A RESOLUTION OF THE MAYOR AND CITY COMMISSION
OF THE CITY OF MIAMI BEACH, FLORIDA, APPROVING
THE AWARD TO AMERICAN DENTAL PLAN, AS THE
NUMBER-ONE RANKED PROPOSER, PURSUANT TO
REQUEST FOR PROPOSAL NUMBER 19-96/99, TO
PROVIDE GROUP MANAGED CARE/INDEMNITY DENTAL
INSURANCE FOR EMPLOYEES AND RETIREES; SAID
SERVICES COMMENCING NO LATER THAN JULY 1, 1997.
WHEREAS, the City is seeking to provide group managed care/indemnity dental insur. ime
for its employees and retirees; and
WHEREAS, the City issued a Request for Proposals (RFP No. 19-96/99) for provi ]ir g
group managed care/indemnity dental insurance for employees and retirees, on November 5, 1
and
WHEREAS, the Office of Risk Management, the Group Insurance Board, and the Hc, all h
Advisory Committee evaluated the proposals received and selected the firm of American Dental Pl~ n
as the top-ranked proposer; and
WHEREAS, on March 19, 1997, the Administration was authorized to ~nalize an agreer nel: tt
and negotiate outstanding terms with American Dental Plan; and
WHEREAS, the Group Insurance Board and the Health Advisory Committee have als a
discussed their respective concerns with American Dental Plan regarding the proposed servio,'s.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE MAYOR AND C]![T' i(
COMMISSION OF THE CITY OF MIAMI BEACH, FLORIDA, that the Mayor and Cil y
Commission herein approve the award to American Dental Plan, as the number-one ranked propose ',
pursuant to Request for Proposal No. 19-96/99, to provide group managed care/indemnity dent~ ..1
insurance for employees and retirees; said services commencing no later than July 1, 1997.

ADMINISTRATION RECOMMENDATION:
To approve the Resolution.
CONTRACT AMOUNT AND FUNDING:
$666,405. (Annual Estimate) Premium contributions will be split 50/50 betwee'a
employee/retiree deductions and City contributions. City contribution fundir g s
available from Budget Accounts of various departments, subject to ORB approva ..
This estimate is based on the existing number of employees and retirees enrolled i:a
each of the plans and based on a two-tier rate structure.
BACKGROUND:
On March 19, 1997, Commission Memorandum 135-97, the Administration was authorized 13
finalize an agreement with American Dental Plan, the top-ranked proposer for RFP No. 19-96/9
for group managed care/indemnity dental insurance program.
ANALYSIS:
On March 20, 1997, the Group Insurance Board met with American Dental Plan to discuss se~ en .1
administrative and contract concerns. As a result of the meeting, American Dental Plan agreed 13
extend the eligibility age for dependent children until 25 years of age; extend the rate guara~te e
period for the indemnity plan until October 1, 1998; and allow all family members under tke
managed care plan to have different participating dentists under the plan.
On April 7, 1997, the Health Advisory Committee and Group Insurance Board had a joint meetin g
and recommended the approval for American Dental Plan to provide the group man~,ge]
care/indemnity dental insurance coverage.
The Group Insurance Board is comprised of the presidents of the American Federation of S'mt ,,
County and Municipal Employees (AFSCME), and the Communications Workirs of America.

(CWA), a retiree representative and three Administration representatives. The Health Advisol~y
Committee is a fourteen member City board comprised of individuals from the community who a'e
involved with the health industry.
CONCLUSION:
The Administration recommends that the Mayor and City Commission approve the attached
Resolution approving the award to American Dental Plan to provide group managed care/indemni,y
dental plan.
