SUBJECT: Evaluation Committee Relative to "equest for Proposals (RFP) 2017- 002 -JC, for
Emergency Medical Transportation B [ling and Collection Services
The purpose of this LTC is to update the Mayor and City Commission on the status of Request for
Proposals (RFP) 2017 - 002 -JC, which seeks to establish an agreement with a qualified firm(s) for
emergency medical transportation billing and collection services.
The City will receive proposals pursuant to this RFP on Wednesday, March 8, 2017. The responsive
proposals will be reviewed by the Evaluation Committee in accordance with the criteria established
in the RFP.
I am considering appointing the following individuals to serve on the Evaluation Committee:
• Frank Betancourt, Division Chief, Fire Department, City of Miami Beach
• Joseph Florio, Technology Administrator, Fire Department, City of Miami Beach
• - Lazaro Guerra, Communications Manager, Fire Department, City of Miami Beach
• Christine Rogers, Emergency Management Specialist, Emergency Management,
City of Miami Beach
• Jose Del Risco, Assistant Director, Parks & Recreation, City of Miami Beach
I am also considering the following individuals as alternates:
• Georgette Daniels, Financial Analyst III, Department of Emergency Management,
City of Miami Beach
• Matthew Fisikelli, Division Chief, Fire Department, City of Miami Beach
• Ozzy Macias, System Support Manager, Information Technology, City of Miami Beach
