A RESOLUTION OF THE MAYOR AND CITY COMMISSION OF THE CITY OF
MIAMI BEACH, FLORIDA, WAIVING, BY 5/7THS VOTE, THE COMPETITIVE
BIDDING REQUIREMENT, FINDING SUCH WAIVER TO BE IN THE BEST
INTEREST OF THE CITY; AND APPROVING AND AUTHORIZING THE CITY
MANAGER TO EXECUTE A FIVE-YEAR AGREEMENT WITH CITRUS
HEALTH NETWORK, INC., IN A FORM ACCEPTABLE TO THE CITY
ATTORNEY, FOR THE PROVISION OF TWO TREATMENT (2) BEDS TO BE
USED TO PROVIDE SUBSTANCE ABUSE AND MENTAL HEALTH
TREATMENT TO THE CITY'S HOMELESS POPULATION, AT A COST OF
$47,450 ANNUALLY.
WHEREAS, the Homeless Services Division of the Office of Real Estate, Housing, and
Community Development served 1,196 homeless persons in FY 2008/2009, of which 218
reported mental illness and 291 reported chronic addictions (including drugs and alcohol); and
WHEREAS, these chronic conditions pose impediments to finding and maintaining
stable housing; and
WHEREAS, findings by the National Resource Center on Homeless and Mental Illness
revealed that persons with mental and addictive disorders share many of the same treatment
needs, including carefully-designed client engagement and case management strategies,
housing solutions and long-term follow-up and support services, and these same findings
underscore the importance of service integration especially between outreach and engagement;
and
WHEREAS, the ability to directly access treatment beds when someone agrees to
receive services can impact the ability to engage them in services; and
WHEREAS, many treatment programs frequently have waiting lists for placements; and
WHEREAS, Citrus Health Network, Inc. (hereinafter referred to as "Citrus Health") is a
well-known provider of community mental health services, offering comprehensive programs for
homeless individuals, including the first "safe haven" low-demand emergency housing program
in the County; and
WHEREAS, Citrus Health's services include crisis stabilization, outpatient psychiatric
services, day treatment programs, and transitional and supportive permanent housing
programs; and
WHEREAS, since late 2004, the City's Homeless Outreach Team has partnered with
Citrus Health to provide specialized outreach to the City's chronic homeless population and
engage these chronically homeless clients in our community who suffer with mental health
and/or substance abuse problems; and
WHEREAS, pursuant to Resolution No. 2007-26622, on October 1, 2007, the City
entered into a one year agreement with Citrus Health Network, Inc. for the provision of two (2)
medically supported treatment beds to be used to provide substance abuse and mental health
treatment to the City's homeless population, especially those engaged through the specialized
outreach; and
WHEREAS, the 2007 agreement had two additional one-year renewal options, which
were both exercised, the last of which will expire on September 30, 2010; and
WHEREAS, it is recommended that in an effort to continue to complement the intensive
outreach to the chronically mentally ill, the City again contract with Citrus Health, one of the
homeless continuum's most experienced provider of services to the chronically mentally ill, at
the existing fixed fee of $65 per bed, per day, for a maximum of two (2) beds, totaling a
maximum of $47,450 annually.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION
OF THE CITY OF MIAMI BEACH, FLORIDA, that Mayor and City Commission hereby waive,
by 5/7ths vote, the competitive bidding requirement, finding such waiver to be in the best
interest of the City, and authorize the City Manager and City Clerk to execute afive-year
agreement with Citrus Health Network, Inc., in a form acceptable to the City Attorney, for the
provision of two (2) treatment beds to be used to provide substance abuse and mental health
treatment to the City's homeless population, at a cost of $47,450, annually.

ensuring consistency in approach, more intensive engagement and coordination of services. The ability to directly access
treatment beds when someone agrees to receive services can impact the ability to engage them in services. Many
treatment programs frequently have waiting lists for placements.
On October 1, 2007, the City entered into a one year agreement with Citrus Health Network, Inc. for the provision of two
(2) medically supported treatment beds to be used to provide substance abuse and mental health treatment to the City's
homeless population, especially those engaged through specialized outreach. The agreement had two additional one-
yearrenewal options, which were both exercised. The last renewal period will expire on September 30, 2010. The fixed
fee for the existing agreement is $65 per bed, per day, for a maximum of two (2) beds, totaling a maximum of $47,450
annually. The proposed new five-year agreement will include the same annual cost as was negotiated in 2007. Funding
for this program is included in the FY 2010/11 budget.
In an effort to continue to complement the intensive outreach to the chronically mentally ill, it is recommended that the
City again contract with Citrus Health, one of the homeless continuum's most experienced provider of services to the
chronically mentally ill. This relationship with Citrus Health will permit a seamless transition for these hard-to-engage
clients into treatment services..

ANALYSIS
The Homeless Services Division of the Office of Real Estate, Housing, and Community
Development served 1,196 homeless persons in FY 2008/2009. Of these, 218 reported mental
illness and 291 reported chronic addictions (including drugs and alcohol). These chronic conditions
pose impediments to finding and maintaining stable housing.
In 2007, the National Resource and Training Center on Homelessness and Mental Illness reported
that 20 to 39% of the single adult population in the country suffers from some form of severe and
persistent mental illness. The Center reported that the new wave of deinstitutionalization, the denial
of health services, and the unplanned discharge brought on by managed care arrangements may be
contributing to the continued presence of seriously mentally ill persons within the homeless
population.
Currently, the U.S. Department of Health and Human Services, Substance Abuse and Mental Health
Services Administration, reports that about 30% of people who are chronically homeless have
mental health conditions and about 50% have co-occurring substance abuse problems.
Findings by the National Resource Center on Homeless and Mental Illness revealed that persons
with mental and addictive disorders share many of the same treatment needs, including carefully-
designed client engagement and case management strategies, housing solutions and long-term
follow-up and support services. These same findings underscore the importance of service
integration especially between outreach and engagement. According to current outreach and
engagement trends in our City, 18% of engaged clients suffer from mental illness and 24% suffer
from some form of addiction, including alcohol and drugs. The ability to directly access treatment
beds when someone agrees to receive services can impact the ability to engage them in services.

Citrus Health Network, Inc. (hereinafter referred to as "Citrus Health") is a well-known provider of
community mental health services, offering comprehensive programs for homeless individuals,
including the first "safe haven" low-demand emergency housing program in the County. Citrus
Health's services include crisis stabilization, outpatient psychiatric services, day treatment programs,
and transitional and supportive permanent housing programs. It operates 75 transitional housing
beds, and provides more than 510 permanent housing units with supportive services. Its targeted
populations are the mentally ill, the dually diagnosed and especially the chronic mentally ill.
Since late 2004, the City's Homeless Outreach Team has partnered with Citrus Health to provide
specialized outreach to the City's chronic homeless population and engage these chronically
homeless clients in our community who suffer with mental health and/or substance abuse problems.
Citrus Health's Kiva Outreach Team, funded by the Miami-Dade County Homeless Trust, is specially
trained to engage chronically mentally ill homeless individuals into services. Because of the high
concentration of chronically mentally ill in the City's homeless population, in 2006, the Homeless
Trust authorized Citrus Health to provide these specialized outreach services on Miami Beach.
Citrus Health coordinates its outreach activities with our outreach team, ensuring consistency in
approach, more intensive engagement and coordination of services. Due to the nature of mental
illness, the engagement process can often be protracted, requiring constant engagement to form a
trust relationship that can lead to acceptance of services. The Kiva outreach teams work closely with
our outreach teams, and have established relationships with many of our chronic mentally ill
homeless.
Pursuant to Resolution No. 2007-26622, on October 1, 2007, the City entered into a one year
agreement with Citrus Health Network, Inc. for the provision of two (2) medically supported
treatment beds to be used to provide substance abuse and mental health treatment to the City's
homeless population, especially those engaged through the specialized outreach. This will permit
for a seamless transition into treatment with an agency that has assisted in their engagement into
services. The agreement had two additional one-year renewal options, which were both exercised.
The last renewal period will expire on September 30, 2010. The fixed fee for the existing agreement
is $65 per bed, per day, for a maximum of two (2) beds, totaling a maximum of $47,450 annually.
The proposed new five-year agreement will include the same annual cost as was negotiated in
2007. Funding for this program is included in the FY 2010/11 budget.
In an effort to continue to complement the intensive outreach to the chronically mentally ill, it is
recommended that the City again contract with Citrus Health, one of the homeless continuum's most
experienced provider of services to the chronically mentally ill. Continuing our relationship with
Citrus Health will permit a seamless transition for these hard-to-engage clients from the streets into
treatment, especially as many of these homeless persons are known to Citrus Health via their
specialized outreach efforts.
CONCLUSION
In light of the number of chronic homeless mentally ill persons in our City, and the existing
relationship with Citrus Health Network, Inc. to provide specialized outreach to engage these
homeless persons into services, and the benefit of having access to beds to serve the City's chronic
mentally ill population, the Administration recommends that the Mayor and City Commission waive
competitive bidding and authorize the City Manager to enter into an agreement with Citrus Health
Network, Inc., for two treatment beds, in order to better address chronic homelessness in our City.
