Dear Dr Keroff,
In accordance with the Professional Service Agreement between the City of Miami Beach
("City") and you, which was executed on October 1St, 2005, the City is hereby exercising its
option of extending this contract for an additional one-year period commencing on October 1't,
2009,and ending on September 30th,2010.All other terms and conditions of the current service
contract would remain in place through this extension.

As Medical Director of Emergency Medical Services, I agree to continue to provide
medical consulting, training, and advisory services to the Miami Beach Fire
Department Rescue Division for the period commencing on October 1, 2009 through
September 30, 2010, and am willing to extend this agreement with the City at the
same terms and conditions.
