Amendment #001 to the Children's Trust Agreement Zo`c _ 2d13 S� AMENDMENT# 001 THIS AMENDMENT, Amendment # 001, is entered into between The Children's Trust and The City of Miami Beach, hereafter referred to as "Provider" and amends contract #1716-1090 originally executed on July 27, 2016. The purpose of this Amendment is as follows: The reclassification of contracted employees as City of Miami Beach employees, in both budget and staffing plan, to comply with IRS regulations. This Amendment will not modify Section D. Total Funding. This Amendment will modify the Staffing Plan in Attachment A-1: Scope of Services, on pages 32 -37 in original scope of services staffing plan. Amended budget, Attachment B-1, will modify original budget Attachment B, on pages 44-65 in original budget. The "One-to-One Shadows" and "All Star Facilitator" are the positions that have been brought in-house as City Employees, titled "All Stars One-to-One Facilitator". The following modifications are hereby incorporated into the current contract: All provisions in the contract and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform to this contract. All provisions not in conflict with this Amendment are still in effect and are to be performed at the level specified in the contract. PROGRAM DESCRIPTION City of Miami Beach is implementing the Miami Beach All Stars Program.The program serves 100 low-income youth living in high need neighborhoods, who attend 6th, 7th, and 8th grade at Nautilus Middle School, and whose families face daily economic, social and familial challenges. Miami Beach All Stars Program incorporates Life Skills activities and academic support programming year-round and offers STEM and enrichment activities that include Art/Theatre and Sports. All Stars Curriculum is an interactive, evidence- based program that consists of intensive, thirteen 1-hour group sessions delivered 2 times per week and complementary life-skills elements of All Stars are infused in subsequent activities that occur throughout the school year. In addition to academic support and life skills activities youth participating in the program can select from the following enrichment activities based on their interests: STEM programming and competitions; Theatre/Drama workshops and performances; and SUPER (Sports United to Promote Education and Recreation), a team-sports-based program, for basketball and flag football to learn the sport as well as life skills related to the activity. Additionally,youth participate in service learning projects that focus on fostering positive social change by using creativity, knowledge, and life experiences to address issues that affect them: education equity, and school and community climate. The goal of the City's Miami Beach All Stars program is to equip youth with social, emotional and cognitive skills that promote academic success, healthy child development, and healthy successful families. la. Live in high need neighborhoods; 2. Attend 6th, 7th, and 8th grade in Miami Beach Title I School that receive financial assistance due to high numbers or high percentages of children from low-income families. Recruitment: • The City's All Star Program successfully utilizes word-of-mouth as its primary recruitment tool. Youth who participate in All Stars consistently refer peers who may benefit from the program. • Miami Beach All Stars participants are recruited via our integrated network of providers which includes our feeder pattern middle schools as well as street-level engagement in parks, City youth recreation facilities, PTSA meeting presentations and community events as well as the harnessing the marketing reach of our City's website and print/electronic media. • The program also anticipates receiving referrals from the City's Children's Trust-funded youth and family programs. Enrollment Process: Interested youth are screened for eligibility criteria. Parents and youth are provided with an enrollment packet that includes the following tools: 1.
