SUBJECT: FLORIDA LEAGUE OF CITIES MUNICIPAL SELF- INSURED
WORKER'S COMPENSATION CONTRACT

Services , Inc. , representing the Florida League of Cities Municipal Self- Insured
Worker's Compensation Fund, to handle the City of Miami Beach's Worker's

company thirty (30) days prior written notice of cancellation. The amount of
funds required to renew the contract is as follows:
A 25% deposit of the estimated yearly premium in the amount of $186, 124
and the monthly premium for November 1, December 1 and January 1 of $50,761 . 37
respectively, as needed.
If the present company is not awarded the new bid, they will return the pro rated
unused share of premium advanced to them.
