AMENDMENT # 001
THIS AMENDMENT, Amendment #001, is entered into between The Children's Trust and The City of Miami
Beach, hereafter referred to as `Provider" and amends the Service Partnership contract #1179 originally
executed on January 25, 2011.
The purpose of this Amendment is to amend the scope of services by adding an additional Type 1 activity which
was not included in the scope. This activity was intended to be included in the original scope of service however,
due to numerous scope_ negotiations it was left out of the final scope.
This Amendment does not affect the dollar amount of Section D Total Funding nor Attachment B: Budget and
Method of Payment.
This Amendment will modify Attachment A- Scope of Services as noted below.
The following modifications are hereby incorporated into the current.contract:
1 §.'Attachment A: Scope of Services, Page 18 Section 4. Performance Measures is hereby amended to add the
additional activity as stated below:
Employment Services - Services will be
provided on an ad hoc basis contingent
upon needs identified through carp' plans
and will be provided to youth aged 16 and
up whose families are in need of additional
income and who are not able to find
unsubsidized employment. Success
University will assist those students whose
legal status is pending in applying for
Individual Taxpayer Identification Numbers.
Partner Provider will supervise youth at all
times and employ them for community .
service type activities such as beach
clean -up, ' waterway restoration, graffiti 25 youth 38
removal, and assistance at Focal festivals.
Employment Js contingent upon students
providing documentation of. school
attendance to employment supervisor.
Employment is temporary and is limited to
no more than 90 hours per person, per

the school year is limited to no more than

Frequency: 45 sessions
Intensity. 2 -4 hours per.session
Duration: 4 -6 months
All provisions in the contract and any attachments thereto in conflict with this Amendment shall be and are
hereby changed, to conform to this contract.
All provisions not in conflict with this Amendment are still in effect and are to be performed at the level specified
in the contract.
