and it has been established that this additional service(s) request appears fair and reasonable; WHEREAS, this Amendment No. 9l3increases the previously contracted amount of $1,263,023 by $24,376; resulting in a total fee of $t;~8~;999. fp~,65,669. SCHEDULE "A-J"8~~ SCOPE OF SERVICES CITY OF MIAMI BEACH RIGHT-OF-WAY INFRASTRUCTURE IMPROVEMENTS PROGRAM NEIGHBORHOOD N0.3 -NORMANDY SHORES SCOPE OF SERVICES The following new Task 4.11 shall be added to the existing scope of services: Task 4.11 - permitting Assistance for the stormwater Drainage from the Normandy Golf Course Club House and Parking Lot In this effort, the Consultant shall modify the existing Miami Dade DERM Class II permit, FDEP ,Well permit, and SFWMD ERP permit to incorporate additional stormwater drainage from the Normandy Golf Course Club House and parking lot. Additional modeling and design of facilities to pretreat this additional flow will be necessary for incorporation into the application. Deliverables - Assemble applications and supportive documentation for ' modification of .existing permits. Assemble permit modification request to the SFWMD ERP permit to include the additional stormwater drainage flow and structures. Assemble permit modification request of the FDEP Class V Well permit to include the additional stormwater drainage flow and structures. Assemble DERM Class II permit modification to include the additional stormwater drainage flow and structures. Attend meetings with the City, Program Manager, and/or permitting agency staff, and generate meeting minutes, as required to review, discuss and finalize permit procurement. Respond to Requests for Additional Information from the review agencies as needed. Notify the City of checks request for permit application fees and provide fee back-up information. Schedule -Assemble Request for Permit Modification on or before March 30, 2009. r` PAYMENT AND COMPENSATION The total fee proposed for this scope of work shall be snot-to-exceed basis in the amount of $24,376.
