AMENDMENT N0.4
TO THE EXISTING PROFESSIONAL ARCHITECTURAL
AND ENGINEERING SERVICES AGREEMENT
BETWEEN
THE CITY OF MIAMI BEACH, FLORIDA
AND
ARCHITEKNICS,INC.,
DATED AUGUST 11, 2006,
FOR THE PROVISION OF ADDITIONAL SERVICES, IN THE NEGOTIATED AMOUNT
OF$2,200, NECESSARY FOR THE DESIGN AND REVISION TO THE CONSTRUCTION
CONTRACT DOCUMENTS AND CONSTRUCTION ADMINISTRATION FOR THE ADDITION
OF A NEW KITCHEN DOOR AND AIR CURTAIN SYSTEM, INCLUSION OF A MISTING
SYSTEM TO THE EXTERIOR DINING AREA, AND MODIFICATION TO THE PLUMBING
DRAWINGS FOR THE INCLUSION OF A HOSE BIBB TO THE EXTERIOR DINING AREA
ALL FOR THE REPICA CLUB HOUSE BUILDING AT NORMANDY SHORES GOLF COURSE
This Amendment No. 4 to the Agreement made and entered this ~ ay of
~ , 2010, by and between the CITY OF MIAMI BEACH, a Municipal Corporation
existing nder the laws of the State of Florida, having its principal offices at 1700 Convention
Center Drive, Miami Beach, Florida, 33139 (hereinafter referred to as City), and
ARCHITEKNICS, Inc., a Florida Corporation having its offices at 7450 Southwest 48th Street
Miami, Florida 33155 (hereinafter referred to as Consultant).
RECITALS
WHEREAS, on May 10th, 2006, in accordance with Resolution No. 2006-26198, the Mayor and
City Commission approved a Professional Services Agreement with Consultant for design services for
the new Normandy Shores Golf Course cart barn building; the renovation of the maintenance building
and construction of a new chemical storage building and sand containment bins; the construction of two
restroom buildings; and the renovation of the club house; and
WHEREAS, on May 10th, 2006, in accordance with Resolution No. 2006-26197, the Mayor and
City Commission approved an Agreement for Pre-Construction Services with Coastal Construction
Company, to provide value engineering, constructability, and cost estimating assistance to Consultant
(Coastal Agreement); and
WHEREAS, on May 10t", 2006, in accordance with Resolution No. 2006-26197, the Mayor and
City Commission approved Amendment No. 2-A to the Coastal Agreement for partial demolition services
at the club house; and
WHEREAS, this partial demolition exposed structural problems and existing conditions that
differed from record drawings; and

WHEREAS, the City has established that it would be more cost effective and less subject to delay
if the existing structure is completely demolished and a replica club house structure is built; and
WHEREAS, at its February 5, 2008 meeting, the City's Design Review Board approved the
demolition of the existing structure and the construction of a replica club house; and
WHEREAS, on February 13, 2008, in accordance with Resolution No. 2008-26763, the Mayor
and City Commission approved the Amendment No. 3 to the Agreement, in the amount of $209,525, for
design and construction administration of a replica club house, for a new maximum fee of $138,175.
WHEREAS, the City and Consultant have negotiated the following Amendment No. 4 to the
Agreement, in the amount of $2,200, for the design and revision to the construction contract documents
and construction administration for the addition of a new kitchen door and air curtain system; inclusion of
a misting system to the exterior dining area; and modification to plumbing drawings (for the inclusion of a
hose Bibb to the exterior dining area); all for the replica club house building.
NOW, THEREFORE, the parties hereto, and in consideration of the mutual promises, covenants,
agreements, terms, and conditions herein contained, and other good and valuable
consideration, the respect and adequacy are hereby acknowledged, do agree as follows:
1. ABOVE RECITALS
The above recitals are true and correct and are incorporated as part of this
Amendment No. 4
2. MODIFICATIONS
The Agreement is required to be amended as follows:
2.1 SCHEDULE B -entitled "Consultant Compensation" is amended as follows:
ADD the enclosed Authorization for A/E Additional Services form per Article
5.3.3 (Exhibit 1 -additional service in the net amount of $2,200).
2.2 ADD the following Task:
Task 1.6 -Document Revisions
Based upon the input provided by the residents, consultant shall incorporate
the necessary contract document as approved by the City. (Exhibit 2 -
Letter from Architeknics Re: NSGC -Clubhouse Revision to Contract
Documents Proposal).
3. OTHER PROVISIONS
All other provisions of the Agreement, other than the deliverable schedule
specifically to this project, as amended, are unchanged.
4. RATIFICATION
The City and Consultant ratify the terms of the Agreement, as defined by this
Amendment No. 4
IN WITNESS WHEREOF, the parties hereto have caused this Amendment No. 4 to be
executed in their names by their duly authorized officials as the date first set forth above.

The following is our proposal for a lump sum additional services fee to revise permitted
construction contract documents as requested by City Commissioners through CIP. The
.scope of work includes the following revisions:
1) add "misting system" to outdoor dining area perimeter;
2) add a new door to the kitchen in the east wall;
3) add a new hose Bibb in outdoor dining area .
These scopes of work will require revisions to documents within the following disciplines:
architectural, landscape, structural, foodservice, plumbing and electrical. -
Revisions to the following documents for Bldg. Dept. and Contractor submittal are as
follows: Sheets L-l, IR-1, LS-1, LS-2, A-1.3, A-1.5, A-2.1, A-2.2, A-2.3, A-3.1, A-4.1, A-5.2, A-
9.1, A-9.2, S-2, S-5, S-7, P-1, P-2.2, P-3, E3.0, E4.0, E6.0, FS-1.1, FS-1.2, FS-1 .3, FS-1.4, FS-1.5.
The Lump Sum Fee for this work is $ 2,200.00
