AMENDMENT#001
THIS AMENDMENT, Amendment#001, is entered into between The Children's Trust and The City of Miami
Beach, hereinafter referred to as the"Provider"and amends the Youth Development contract#1476-1090
originally executed on August 14, 2013.
The purpose of this Amendment is to amend the scope of services to account that the outcome measure by All
Stars Youth Survey has been updated and restructured by the Tanglewood Research Company. Due to these
changes, the questions that were stated in original Scope of Services to be measured need to be updated so to
sync with the updated All Stars Youth Survey. The scoring will remain the same as stated on the Scope.
This Amendment does not affect the dollar amount of Section D Total Funding nor Attachment B: Budget and
Method of Payment. This Amendment will modify Attachment A-Scope of Services as noted below
1. Attachment A: Scope of Services, Page 25: Section C. Outcomes Measures, under Data
Source/Measurement Tool in the "Meaningful Improvement" section for all outcome measures hereby
amended to state:
• 85% of youth will increase positive peer relationships-questions 16, 32, 40, 41, 44
• 85% of youth will improve relationships with parents and other adults-questions 12, 14, 20, 23,
27, 34, 36
• 85% of youth will improve decision-making skills-questions 11, 13, 16, 19, 21, 38, 43
All provisions in the contract and any attachments thereto in conflict with this Amendment shall be and are
hereby changed to conform to this contract.
All provisions not in conflict with this Amendment are still in effect and are to be performed at the level specified
in the contract.
This Amendment is hereby made a part of the contract.
IN WITNESS THEREOF, the parties hereto have caused the above referenced Amendments to be executed by
