TO THE PROFESSIONAL ARCHITECTURE
AND ENGINEERING SERVICES AGREEMENT"
BETWEEN
THE CITY OF MIAMI BEACH, FLORIDA
AND
ARQUITECTONICA INTERNATIONAL CORP.,

FOR THE PROVISION OF PROFESSIONAL SERVICES,
IN AN AMOUNT NOT -TO- EXCEED $53,500.00
FOR ADDITIONAL THRESHOLD INSPECTION SERVICES FOR THE PURDY
AVENUE GARAGE AND RETAIL PROJECT.
This Amendment No. 2 to the Agreement made and entered this gf day of
J t4 nc 2011 by and between the CITY OF MIAMI BEACH, a Municipal
Corporation existing under the laws of the State of Florida (hereinafter referred to
as CITY), having its principal offices at 1700 Convention Center" Drive, Miami
Beach, Florida 33139 and ARQUITECTONICA - INTERNATIONAL CORP., a
Florida Corporation having its offices at 2900 Oak Avenue, Miami, Florida 33133
(hereinafter, referred to as Consultant).
RECITALS
WHEREAS, as authorized pursuant to Resolution No. 2008- 26969, City and
Consultant are parties to a professional A/E services agreement (Agreement) for
services for the Purdy Avenue ,Garage and Retail Project (the Project); and
WHEREAS, the Agreement provides for the planning, design, bidding / award
construction administration, and additional services for the Project; and
WHEREAS, the Agreement provides for a fee for Consultant in, a not -to-
exceed amount of $414,866.00; and
WHEREAS, on November 23` 2010; Amendment No. 1 to the Agreement
was .executed in the amount of $3,500 for revisions to the completed mechanical
electrical / plumbing'/ Fife- safety drawings to have the life - safety, emergency
lighting powered by the emergency generator (rather_ than have the emergency
garage lighting powered by. battery back -up), similar to other municipal parking -
garages.
WHEREAS, on April 13t ", . 2011, pursuant to Resolution No.. 2011- 27629, the
Mayor and City Commission approved the following Amendment No. 2 to the
Agreement, in the amount of $53,500, for additional construction' administration
services to perform threshold inspection services.

Scope of Services
The scope of work consists of:
Threshold Inspection services to be performed by DDA Engineers, P.A.'
per the Additional Services Proposal issued by Arquitectonica on
2/15/2011 (Attachment..1). The anticipated inspections, hours per
inspection and total inspection - hours .are provided on Attachment 2.
Payment and Compensation
The total fee proposed for this Scope of Work shall be on a not -to- exceed basis,
in the amount of $53,500.00.

Re: Additional Services for Threshold Inspection Services
Dear Mr. Sinnes:
Arquitectonica is pleased to submit this Additional Services Proposal for the owner - requested Threshold
Inspection Services provided by DDA Engineers, PA for the Purdy Avenue Garage and Retail Project.
The additional services as described below are outside the. scope of work identified in the 28 April 2009
A -E Agreement.
Project Description
The Project is a mixed -use building consisting of 29,350 SF of ground floor retail and 141,000 SF of
parking on 4 levels above grade, accommodating a total of 435 parking spaces.
Scope of Services
Arquitectonica will contract DDA Engineers to provide the following scope of work: Inspecting all the
reinforcing steel in the foundation, columns, slabs, reinforced masonry and being present for all concrete
pours. DDA will also ensure that the delegated shoring engineer has inspected the shoring and reshoring
in the field and that he issues a letter attesting to the fact that it has been installed according to his
prepared and sealed shop drawings. The inspections will be provided on an on -call basis and
scheduling of inspections will be done by the contractor.
Professional Fees
The aforementioned services by DDA Engineers will be provided at an hourly rate of Eighty Five dollars
per hour ($85.00 /hr), not to exceed Fifty Thousand dollars ($50,000). DDA /Arquitectonica will notify the
Owner in writing prior to exceeding the budgeted amount.' If the budgeted amount is exceeded
Arquitectonica will prepare a new additional services proposal for Owner's approval prior to continuing
with the work. Fees and reimbursable expenses are to be billed on a monthly basis.
Reimbursable Expenses
Reimbursable expenses, as per the terms of the executed A -E Agreement, will be invoiced at cost, not to
exceed Three Thousand.Five Hundred dollars ($3,500).
Schedule
The schedule of work shall be in line with the General Contractor's construction schedule.
Contract
All terms of the 7 April 2009 A -E Agreement are applicable to this Additional Service.

NOW, THEREFORE the parties hereto, and, in consideration of the mutual
promises, covenants, agreements,' terms, and .conditions .herein contained, and
( other good and valuable consideration„ the respect and adequacy are hereby
acknowledged, do agree as follows:
1. ABOVE RECITALS .
The above recitals are true and correct and are incorporated as a part of
this Amendment No. 2. }
2. MODIFICATIONS
The Agreement is amended to include- the additional scope of services in
Schedule "A -1 ", attached hereto.
3. OTHER PROVISIONS
All other provisions of the Agreement, as amended, are unchanged.
4. RATIFICATION
The City and Consultant ratify the terms of the Agreement, as amended by
this Amendment No. 2.
Its WITNESS WHEREOF, the parties hereto have caused this Amendment No. 2 .
to be executed in their names by their duly authorized officials as of the date first
set forth above.
