go (7- 30000 AGREEMENT BETWEEN THE CITY OF MIAMI BEACH, FLORIDA AND CAMILLUS HOUSE INC FOR EMERGENCY SHELTER SERVICES THIS AGREEMENT made and entered into this 9'14 04} � ' C , by and between the CITY OF MIAMI BEACH, FLORIDA, a Florida municipal corporation (hereinafter referred to as City), having its principal offices at 1700 Convention Center Drive, Miami Beach, Florida, 33139, and CAMILLUS HOUSE INC., a Florida corporation (hereinafter referred to as Contractor), whose address is 1603 NW 7T" Avenue, Miami, Florida, 33136. SECTION 1 1.1 DEFINITIONS Agreement: This Agreement between the City and Contractor, and any exhibits and/or attachments hereto. City Manager: The Chief Administrative Officer of the City. Contractor: For the purposes of this Agreement, Contractor shall be deemed to be an independent contractor, and not an agent or employee of the City. Services: All services, work and actions by the Contractor performed pursuant to or undertaken under this Agreement, as described in Section 2 and Exhibit "A" hereto. Fee (Compensation): Amount paid to the Contractor to cover the costs of the Services. Risk Manager: The Risk Manager of the City, with offices at 1700 Convention Center Drive, Third Floor, Miami Beach, Florida 33139, telephone number (305) 673-7000, Ext. 6435, and fax number (305) 673-7023. City of Miami Beach-Camillus House Page 1 SECTION 2 SCOPE OF WORK(SERVICES) The scope of work to be performed by Contractor is set forth in Exhibit "A," and "B", each entitled "Scope of Services" and "Service Deliverables". SECTION 3 COMPENSATION 3.1 FIXED FEE For the term of this Agreement, the City agrees to pay a fixed fee of $26.35 per bed, per day, for a minimum of twelve (12) beds, in an amount not to exceed One Hundred Ninety-Eight Thousand One Hundred Twenty-Five Dollars and Sixty-Five Cents ($198,125.65) per year for Year One of this Agreement, to be used by Contractor to provide Emergency Shelter Services (such Services as set forth in Exhibit "A" and "B" hereto). In Year Two, the fixed fee shall be $27.14 per bed, per day, fora minimum of Twelve (12) beds, in an amount not to exceed Two Hundred Four Thousand Sixty-Nine Dollars and Forty-Two Cents ($204,069.42). In Year Three, the fixed fee shall be $27.95 per bed, per day, for a minimum of twelve (12) beds, in an amount not to exceed Two Hundred Ten Thousand One Hundred Ninety-One Dollars and Fifty-One Cents ($210,191.51). In Year Four, the fixed fee shall be $28.79 per bed, per day, for a minimum of twelve (12) beds, in an amount not to exceed Two Hundred Sixteen Thousand Four Hundred Ninety-Seven Dollars and Twenty-Five Cents ($216,497.25). In Year Five, the fixed fee shall be $29.66 per bed, per day, for a minimum of twelve (12) beds, in an amount not to exceed Two Hundred Twenty-Two Thousand Nine Hundred Ninety-Two Dollars and Sixteen Cents ($222,992.16). 3.2 INVOICING Contractor shall submit a monthly invoice, which includes a description of the Services provided including the number of beds provided and clients served. 3.3 METHOD OF PAYMENT Payments shall be made within forty-five (45) days of the date of invoice, in a manner satisfactory to and as approved and received by the City. Contractor shall mail all invoices to: City of Miami Beach Office of Housing & Community Services Attention: Maria L. Ruiz 1700 Convention Center Drive Miami Beach, Florida 33139 City of Miami Beach-Camillus House Page 2 SECTION 4 GENERAL PROVISIONS 4.1 RESPONSIBILITY OF THE CONTRACTOR With respect to the performance of the Services, the Contractor shall exercise that degree of skill, care, efficiency and diligence normally exercised by recognized professionals with respect to the performance of comparable services. In its performance of the Services, the Contractor shall comply with all applicable laws, ordinances, and regulations of the City, Miami-Dade County, the State of Florida, and the federal government, as applicable. 4.2 DURATION AND EXTENT OF AGREEMENT (TERM) The term of this Agreement shall commence upon execution of this Agreement by all parties hereto, as set forth in the Effective Date on page 1 hereof, and shall continue, for five (5) years, unless otherwise terminated by the City. The terms of this agreement can be extended for an additional two (2), one (1) year terms with the approval of the City Manager. 4.3 TIME OF COMMENCEMENT OF SERVICES The Services to be rendered by the Contractor shall be commenced upon receipt of a written Notice to Proceed from the City subsequent to execution of the Agreement by the parties. 4.4 INDEMNIFICATION Contractor agrees to indemnify and hold harmless the City of Miami Beach and its officers, employees and agents, from and against any and all actions, claims, liabilities, losses, and expenses, including, but not limited to, attorneys' fees, for personal, economic or bodily injury, wrongful death, loss of or damage to property, at law or in equity, which may arise or be alleged to have arisen from the negligent acts, errors, omissions or other wrongful conduct of the Contractor, its employees, agents, sub-consultants, or any other person or entity acting under Consultant's control, in connection with the Contractor's performance of the Services pursuant to this Agreement; and to that extent, the Contractor shall pay all such claims and losses and shall pay all such costs and judgments which may issue from any lawsuit arising from such claims and losses, and shall pay all costs and attorneys' fees expended by the City in the defense of such claims and losses, including appeals. The Contractor's obligation under this Subsection shall not include the obligation to indemnify the City of Miami Beach and its officers, employees and agents, from and against any actions or claims which arise or are alleged to have arisen from the grossly negligent or wrongful act or omission of the City and its officers, employees City of Miami Beach-Camillus House Page 3 and agents. The parties each agree to give the other party prompt notice of any claim coming to its knowledge that in any way directly or indirectly affects the other party. 4.5 TERMINATION, SUSPENSION AND SANCTIONS 4.5.1 Termination for Cause If the Contractor shall fail to fulfill in a timely manner, or otherwise violate any of the covenants, agreements, or stipulations material to this Agreement, the City shall thereupon have the right to terminate the Services then remaining to be performed. Prior to exercising its option to terminate for cause, the City shall notify the Contractor of its violation of the particular terms of this Agreement and shall grant Contractor seven (7) days to cure such default. If such default remains uncured after seven (7) days, the City, upon three (3) days' notice to Contractor, may terminate this Agreement and the City shall be fully discharged from any and all liabilities, duties and terms arising out of/or by virtue of this Agreement. Notwithstanding the foregoing, if the default is of a nature that cannot be cured, such as perpetrating a fraud or making a material misrepresentation in connection with the performance of this Agreement, the termination shall be effective upon receipt of the termination notice and no cure period shall apply. Notwithstanding the above, the Contractor shall not be relieved of liability to the City for damages sustained by the City by any breach of the Agreement by the Contractor. The City, at its sole option and discretion, shall additionally be entitled to bring any and all legal/equitable actions that it deems to be in its best interest in order to enforce the City's right and remedies against the defaulting party. The City shall be entitled to recover all costs of such actions, including reasonable attorneys' fees. To the extent allowed by law, the defaulting party waives its right to jury trial and its right to bring permissive counter claims against the City in any such action. 4.5.2 Termination for Convenience of City NOTWITHSTANDING SECTION 4.5.1, THE CITY MAY ALSO, FOR ITS CONVENIENCE AND WITHOUT CAUSE, TERMINATE THIS AGREEMENT AT ANY TIME DURING THE TERM HEREOF BY GIVING WRITTEN NOTICE TO CONTRACTOR OF SUCH TERMINATION, WHICH SHALL BECOME EFFECTIVE THIRTY (30) DAYS FOLLOWING RECEIPT BY THE CONSTRACTOR OF THE WRITTEN TERMINATION NOTICE. IN THAT EVENT, ANY FINISHED OR UNFINISHED DOCUMENTS AND OTHER MATERIALS PREPARED AND OR OTHERWISE COMPILED BY CONTRACTOR PURSUANT TO ITS PROVISION OF THE SERVICES City of Miami Beach-Camillus House Page 4 CONTEMPLATED IN SECTION 2, SHALL BE PROMPTLY ASSEMBLED AND DELIVERED TO THE CITY, AT CONTRACTOR SOLE COST AND EXPENSE. IF THE AGREEMENT IS TERMINATED BY THE CITY AS PROVIDED IN THIS SUBSECTION, CONTRACTOR SHALL BE PAID FOR ANY SERVICES SATISFACTORILY PERFORMED, AS DETERMINED BY THE CITY AT ITS SOLE DISCERTION, UP TO THE DATE OF TERMINATION; PROVIDED, HOWEVER, THAT AS A CONDITION PRECEDENT TO SUCH PAYMENT, CONTRACTOR SHALL HAVE DELIVERED ANY AND ALL DOCUMENTS, MATERIALS, ETC, TO CITY, AS REQUIRED HEREIN. 4.5.3 Termination for Insolvency The City also reserves the right to terminate the remaining Services to be performed in the event the Contractor is placed either in voluntary or involuntary bankruptcy or makes an assignment for the benefit of creditors. In such event, the right and obligations for the parties shall be the same as provided for in Section 4.6.2. 4.5.4 TERMINATION FOR LACK OF FUNDS It is expressly understood and agreed by the parties hereto that monies to be used by Contractor, as contemplated by this Agreement, may originate from City of Miami Beach Resort Tax, General Fund and/or Community Development Block Grant (CDBG) funds from the US Department of Housing and Urban Development (HUD) and must be implemented in full compliance with all of HUD's rules and regulations. It is expressly understood and agreed that in the event of curtailment or non-production of said federal grant funds, the financial sources necessary to continue to pay the Contractor all or any portions of the funds contemplated herein will not be available, and that this Agreement will thereby terminate effective as of the time that it is determined by the City, in its sole discretion and judgment, that said funds are no longer available. In the event of such determination, the Contractor agrees that it will not look to, nor seek to hold liable, the City nor any individual member of the City Commission and/or City Administration thereof personally for the performance of this Agreement and all of the parties hereto shall be released from further liability each to the other under the terms of this Agreement. In the event of such termination, the rights and obligations for the parties shall be the same as provided for in Section 4.5.2. 4.5.5 Sanctions for Noncompliance with Nondiscrimination Provisions In the event of the Contractor's noncompliance with the nondiscrimination provisions of this Agreement, as applicable, the City shall impose such sanctions as the City or the State of Florida may determine to be appropriate, including but not limited to, withholding of payments to the Contractor under the Agreement until the Contractor complies and/or cancellation, termination or City of Miami Beach-Camillus House Page 5 suspension of the Services and/or the Agreement. In the event the City cancels or terminates the Services and/or the Agreement pursuant to this Subsection the rights and obligations of the parties shall be the same as provided in Section 4.5.2. 4.6 CHANGES AND ADDITIONS Any changes and additions to the terms of this Agreement shall be by a written amendment, signed by the duly authorized representatives of the City and Contractor. No alteration, change, or modification of the terms of this Agreement shall be valid unless amended in writing, signed by the parties hereto, and approved by the City. 4.7 RELIGIOUS ORGANIZATION OR OWNED PROPERTY As applicable, Emergency Shelter Grant funds may be used by religious organizations or on property owned by religious organizations only with the prior written approval from the City and only in accordance with requirements set in 24 CFR 576.23. 4.8 COMPLIANCE WITH LOCAL, STATE, AND FEDERAL REGULATIONS The Contractor agrees to comply with all applicable federal regulations as they may apply to program administration and to carry out each activity in compliance with the laws and regulations as described in 24 CFR 576, as same may be amended from time to time. Additionally, the Contractor will comply with all state and local (City and County) laws and ordinances hereto applicable. It shall be Contractor's sole and absolute responsibility to continually familiarize itself with any and all applicable federal regulations, as well as any and all applicable state and local laws and ordinances. 4.9 CONFORMITY TO HUD REGULATIONS Section Deleted. 4.10 AUDIT AND INSPECTIONS With 24-hour written notice, the City and/or such representatives as the City may deem to act on its behalf, may, during normal business hours, audit, examine and make audits of all contracts, invoices, materials, payrolls, records of personnel, conditions of employment and other data relating to all matters covered by this Agreement. Contractor shall maintain any and all records necessary to document compliance with the provisions of this Agreement. City of Miami Beach-Camillus House Page 6 4.11 ASSIGNMENT, TRANSFER OR SUBCONTRACTING The Contractor shall not subcontract, assign, or transfer any work under this Agreement without the prior written consent of the City. 4.12 SUB-CONTRACTORS The Contractor shall be liable for the Contractor's services, responsibilities and liabilities under this Agreement and the services, responsibilities and liabilities of sub-contractors, and any other person or entity acting under the direction or controls of the Contractor. When the term "Contractor" is used in this Agreement, it shall be deemed to include any sub-contractors and any other person or entity acting under the direction or control of Contractor. All sub-contractors must be approved of in writing prior to their engagement by Contractor. 4.13 EQUAL EMPLOYMENT OPPORTUNITY In connection with the performance of this Agreement, and only in accordance with applicable federal law, the Contractor shall not discriminate against any employee or applicant for employment because of race, color, religion, ancestry, sex, intersexuality, gender identity, sexual orientation, age, and national origin, place of birth, marital or familial status, or disability. The Contractor shall take affirmative action to ensure that applicants are employed and that employees are treated during their employment without regard to their race, color, religion, ancestry, sex, intersexuality, gender identity, sexual orientation, age, national origin, place of birth, marital or familial status, or disability, as applicable. Such action shall include, but not be limited to the following: employment, upgrading, demotion, or termination; recruitment or recruitment advertising; layoff or termination; rates of pay, or other forms of compensation; and selection for training, including apprenticeship. 4.14 CONFLICT OF INTEREST The Contractor agrees to adhere to and be governed by the Metropolitan Miami-Dade County Conflict of Interest Ordinance (No. 72-82), as amended; and by the City of Miami Beach Charter and Code, which are incorporated by reference herein as if fully set forth herein, in connection with the Agreement conditions hereunder. The Contractor covenants that it presently has no interest and shall not acquire any interest, direct or indirectly which should conflict in any manner or degree with the performance of the Services. The Contractor further covenants that in the performance of this Agreement, no person having any such interest shall knowingly be employed by the Contractor. No member of City of Miami Beach-Camillus House Page 7 or delegate to the Congress of the United States shall be admitted to any share or part of this Agreement or to any benefits arising there from. 4.15 PATENT RIGHTS; COPYRIGHTS; CONFIDENTIAL FINDINGS Any patentable result arising out of this Agreement, as well as all information, design specifications, processes, data and findings, shall be made available to the City for public use. No reports, other documents, articles or devices produced in whole or in part under this Agreement shall be the subject of any application for copyright or patent by or on behalf of the Contractor or its employees or subcontractors. 4.16 NOTICES All notices and communications relating to the day-to-day activities shall be exchanged between the Project Manager appointed by the Contractor and the Program Coordinator designated by the City. The Contractor's Project Manager and the City's Program Coordinator shall be designated promptly upon commencement of services. All other notices and communications in writing required or permitted hereunder may be delivered personally to the representatives of the Contractor and the City listed below or may be mailed by registered mail. Ruiz, Director City of Miami Beach City of Miami Beach-Camillus House Page 8 1700 Convention Center Drive Miami Beach, Florida 33139 4.17 LITIGATION JURISDICTION VENUE This Agreement shall be governed by, and construed in accordance with, the laws of the State of Florida, both substantive and remedial, without regard to principles of conflict of laws. The exclusive venue for any litigation arising out of the Agreement shall be Miami-Dade County, Florida, if in State court, and the U.S. District Court, Southern District of Florida, if in federal court. BY ENTERING INTO THIS AGREEMENT, CONTRACTOR AND CITY EXPRESSLY WAIVE ANY RIGHTS EITHER PARTY MAY HAVE TO A TRIAL BY JURY OR ANY CIVIL LITIGATION RELATED TO, OR ARISING OUT OF, THIS AGREEMENT. 4.18 ENTIRETY OF AGREEMENT This writing and any exhibits and/or attachments incorporated (and/or otherwise referenced for incorporation) herein embody the entire Agreement and understanding between the parties hereto, and there are no other agreements and understandings, oral or written, with reference to the subject matter hereof that are not merged herein and superseded hereby. 4.19 INSURANCE REQUIREMENTS The Contractor shall not commence any work and/or Services pursuant to this Agreement until all insurance required under this Section has been obtained and such insurance has been reviewed and approved by the City's Risk Manager. Contractor shall maintain and carry in full force during the term of this Agreement the following insurance: 1. Contractor General Liability, in the amount of $1,000,000, naming the City of Miami Beach, Florida as an additional insured. 2. Workers Compensation & Employers Liability, as required pursuant to Florida Statutes. All insurance required hereunder must be furnished by insurance companies authorized to do business in the State of Florida. Original certificates of insurance for the above coverage must be submitted to the City's Risk Manager at the Office of the Risk Manager of the City of Miami Beach, 1700 Convention Center Drive, Miami Beach, Florida 33139. City of Miami Beach-Camillus House Page 9 The Contractor is solely responsible for obtaining and submitting all insurance certificates for its sub-contractors. All insurance policies must be issued by companies authorized to do business under the laws of the State of Florida. The companies must be rated no less than "B+" as to management and not less than "Class VI" as to strength by the latest edition of Best's Insurance Guide, published by A.M. Best Company, Oldwick, New Jersey, or its equivalent. Compliance with the foregoing requirements shall not relieve the Contractor of the liabilities and obligations under this Section or under any other portion of this Agreement, and the City shall have the right to obtain from the Contractor specimen copies of the insurance policies in the event that submitted certificates of insurance are inadequate to ascertain compliance with required overage. All of Contractor's certificates, as required in this Section 4.19 (1), shall contain endorsements providing that written notice shall be given to the City at least thirty (30) days prior to termination, cancellation or reduction in coverage in the policy. The insurance certificate for the General Liability shall reflect that the City is an additional insured. The Contractor shall not commence any work and/or Services pursuant to this Agreement until the City's Risk Manager has received, reviewed and approved, in writing, certificates of insurance showing that the requirements of this Section (in its entirety) have been met and provided for. 4.20 LIMITATION OF CITY'S LIABILITY The City desires to enter into this Agreement only if in so doing the City can place a limit on the City's liability for any cause of action for money damages due to an alleged breach by the City of this Agreement, so that its liability for any such breach never exceeds the sum of $1,000. Contractor hereby expresses its willingness to enter into this Agreement with Consultant's recovery from the City for any damage action for breach of contract to be limited to a maximum amount of$1,000. Accordingly, and notwithstanding any other term or condition of this Agreement, Contractor hereby agrees that the City shall not be liable to the Contractor for damages in an amount in excess of $1,000 for any action or claim for breach of contract arising out of the performance or non- performance of any obligations imposed upon the City by this Agreement. Nothing contained in this paragraph or elsewhere in this Agreement is in any way intended to be a waiver of the limitation placed upon the City's liability as set forth in Section 768.28, Florida Statutes. City of Miami Beach-Camillus House Page 10 4.21 CONSULTANT'S COMPLIANCE WITH FLORIDA PUBLIC RECORDS LAW (A) Contractor shall comply with Florida Public Records law under Chapter 119, Florida Statutes, as may be amended from time to time. (B) The term "public records" shall have the meaning set forth in Section 119.011(12), which means all documents, papers, letters, maps, books, tapes, photographs, films, sound recordings, data processing software, or other material, regardless of the physical form, characteristics, or means of transmission, made or received pursuant to law or ordinance or in connection with the transaction of official business of the City. (C) Pursuant to Section 119.0701 of the Florida Statutes, if the Contractor meets the definition of "Contractor" as defined in Section 119.0701(1)(a), the Contractor shall: (1) Keep and maintain public records required by the City to perform the service; (2) Upon request from the City's custodian of public records, provide the City with a copy of the requested records or allow the records to be inspected or copied within a reasonable time at a cost that does not exceed the cost provided in Chapter 119, Florida Statutes or as otherwise provided by law; (3) Ensure that public records that are exempt or confidential and exempt from public records disclosure requirements are not disclosed, except as authorized by law, for the duration of the contract term and following completion of the Agreement if the Contractor does not transfer the records to the City; (4) Upon completion of the Agreement, transfer, at no cost to the City, all public records in possession of the Contractor or keep and maintain public records required by the City to perform the service. If the Contractor transfers all public records to the City upon completion of the Agreement, the Contractor shall destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. If the Contractor keeps and maintains public records upon completion of the Agreement, the Contractor shall meet all applicable requirements for retaining public records. All records stored electronically must be provided to the City, upon request from the City's custodian of public records, in a format that is compatible with the information technology systems of the City. (C) REQUEST FOR RECORDS; NONCOMPLIANCE. (1) A request to inspect or copy public records relating to the City's contract for services must be made directly to the City. If the City does not possess the requested records, the City shall immediately notify the Contractor of the request, and the Contractor must City of Miami Beach-Camillus House Page 11 provide the records to the City or allow the records to be inspected or copied within a reasonable time. (2) Contractor's failure to comply with the City's request for records shall constitute a breach of the Agreement, and the City, at its sole discretion, may: (1) unilaterally terminate the Agreement; (2) avail itself of the remedies set forth under the Agreement; and/or (3) avail itself of any available remedies at law or in equity. (3) A Contractor who fails to provide the public records to the City within a reasonable time may be subject to penalties under s. 119.10. (D) CIVIL ACTION. (1) If a civil action is filed against a Contractor to compel production of public records relating to the City's contract for services, the court shall assess and award against the Contractor the reasonable costs of enforcement, including reasonable attorney fees, if: a. The court determines that the Contractor unlawfully refused to comply with the public records request within a reasonable time; and b. At least eight (8) business days before filing the action, the plaintiff provided written notice of the public records request, including a statement that the Contractor has not complied with the request, to the City and to the Contractor. (2) A notice complies with subparagraph (1)(b) if it is sent to the City's custodian of public records and to the Contractor at the Contractor's address listed on its contract with the City or to the Contractor's registered agent. Such notices must be sent by common carrier delivery service or by registered, Global Express Guaranteed, or certified mail, with postage or shipping paid by the sender and with evidence of delivery, which may be in an electronic format. (3) A Contractor who complies with a public records request within eight (8) business days after the notice is sent is not liable for the reasonable costs of enforcement. The City adheres to a strengths- based client engagement strategy that seeks to utilize a client's strengths to devise plans that incorporate these natural strengths to overcome existing service gaps or personal barriers. We seek to adhere to cultural competency standards and engage the client at his/her level of comprehension. In order to achieve this and ensure that finite public resources are used efficiently, the City will expect the following: Intake & Consent — The Intake provides prospective clients with an overview of shelter services and the benefits and expectations of program participation. Client consents required for program participation are executed by the City enabling the provision of further services by the City and Contractor. Assessment — An assessment documents the natural supports and needs of the client and his/her family. The assessment also indicates the client's number in the Homeless Management Information System (HMIS). Assessments must meet the following criteria: • Information collected is documented completely and accurately • Adherence to grammar and spelling rules for the English language City of Miami Beach-Camillus House Page 14 • A sincere and thorough effort to understand the client, his/her family, and their respective strengths and needs • Effective engagement to ensure that the client and his/her family are informed regarding program services and expectations as well as the means/process by which to access additional services • All client and family needs have been identified and subsequently addressed via Referral Care Plan — The Care Plan outlines the specific strategies and steps to be taken by the client to address personal and family stability and the subsequent goal of obtaining permanent housing. The plan identifies the issues to be addressed and the respective community-based agencies or resources that will be harnessed in response. The Care Plan should evolve to guide clients to permanent housing and independent living. Care Plan Contact/Progress Update — A Care Plan Contact provides an opportunity to monitor the client's progress towards the goals identified via the Care Plan as well as to identify any new concerns or issues that require address through referral. A Care Plan Contact is conducted in person. The contact will document following minimum criteria: • Progress on the Care Plan is reviewed and documented • The identification of any additional needs or issues affecting school attendance were discussed and, when appropriate, referrals were subsequently issued Care Plan Contacts are to take place within the following timeframes: • Care Plan Contact#1: Three business (3) days of shelter placement • Care Plan Contact#2 and forward: Weekly Care Plan Close-Out— The Care Plan Close-Out documents the client's successful completion of all items and strategies identified in the Care Plan. Conversely, failure to complete the Care Plan should trigger a referral to an alternate program or exploration of alternate options for the client including termination. Clients are expected to demonstrate ongoing progress towards independence as a condition of program participation. A Care Plan Close-Out is conducted in person with the client. The contact will document following minimum criteria: • Completion of the Care Plan is reviewed and documented • Documentation of client destination (i.e. independent housing, supportive housing, other shelter, etc. Client Termination — The Contractor may terminate clients for failure to adhere to Care Plan goals and timelines as well as failure to adhere to shelter rules. The Contractor will notify the City in writing of any terminations and the reasons for such termination. City of Miami Beach-Camillus House Page 15 Initial Care Plan — The City will provide an Initial Care Plan as part of the referral and placement process to shelter. This Plan, which the client consented to as a condition of accepting shelter, will identify the initial actions/goals a client must achieve to maintain shelter. Typically, this will include: • Adherence to shelter rules (including TB test) • Obligatory meetings with the on-site, assigned care manager • Application for eligible entitlements (including phone service) • Application for replacement identification documents (if applicable) • Sobriety supports (for those clients experiencing addiction) • Income generation The Initial Care Plan establishes the immediate, actionable steps that a client must take towards independence. These steps each have deadlines for completion. If a client fails to meet a deadline and the cause for this failure is his/her inaction, the client is subject to termination from the program. It should be underscored that the terms of shelter acceptance were stipulated in the Initial Care Plan. Any deviation or performance failure on the client's part is, therefore, a rejection of the shelter offer. Typically, the Initial Care Plan will provide tasks that keep the client engaged to outcomes-designed goals until the first full meeting with his/her assigned on-site care manager. The Contractor will grow and expand the Care Plan to reflect the evolving, emerging needs of the client as he/she makes progress towards independence. Referrals — Referrals address the identification of a specific client need and the subsequent identification of a community-based resource and/or strategy to address the need. Referrals must be provided for all service needs recognized in the intake and assessment and subsequent Care Plan Progress or client-initiated requests. Referrals must be issued with the following client-focused interests in mind: • Is the referral appropriate within the scope of the client's Care Plan? • Does the referral address the need identified? • Can the client access the services being sought via the referral? • Is the agency provided in the referral the most accessible, most appropriate provider for the services needed? • Is the referral culturally appropriate? • Does the client consent to receiving the referral? These referrals must be documented and included within the client's Care Plan. City of Miami Beach-Camillus House Page 16 EXHIBIT "B" Service Deliverables Services must be delivered as follows: Care Plan Development While the Initial Care Plan gets a client to the shelter as a condition of accepting services, it will be the shelter provider's responsibility to amend the Care Plan to reflect the necessary, progressive steps leading to the client's eventual (and hopefully expeditious) transition to independent, permanent housing. This evolving Care Plan will address specific strategies and timelines to achieve the following: • Permanent, sustained source of income (enabling self-sustenance) • Permanent, sustainable housing (suitable and appropriate to client needs and income stability) • Natural supports (individual- and community-level engagements to eliminate social isolation and provide barriers to recidivism and a return to homelessness) • Medical supports (for those with either physical or medical conditions that can pose a risk factor for homelessness) • Sobriety supports (for those with addiction issues) such as Alcoholic Anonymous, etc. • Legal status advocacy (for those without established legal status; this may include the obtaining of a tax identification number along with a correlating, realistic action plan to meet all of the supports listed previously) • Adult education opportunities, such as General Education Development, English as a second language, and or technical education/certification, etc. • Community integration efforts, recreational activities within the community, church and faith support, volunteer opportunities, etc. The Care Plan is a written document expected by the client that specifically delineates the goals identified and expected and the specific strategies (referrals/actions/achievements) for the fulfillment of these goals. More so, the Care Plan delineates the specific timeframes by which each interim goal or strategy is expected to be fulfilled as well as the means for their achievement. Professional Supports must be Natural Supports should remain culturally competent,accessible and even after the client exits the appropriate Continuum of Care The Care Plan provides the framework for expectations, timelines, resources and outcomes. The Care Plan should be a vibrant document that establishes accountability and serves as the record of all progress, services and interventions applied. Sometimes, despite best efforts, goals are not achieved. In those cases in which client inaction or deliberate action were the cause for failure, the client is subject to program termination. In those cases in which best efforts adhered to the strategies proscribed but did not achieve outcome, the in-house care manager has the discretion to authorize additional time for goal achievement or reconsider the viability of the goal and suggest an alternative. Such decisions should be based on the facts at hand and the client's overall adherence to the Care Plan and natural resources. The Care Plan is an evolving, progressive document and should be amended as frequently as necessary to drive the client to the ultimate goal of independence. Goals that are too broadly defined should be avoided as should also a fixed chronological and universal application of expectations. Care Plans should reflect. the client's individual strengths and assets with a conscious recognition and respect for the availability of resources and the client's cultural and physical identity. There is no such thing as one Care Plan fits all. Care Plans are expected to be the product of City of Miami Beach-Camillus House Page 19 client assessment, realistic client expectations and the realistic incorporation of interventions and resources. Engagement Strategies The Contractor will adhere to the following engagement strategies in the delivery of services: • Advise the client of the scheduled weekly Care Plan meeting and its requirements and timelines. • Provide client information regarding the Care Coordination process and other available services. • Reintroduce the services available and the Care Plan goals during each client interaction. • Document every interaction with or on behalf client via the Care Plan Contact/Progress Report. • Meet with Homeless Outreach Program Coordinator or his/her designee on a monthly basis to discuss client progress. Discharge Planning Discharge typically results under two conditions: 1. The client has failed to adhere to his/her Care Plan and is being terminated for cause; or 2. The client is ready to be transitioned to an alternate housing environment (which may include relocation; an alternate shelter; transitional, supportive and permanent housing). Clients whose cause for failure cannot be clearly ascertained should be placed on notice and trigger an interventional action. Such notice should include immediate intervening steps (Care Plan amendment) that the client is required to take in order to maintain his/her program enrollment. As with all Care Plan items, specific strategies and benchmarks (including timelines) must be included. Clients who willfully fail adherence to their Care Plans should be discharged as soon as they violate Care Plan requirements. Clients who are being terminated for cause should be notified in writing as to the reason(s) for termination. Such notice should be forwarded to the City upon issuance. Additional Client Services The City expects that the following services to be provided, as appropriate: 1. Testing for tuberculosis within forty-eight (48) business hours of admittance to shelter. City of Miami Beach-Camillus House Page 20 2. Case Management staff will be bilingual and should have some social service experience working with homeless populations and/or preferably a Bachelor of Arts degree in Social Work. 3. Clients will receive case management services from the City with an emphasis on obtaining stability, entitlements, and housing, this includes: a. Development of a Care Plan and documentation of contacts; b. Referrals for specialized health, substance abuse, mental health screening and treatment will be made for clients based on need or their request; c. Referral to other entitlement agencies including, but not limited to, Veterans Administration, SNAP and Medicaid; d. Referral to educational and vocational training; e. Create and maintain client resume on file; f. Referral for employment; and g. Referral to permanent housing, among others 4. Clean clothing and hygiene products, if available. 5. Random drug and alcohol tests will be conducted monthly and/or at shelter staffs discretion with test results included in client's case file. 6. The City reserves the right to conduct periodic audits of client files to ensure adherence to service benchmarks. Employee/ Contractor File Review The following documentation must be included in the employee/Contractor file for those employees/Contractors providing services under this contract. The following must be included in the employee files: • Employment Application • Evidence of degree/credentials • Job Description Signed by Employee • Evidence of Required Experience • Florida Background Criminal Screening • Proof of Knowledge of Policies & Procedures • Confidentiality Agreement Re: Client Information • Documentation of Agency Training/In-Service Training • Evidence of completion of mandatory inclusion training provided by Pridelines • 1-9 Verification on File The City reserves the right to inspect employee/Contractor files with due notice (at least forty-eight (48) hours in advance of planned site visit) to ensure adherence to contractual expectations. City of Miami Beach-Camillus House Page 21 Reporting Requirements The Contractor will provide the City with a Monthly Progress Report of all clients and reimbursement request by 5:00 PM on the fifth (5th) of the following month. In the event that the fifth of the month lands on a Saturday, Sunday or holiday, the report must be submitted the following business day. Monthly reports and reimbursement requests will be submitted via any of the following methods: • Electronic mail • Standard mail • Hand delivery Monthly reports will not be considered acceptable unless the following is met: • Forms are completely and accurately filled • Reports bear the signature of the person submitting the report on behalf of the Contractor Monitoring & Performance Reviews The City reserves the right to inspect, monitor and/or audit the Contractor to ensure contractual compliance. This includes, but is not limited to: • Review of on-site service delivery • Inspection and review of client, budgetary and employee files (for those employees providing services under this contract) The City will provide the Contractor a minimum of two (2) business days' notice prior to a monitoring visit. RECOMMENDATION Adopt the Resolution. ANALYSIS As part of its efforts to reduce homelessness, the City purchases emergency shelter beds at a variety of local area shelters. These shelters are contracted to provide a variety of services including meals and care coordination/case management to support a person in transitioning from homelessness to independent, sustainable living. The shelters are expected to deliver culturally-competent services to men, women and families with children. Since each shelter typically serves designated populations, the City contracts with a variety of providers to ensure that it can serve anyone who becomes homeless in our City. Therefore, the City request proposals from qualified care facilities to provide emergency shelter beds with care coordination services to homeless individuals from the City of Miami Beach in accordance with the priorities and goals identified in the City of Miami Beach 2013-2017 Consolidated Plan and the City's Key Intended Outcomes. The successful proposer(s) will be able to demonstrate participation in the housing and delivery services network(Continuum of Care) created by the Miami- Dade County Homeless Trust to address the needs of homeless individuals and families, including those with a history of substance abuse, mental illness, HIV/AIDS, and other special needs. Page 647 of 2353 RFQ PROCESS On March 1, 2017, the City Commission approved the issuance of Request for Proposals (RFP) No. 2017-075-KB for emergency shelter beds to serve homeless population. On March 2, 2017, the RFP was issued. A voluntary pre-proposal conference to provide information to the proposers submitting a response was held on March 14, 2017. The Procurement Department issued bid notices to 26 companies utilizing www.publicpurchase.com website and seven (7) companies via email. Thirty-four (34) prospective bidders accessed the advertised solicitation. Responses were due and received on April 17, 2017. The City received a sole proposal from Camillus House, Inc. On July 7, 2017, pursuant to Sec. 3-367 of the City Code that states that when only one bid response is received, the City Manager may reject the bid and, if he determines that same is in the best interest of the city, re-advertise the item," the City Manager authorized the rejection of the sole proposal received pursuant to Request For Proposals (RFP) No. 2017-075-KB for Emergency Shelter Beds to Serve Homeless Population and authorized the issuance of a new solicitation in an effort to secure additional contracts for emergency shelter beds. On July 19, 2017, the subsequent RFP was issued. A voluntary pre-proposal conference to provide information to the proposers submitting a response was held on July 27, 2017. Responses were due and received on August 18, 2017. The City received proposals from Camillus House, Inc. and The Salvation Army. The Evaluation Committee appointed by the City Manager pursuant to LTC#423-2017 convened on August 31, 2017 to consider the responsive proposals received. The committee was comprised of Lori Bakkum, Member, Committee on the Homeless, City of Miami Beach, Cristina Cabrera, Parent- Child Program Coordinator, City of Miami Beach, Nancy Grant, Homeless Program Coordinator, City of Miami Beach, and Ken Stenicky, Chair, Miami Beach Service Partnership. The Committee was provided an overview of the project, information relative to the City's Cone of Silence Ordinance and the Government Sunshine Law. The Committee was also provided with general information on the scope of services and a copy of each proposal. The Committee was instructed to score and rank each proposal pursuant to the evaluation criteria established in the RFQ. The evaluation process resulted in the ranking of proposers as follows: Camillus House, Inc., as the first-ranked proposer and The Salvation Army, as second-ranked proposer. Camillus House. Inc. According to the firm, Camillus House has provided humanitarian services to the indigent and homeless populations of Miami-Dade County, Florida for more than 50 years. Camillus House was established by the Little Brothers of the Good Shepherd in 1960; Camillus House has grown steadily over the years from a small overnight shelter into a full service center offering a "system of care" for persons who are poor and homeless. Every service offered at Camillus is carried out with the deeply held belief that every human being is precious in the eyes of the Lord and deserves love, respect and a chance to live a dignified life. Additionally, Camillus House has been the City's provider for emergency shelter beds to serve homeless population since 2014. The Salvation Army According to the firm, The Salvation Army opened its doors in Miami-Dade County in 1913 and is in the midst of celebrating its centennial anniversary. The Miami area command is made of its administrative offices which include its social services department with a 280-bed emergency shelter, four corps community centers, the adult rehabilitation center and emergency disaster services. Their dedicated staff is currently composed of fifty-two (52) individuals and many of the staff has been with Page 648 of 2353 the Miami Area Command for almost thirty(30)years. Additionally, The Salvation Army has been the City's provider for emergency shelter beds to serve its homeless population since 2012. CONCLUSION After reviewing the qualifications of each firm, consideration of staffs input, and the Evaluation Committee's comments and rankings, I recommend that the Mayor and the City Commission authorize the Administration to award contracts to Camillus House Inc., as the top ranked proposer, and The Salvation Army, as the second ranked proposer; authorize the Mayor and City Clerk to execute the agreements pending successful negotiations by the Administration and form approval by the City Attorney's office; further, authorize the City Manager to extend the existing agreements with Camillus House, Inc., and The Salvation Army, for emergency shelter beds to serve the homeless population, on a month-to-month basis, terminable by the City for convenience and without cause, upon thirty(30)days written notice, until such time as the new agreements can be executed. The City Manager's recommendation need not be consistent with the scoring results identified herein and takes into consideration Miami Beach City Code Section 2-369, including the following considerations: (1)The ability, capacity and skill of the Proposer to perform the contract. (2)Whether the Proposer can perform the contract within the time specified, without delay or interference. (3)The character, integrity, reputation,judgment, experience and efficiency of the Proposer. (4)The quality of performance of previous contracts. (5)The previous and existing compliance by the Proposer with laws and ordinances relating to the contract. The City Commission shall consider the City Manager's recommendation and may approve such recommendation. The City Commission may also, at its option, reject the City Manager's recommendation and select another Proposal or Proposals which it deems to be in the best interest of the City, or it may also reject all Proposals. Postponement/Cancellation/Acceptance/Rejection. The City may, at its sole and absolute discretion, reject any and all, or parts of any and all, responses; re-advertise this RFP; postpone or cancel, at any time, this RFP process; or waive any irregularities in this RFP, or in any responses received as a result of this RFP. Reasonable efforts will be made to either award the proposer the contract or reject all proposals within one-hundred twenty(120) calendar days after proposal opening date. A proposer may withdraw its proposal after expiration of one hundred twenty (120) calendar days from the date of proposal opening by delivering written notice of withdrawal to the Department of Procurement Management prior to award of the contract by the City Commission. 22. PROPOSER'S RESPONSIBILITY. Before submitting a response, each Proposer shall be solely responsible for making any and all investigations, evaluations, and examinations, as it deems necessary, to ascertain all conditions and requirements affecting the full performance of the contract. Ignorance of such conditions and requirements, and/or failure to make such evaluations, investigations, and examinations, will not relieve the Proposer from any obligation to comply with every detail and with all provisions and requirements of the contract, and will not be accepted as a basis for any subsequent claim whatsoever for any monetary consideration on the part of the Proposer. 23. COSTS INCURRED BY PROPOSERS.All expenses involved with the preparation and submission of Proposals, or any work performed in connection therewith, shall be the sole responsibility (and shall be at the sole cost and expense) of the Proposer, and shall not be reimbursed by the City. 24. RELATIONSHIP TO THE CITY. It is the intent of the City, and Proposers hereby acknowledge and agree, that the successful Proposer is considered to be an independent contractor, and that neither the Proposer, nor the Proposer's employees, agents, and/or contractors, shall, under any circumstances, be considered employees or agents of the City. 24. OCCUPATIONAL HEALTH AND SAFETY. In compliance with Chapter 442, Florida Statutes, any toxic substance listed in Section 38F-41.03 of the Florida Administrative Code delivered as a result of this proposal must be accompanied by a Material Safety Data Sheet(MSDS)which may be obtained from the manufacturer. 25. ENVIRONMENTAL REGULATIONS. The City reserves the right to consider a proposer's history of citations and/or violations of environmental regulations in investigating a proposer's responsibility, and further reserves the right to declare a proposer not responsible if the history of violations warrant such determination in the opinion of the City. Proposer shall submit with its proposal, a complete history of all citations and/or violations, notices and dispositions thereof. The non-submission of any such documentation shall be deemed to be an affirmation by the Proposer that there are no citations or violations. Invoices must be consistent with Purchase Order format. 29. COPYRIGHT, PATENTS & ROYALTIES. Proposer shall indemnify and save harmless the City of Miami Beach, Florida, and its officers, employees, contractors, and/or agents, from liability of any nature or kind, including cost and expenses for, or on account of, any copyrighted, patented, or unpatented invention, process, or article manufactured or used in the performance of the contract, including its use by the City of Miami Beach, Florida. If the Proposer uses any design, device or materials covered by letters, patent, or copyright, it is mutually understood and agreed, without exception, that the proposal prices shall include all royalties or cost arising from the use of such design, device, or materials in any way involved in the work. 30. DEFAULT: Failure or refusal of the selected Proposer to execute a contract following approval of such contract by the City Commission, or untimely withdrawal of a response before such award is made and approved, may result in a claim for damages by the City and may be grounds for removing the Proposer from the City's vendor list. 31. MANNER OF PERFORMANCE. Proposer agrees to perform its duties and obligations in a professional manner and in accordance with all applicable Local, State, County, and Federal laws, rules, regulations and codes. Lack of knowledge or ignorance by the Proposer with/of applicable laws will in no way be a cause for relief from responsibility. Proposer agrees that the services provided shall be provided by employees that are educated,trained, experienced, certified, and licensed in all areas encompassed within their designated duties. Proposer agrees to furnish to the City any and all documentation, certification, authorization, license, permit, or registration currently required by applicable laws, rules, and regulations. Proposer further certifies that it and its employees will keep all licenses, permits, registrations, authorizations, or certifications required by applicable laws or regulations in full force and effect during the term of this contract. Failure of Proposer to comply with this paragraph shall constitute a material breach of this contract. Where contractor is required to enter or go on to City of Miami Beach property to deliver materials or perform work or services as a result of any contract resulting from this solicitation, the contractor will assume the full duty, obligation and expense of obtaining all necessary licenses, permits, and insurance, and assure all work complies with all applicable laws. The contractor shall be liable for any damages or loss to the City occasioned by negligence of the Proposer, or its officers, employees, contractors, and/or agents, for failure to comply with applicable laws. 32. SPECIAL CONDITIONS. Any and all Special Conditions that may vary from these General Terms and Conditions shall have precedence. 33. NON-DISCRIMINATION. The Proposer certifies that it is in compliance with the non-discrimination clause contained in Section 202, Executive Order 11246, as amended by Executive Order 11375, relative to equal employment opportunity for all persons without regard to race, color, religion, sex or national origin. The city may consider any evidence available regarding the financial, technical, and other qualifications and abilities of a Proposer, including past performance (experience) in making an award that is in the best interest of the City, including: A. Pre-award inspection of the Proposer's facility may be made prior to the award of contract. B. Proposals will only be considered from firms which are regularly engaged in the business of providing the goods and/or services as described in this solicitation. C. Proposers must be able to demonstrate a good record of performance for a reasonable period of time, and have sufficient financial capacity, equipment, and organization to ensure that they can satisfactorily perform the services if awarded a contract under the terms and conditions of this solicitation. D. The terms "equipment and organization", as used herein shall, be construed to mean a fully equipped and well established company in line with the best business practices in the industry, and as determined by the City of Miami Beach. F.The City may require Proposer s to show proof that they have been designated as authorized representatives of a manufacturer or supplier, which is the actual source of supply. In these instances, the City may also require material information from the source of supply regarding the quality, packaging, and characteristics of the products to be supply to the City. 35. ASSIGNMENT. The successful Proposer shall not assign, transfer, convey, sublet or otherwise dispose of the contract, including any or all of its right, title or interest therein, or his/her or its power to execute such contract, to any person, company or corporation, without the prior written consent of the City. 36. LAWS, PERMITS AND REGULATIONS. The Proposer shall obtain and pay for all licenses, permits, and inspection fees required to complete the work and shall comply with all applicable laws. 37. OPTIONAL CONTRACT USAGE. When the successful Proposer(s) is in agreement, other units of government or non-profit agencies may participate in purchases pursuant to the award of this contract at the option of the unit of government or non-profit agency. 38. VOLUME OF WORK TO BE RECEIVED BY CONTRACTOR. It is the intent of the City to purchase the goods and services specifically listed in this solicitation from the contractor. However, the City reserves the right to purchase any goods or services awarded from state or other governmental contract, or on an as-needed basis through the City's spot market purchase provisions. 39. DISPUTES. In the event of a conflict between the documents, the order of priority of the documents shall be as follows: A. Any contract or agreement resulting from the award of this solicitation; then B. Addendum issued for this solicitation, with the latest Addendum taking precedence; then C. The solicitation; then D. The Proposer shall indemnify and hold harmless the City and its officers, employees, agents and instrumentalities from any and all liability, losses or damages, including attorney's fees and costs of defense, which the City or its officers, employees, agents or instrumentalities may incur as a result of claims, demands, suits, causes of actions or proceedings of any kind or nature arising out of, relating to or resulting from the performance of the agreement by the contractor or its employees, agents, servants, partners, principals or subcontractors. The contractor shall pay all claims and losses in connection therewith, and shall investigate and defend all claims, suits or actions of any kind or nature in the name of the City, where applicable, including appellate proceedings, and shall pay all costs, judgments, and attorney's fees which may be incurred thereon. The Proposer expressly understands and agrees that any insurance protection required by this Agreement or otherwise provided by the contractor shall in no way limit the responsibility to indemnify, keep and save harmless and defend the City or its officers, employees, agents and instrumentalities as herein provided. The above indemnification provisions shall survive the expiration or termination of this Agreement. 41. CONTRACT EXTENSION. The City reserves the right to require the Contractor to extend contract past the stated termination date for a period of up to 120 days in the event that a subsequent contract has not yet been awarded. Additional extensions past the 120 days may occur as needed by the City and as mutually agreed upon by the City and the contractor. 42. FLORIDA PUBLIC RECORDS LAW. Proposers are hereby notified that all Bid including, without limitation, any and all information and documentation submitted therewith, are exempt from public records requirements under Section 119.07(1), Florida Statutes, and s. 24(a), Art. 1 of the State Constitution until such time as the City provides notice of an intended decision or until thirty (30) days after opening of the proposals, whichever is earlier. Additionally, Contractor agrees to be in full compliance with Florida Statute 119.0701 including, but not limited to, agreement to (a) Keep and maintain public records that ordinarily and necessarily would be required by the public agency in order to perform the services; (b) provide the public with access to public records on the same terms and conditions that the public agency would provide the records and at a cost that does not exceed the cost provided in this chapter or as otherwise provided by law; (c) Ensure that public records that are exempt or confidential and exempt from public records disclosure requirements are not disclosed except as authorized by law; (d) Meet all requirements for retaining public records and transfer, at no cost, to the public agency all public records in possession of the contractor upon termination of the contract and destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. Proposers are expected to be familiar with, and comply with, all Federal, State, County, and City laws, ordinances, codes, rules and regulations, and all orders and decrees of bodies or tribunals having jurisdiction or authority which, in any manner, may affect the scope of services and/or project contemplated by this RFP (including, without limitation, the Americans with Disabilities Act, Title VII of the Civil Rights Act, the EEOC Uniform Guidelines, and all EEO regulations and guidelines). Ignorance of the law(s) on the part of the Proposer will in no way relieve it from responsibility for compliance. 44. CONFLICT OF INTEREST. All Proposers must disclose, in their Proposal, the name(s) of any officer, director, agent, or immediate family member(spouse, parent, sibling, and child)who is also an employee of the City of Miami Beach. Further, all Proposers must disclose the name of any City employee who owns, either directly or indirectly, an interest of ten (10%) percent or more in the Proposer entity or any of its affiliates. A Proposer may submit a modified Proposal to replace all or any portion of a previously submitted Proposal up until the Proposal due date and time. Modifications received after the Proposal due date and time will not be considered. Proposals shall be irrevocable until contract award unless withdrawn in writing prior to the Proposal due date, or after expiration of 120 calendar days from the opening of Proposals without a contract award. Letters of withdrawal received after the Proposal due date and before said expiration date, and letters of withdrawal received after contract award will not be considered. 47. EXCEPTIONS TO RFP. Proposers must clearly indicate any exceptions they wish to take to any of the terms in this RFP, and outline what, if any, alternative is being offered. All exceptions and alternatives shall be included and clearly delineated, in writing, in the Proposal. The City, at its sole and absolute discretion, may accept or reject any or all exceptions and alternatives. Proposers shall not offer any gratuities, favors, or anything of monetary value to any official, employee, or agent of the City, for the purpose of influencing consideration of this Proposal. Pursuant to Sec. 2-449 of the City Code, no officer or employee of the City shall accept any gift, favor or service that might reasonably tend improperly to influence him in the discharge of his official duties. 49. SUPPLEMENTAL INFORMATION. City reserves the right to request supplemental information from Proposers at any time during the RFP solicitation process, unless otherwise noted herein. 50. ADDITIONAL SERVICES. Although this solicitation and resultant contract identifies specific goods, services or facilities ("items"), it is hereby agreed and understood that the City, through the approval of the Department and Procurement Directors (for additional items up to $50,000) or the City Manager (for additional items greater than $50,000), may require additional items to be added to the Contract which are required to complete the work. When additional items are required to be added to the Contract, awarded vendor(s), as applicable to the item being requested, under this contract may be invited to submit price quote(s) for these additional requirements. If these quote(s) are determined to be fair and reasonable, then the additional work will be awarded to the current contract vendor(s) that offers the lowest acceptable pricing. Provide an organizational chart of all personnel, including key care coordination personnel, to be used for this project if awarded, the role that each will play in providing the services detailed herein and their qualifications. A resume of each individual, including education, experience, and any other pertinent information, shall be included for each individual to be assigned to this contract. 2.3 Financial Capacity. Fulfillment of Camillus House duties: Camillus staff members must ensure that activities related to campaigning for or holding public office do not interfere with fulfillment of required Camillus duties. • If a Camillus employee does choose to run for a public office,s/he must obtain prior approval from their Supervisor.Approval is not based upon the nature of the campaign itself,but is based upon the ability of the employee to fulfill their work obligations while also undertaking and fulfilling duties associated with conducting a campaign and holding office. • A Camillus staff member may request a Leave of Absence,if necessary, in order to run for or hold public office.The request should be made in accordance with the standard procedure for requesting a 10 CAMILLUS HOUSE POLICY#B-40-009 Leave of Absence.The request will be considered according to the standard guidelines for considering such requests. The request may be approved or denied based upon the surrounding circumstances and ability of Camillus to accommodate the request without undue disruption to administration and operation of programs. III. PROCEDURES FOR LOBBYING AND ADVOCACY WORK Camillus House is permitted to engage in some lobbying,advocacy,and education activities within the confines of Federal and all other applicable regulations. A. LOBBYING I. Engagement of Professional Services: Camillus House may under some circumstances engage the services of a registered lobbying firm.This may be done in order to advocate for specific issues or for resources. • If a lobbying firm is engaged,the firm must show that it is in compliance with all applicable laws and regulations relating to lobbying activities,including any applicable registration and disclosure requirements. • Any subcontracts or agreements with a lobbying firm must clearly delineate the nature of the relationship and scope of work to be performed. • All subcontracts or agreements with a lobbying firm must be approved by a majority of the Board of Directors'Executive Committee. 2. Unsubstantial Amount Rule: Camillus House must monitor the length and type of lobbying activities conducted, in order to ensure that such activities are not significant enough to constitute a substantial amount of resources or effort,as defined by IRS regulations.The organization's President is responsible for monitoring such activities,with input and assistance from the Director of Governmental Relations. B. ADVOCACY AND EDUCATION Camillus House is committed to advocating for issues that are directly related to the organization's mission.Advocacy and education efforts are permitted,as long as they do not show a preference for or against a particular candidate or political party.This includes advocating for or against specific legislative or ballot initiatives,as long as the advocacy activities do now show a preference for or against a particular candidate or political party. Camillus is proposing 12 emergency beds targeting single adult men who are homeless for service periods of up to 90 days.As with all Camillus shelter programs,case management services will be provided focused on implementing a care coordination model that includes a complete assessment in support of appropriate provision of services and referrals/follow-up on services in support of stabilization, preparing individuals for independent living and achieving the goals of individualized care plans.The program will provide 24/7 staffing and accommodate after hours intake. Funding requested for this program is$115,413($26.35 per bed/day). At the city's discretion,Camillus also has the capacity to provide an additional 13 beds at the same per diem rate. i. EXPERIENCE PROVIDING SIMILAR SERVICES TO HOMELESS POPULATION Camillus House is a private, non-profit organization providing humanitarian and social services to persons who are poor and homeless in Miami-Dade County for over 57 years.Camillus'comprehensive system of care provides outreach,emergency housing, residential and outpatient behavioral health treatment;supported transitional and permanent housing;employment training assistance;emergency services including food&clothing; and primary,sub specialty,ambulatory and dental services through sister agency Camillus Health Concern.The organization is committed to ending chronic homelessness in Miami-Dade County and serves over 235,000 meals each year; houses up to 1,100 persons each night; and provides services to more than 17,000 persons annually through various programs at 13 facility sites and nearly 125 apartments scattered throughout Miami-Dade. 19 Camillus House, Inc. EXPERIENCE DEVELOPING/OPERATING EMERGENCY HOUSING Camillus House has operated one of the most well-known emergency shelters in the Miami area since 1960, providing shelter,food,shower facilities, clothing,and case management,including provision and referrals to needed support services.Camillus has also operated and successfully managed the Emergency Solutions Grant Emergency Shelter(formerly operating at the Beckham Hall facility)for the past sixteen years serving more than 400 persons annually.The agency has also served as the central location for persons living on the streets to receive daily, basic human services.This includes a shower program,meal program, ID program,clothing exchange,and mail services for persons living on the street. Camillus currently manages 220 emergency housing beds at the NCL in the following programs: ■ ESG Grant—50 beds • Homeless Trust Primary Care Emergency Housing—75 beds • City of Miami Shelter—73 beds • FPL City of Miami Beach—12 beds ■ City of Coral Gables—3 beds • Florida DOH TB beds—2 beds • Baptist Health South Florida—5 beds Individuals are provided with non-treatment emergency housing and participate in a 90-day program. Residents are provided assessment, evaluation, ancillary referrals, vocational development, life skills training, employability training,and job search support.Case managers work with the clients to develop individualized care plans and provide continual assistance and guidance throughout program participation. Specific services offered include: Permanent Housing Placement Assistance: The Case Manager focuses specifically on the provision of these services for each client,as the primary goal of the program Benefits Assistance: Camillus House provides SOAR (SSI/SSDI Outreach, Access and Recovery) case management to participants across all programs. This initiative centralizes access to health and social benefits for eligible persons who are homeless. Staff is trained in SOAR case management and applications are submitted for qualified clients. Case management assists clients in procuring benefits from clinics and other agencies offering needed services.Case management services also assist clients in obtaining birth certificates, residency/citizenship,legal aid,work permits,food stamps(SNAP), Medicaid, food vouchers, Safe Link Assurance cell phones, clothing, etc. Camillus will continue to explore options for accessing other types of benefits that may be available or become available such as any future integration of the Affordable Care Act by the CoC. 20 Camillus House, Inc. All clients participate in a formal life skills training program during their first week in the program and at various intervals throughout. Vocational Training/Employment Placement/Services: Job readiness training and skills development services are provided through established linkages with Miami Dade County Public Schools (Lindsey Hopkins Educational Center), and Transitions to assist with vocational development and employment placement as needed. Camillus in association with the City of Miami and CareerSource South Florida,has a full operational CareerSource"One Stop Center"operating out of offices at the NCL campus. Educational Services:Referrals are made to various agencies based on individual client need. Emergency and Non-Emergency Medical Services: Camillus House refers clients to sister agency Camillus Health Concern for medical based on individual client needs. iii. EXPERIENCE PROVIDING SERVICES TO OTHER PUBLIC SERVICE ENTITIES Camillus has provided emergency housing since 1960,health care since 1984,and transitional and permanent housing since 1988.Camillus House has provided services to the Homeless Trust through a multitude of programs,beginning in 1998 with the funding of Camillus'Project Somerville permanent housing facility.Since then,the relationship has been expanded to include 15 current operating grants, including 6 HUD Supportive Housing Program grants and 8 Food and Beverage Grants(ACT Team, Homelessness Prevention, Project Lazarus,Case Management for Veterans,two transition grants,two Emergency Shelter Programs,and the Bethesda Housing First Program).Camillus has continued to exceed its outcome targets in nearly every program,with some examples provided below.All Camillus emergency,transitional,and permanent housing programs comply,at a minimum,with the Homeless Trust's Standards of Care. In addition to funding through the Homeless Trust,Camillus has a range of local,state and federal grants including the Emergency Solutions Grant(ESG)and Brownsville Christian Housing Center through the MDC Public Housing and Community Development department;emergency shelter for the City of Miami Beach,City of Miami,City of Coral Gables and the Florida Department of Health(TB), two County CBO grants;two SAMHSA Behavioral Health Treatment grants as well as contract with the State of Florida through DCF,SFBHN and the 11th Judicial Circuit Court for jail diversion,human trafficking recovery and residential/outpatient behavioral health treatment.These are complimented by a variety of private foundations,and as such,Camillus has demonstrated extensive experience in developing, implementing,evaluating,and continually improving supportive housing initiatives. Camillus has successfully completed numerous construction and capital improvement grants,which while funded through other County agencies,still required extensive coordination with the Homeless Trust. Camillus staff is actively engaged with CoC processes on multiple levels. Management staff sit on the CoC Subcommittee which reviews and approves the CoC's Homeless plan annually as well as on other committees of the Continuum including, Finance,Service and Providers.Additionally, program staff has participated in every homeless census count for the past 6 years. 21 Camillus House,Inc. Examples include: City of Miami Beach(Emergency Housing): • Served 67 clients in one year. • Average service days: 69 out of 90 • Of the 58 who left the program during the year,35(60%)had positive discharges,with 29% achieving permanent housing. Emergency Housing 1 • 85%of clients who stay more than three days will receive a complete bio-psychosocial assessment. Outcome:93%(129/139) • 50%of clients will transition into a higher level of care(transitional or permanent housing) Outcome:67%or 107 clients transitioned into a higher level of care. • 60%of clients will engage in social activities aimed at building rapport with other clients and prepare for transition into more stable housing. Outcome:93%,or 94 clients CAPACITY TO DOCUMENT AND REPORT Camillus has an outstanding record of documentation and reporting scoring well over 100%(via bonus points)on all of its Homelessness Management Information System(HMIS)monitorings.Camillus utilizes the data reporting functions of the HMIS system to evaluate progress of individual clients,as well as the overall program.The Camillus Director of Advancement and Outcomes produces monthly performance reports on all programs based on the stated outcomes of each contract. Management monitors length of stay in shelter or transitional housing,positive outplacements into housing from emergency and transitional housing,increasing income and benefits and employment across all programs. Program teams regularly review monthly performance reports and the status of individual clients in order to make adjustments to improve quality of service, client outcomes and program performance. HOURS OF BUSINESS OPERATION,CLIENT INTAKE,AND CLIENT CURFEWS Camillus House business hours are from Monday to Friday,from 8:00 a.m. until 5:00 p.m. Clients have a curfew of 10:00 p.m.seven days a week. Client intake occurs 24 hours a day,seven days a week. ACCESIBILITY TO PUBLIC TRANSPORTATION The Camillus' NCL campus is located along one of the more vibrant and commercially viable corridors in Miami adjacent to the 1-95 highway leading directly into Miami's medical district.There are two bus stops(one on either side of the 7th avenue which runs in front of the campus)which participants can easily access.A five minute bus ride into downtown Miami provides bayside market shops, restaurants, grocery stores, and access to Miami-Dade County's Metrorail services.A free trolley sponsored by the City of Miami runs through the corridor during baseball games at the new Miami Marlin's Park. Camillus is a Miami-Dade Community Transportation Coordinator(CTC)Provider and FDOT 5310 vehicle grant recipient and provides transportation to participants needing to access to additional community amenities.Scheduled pick-ups are arranged with program staff. NUMBER OF ACCESSIBLE BEDS FOR MOBILITY IMPAIRED PERSONS AVALIABLE When possible,Camillus will dismantle bunked beds to create ground-leveled beds for clients who are mobility impaired. When this cannot be done, priority will be given to the mobility impaired client to access the lower bunk bed.All areas of the campus,including to emergency shelter sleeping areas are mobility accessible. For homeless youth aged 18 and over still enrolled in the Miami-Dade County Public School system,case managers ensure continued enrollment and participation in school. The Camillus NCL facility has close proximity to a number of secondary and charter schools. Camillus also provides 12 beds through the Camillus Youth Housing Initiative(CYHI)to serve youth experiencing homelessness in Miami-Dade County.This collaborative effort assists youth in taking steps to achieve self-sufficiency and independence,through the provision of emergency,transitional housing and other supportive services. EXPERIENCE AND EFFICACY OF EMPLOYMENT PLACEMENT SERVICES; INCLUDING PERFORMANCE STATISTICS THAT LED TO CLIENT SELF-SUFFICIENCY Camillus House has collaborated with City of Miami and CareerSource South Florida(South Florida Workforce Investment Board)for the last two years to provide workforce development services for our clients utilizing the"one-stop"delivery system of resources. CareerSource operates a resource center manned by four full time staff to provide services to homeless clients.These resources include on-site workshops that teach clients a degree of proficiency in resume and cover letter writing, interviewing and job search skills. Camillus also has long-standing relationships with Transitions Inc.to provide workforce development services to clients in shelter,treatment and permanent housing. EXPERIENCE AND EFFICACY IN UTILIZING CARE COORDINATION STRATEGIES TO TRANSITION CUENTS TO INDEPENDENCE;INCLUDING THE FOLLOWING CLIENT DATA:(1)AVERAGE LENGTH OF STAY;(2) AVERAGE LENGTH OF TIME TO INCOME CREATION PER CLIENT STAY;(3)TRANSITION RATE TO PERMANENT HOUSING;AND(4)CARE MANAGER TO CUENT FREQUENCY OF ENGAGEMENT. While the definition of"care coordination"takes on a multitude of interpretations,the basic tenants of the term remain the same. This includes collaboration between a case manager and a client in the construction of an integrated and individualized case plan and ensuring effective delivery of services to clients in order for them to meet their objectives. At Camillus,care coordination also involves facilitating communication between multiple agencies, providing referrals to outside services,as necessary,and reviewing client progress through stated goals and making any modifications as needed. Camillus House has proven successful in providing these tenants of care coordinated services to our clients, as shown through the following measurements: 1) Average length of stay a. The average length of stay for emergency housing program clients at Camillus House is 58 days. 2) Average length of time to income creation per client stay a. During case plan creation,clients discuss their current income status and,in conjunction with the case manager,formulate a method whereby the client will either obtain, 24 Camillus House, Inc. Of the 1327 clients that were served in our emergency housing program in the previous year,501 clients(38%)achieved permanent housing through the support received during their stay. 4) Care manager to client frequency of engagement a. Case managers meet with clients to review and update their case plans on a weekly basis,at minimum,depending on the care coordination needs of the client. ACCESSIBILITY TO ALCOHOLICS ANONYMOUS AND SIMILAR SUPPORT GROUP MEETINGS(EITHER IN- HOUSE OR IN CLOSE PROXIMITY TO FACILITY) Camillus provides on-site Alcoholics and Narcotics Anonymous(AA/NA)services to clients residing in the residential treatment building. All other Camillus clients have access to NA/AA Groups within the community to support participants creating circles of support for their recovery. A list of local meetings is provided to clients upon request. Clients have a plethora of Meeting location options,as there are more than 30 AA/NA Groups that are held 7 days a week and at varying times within a 5 mile radius of the NCL Campus. In addition to utilizing public transportation to attend these meetings,Camillus provides transportation to participants needing to access to additional community amenities.Scheduled pick-ups are arranged with program staff. RULES TO BE FOLLOWED BY PLACED CLIENTS All clients in all housing programs are subject to the same rules and regulations.These rules ensure the safety of all clients and staff at the facility. At intake,all clients sign releases of information and acknowledgement of the program format and rules. Housing provided at NCL Campus is in a community setting and therefore it is expected that persons conduct themselves in a manner that is respectful to others living and working around them. Camillus seeks at all times to work with clients with behavioral health issues to keep them housed, however,the agency has a zero tolerance policy in regards to any violent behavior towards staff and other clients. RULES,PARAMETERS AND/OR PROCESS FOR BANNING CLIENTS Clients are only discharged from the program when they move on to other stable housing(e.g. permanent supportive housing,their own unsubsidized home or move in with family)or for repeated non-compliance with program rules. Once a client has been discharged from the program (be it a positive,negative,or unknown destination),the City of Miami Beach will be notified by Camillus House regarding the discharge,along with any pertinent information supporting the discharge. Clients may be discharged for: ■ Threats of Violence,Actual Violence or Possession of Deadly Weapon • Felony, Drug-Related Criminal Activity or Violent Criminal Activity • Possession of drugs, Drug Paraphernalia or Alcohol on campus(after behavioral contract) 25 Camillus House,Inc. The aim is to keep clients in the program so that they can successfully complete case plan and transition into the job market and a permanent housing situation. IDENTIFICATION OF AFTERHOURS PERSONNEL TO CONTACT WITH EMERGENCIES OF OFF-HOURS PLACEMENTS The program will provide 24/7 staffing and will accommodate after hours intake. Residential Assistants will provide 24/7 monitoring and assistance.This program will accommodate after hours intake for the City's outreach teams.Note that 24/7 wake coverage is also provided throughout the entire campus which means additional support staff is available to assist in an emergency, if needed.Also,24/7 campus security conducts periodic walkthroughs of all facilities. ACCESSIBILITY TO COUNSELING AND MENTAL HEALTH SUPPORT SERVICES Chronically Homeless clients in emergency shelter are eligible for outpatient services through the Camillus SAMHSA funded Grants for the Benefit of Homeless Individuals program. This program provides outpatient services for these individuals with the goal of stabilizing them before and during housing placement. Courses of treatment usually range between six and nine months depending on the needs of the client and the program serves up to 100 persons annually. Camillus is in the process of renewing this grant and will be expanding its scope to serve homeless individuals regardless of chronicity,as well as improve the additional supportive services available to shelter clients. Clients also have the options to access treatment services through the Camillus Institute for Personal Achievement and Success(ISPA) residential treatment program(funded by the State of Florida through SFBHN and SAMHS),as well as through the ISPA Re-entry Program Residential or outpatient treatment(funded by the State of Florida through SFBHN and the 11th Circuit Court) if deemed eligible by the Courts. Female clients identified by the City who are victims of Human Trafficking can also be referred directly into a treatment bed in the Camillus Human Trafficking Recovery Program(funded by the State of Florida through DCF). ADDITIONAL IN-HOUSE OR ACCESSIBLE SERVICES THAT SUPPORT THE SUCCESSFUL TRANSITION OF CLIENTS FROM SHELTER TO PERMANENT HOUSING Placement into permanent housing is the main goal of every Camillus program,and work towards this goal is incorporated into the initial case plan completed on intake. Camillus provides ongoing case management services to clients in all its programs.Permanent supportive housing participants receive ongoing life skills training to assist individuals in developing the skills needed to live independently and without assistance.Job skills training and vocational training resources are used to ensure participants obtain employment or remain employed and are able to meet their lease/rent obligations.Persons recovering from substance abuse are required to participate in ongoing AA/NA meetings to avoid 26 Camillus House, Inc. Camillus House operates its emergency shelter service,located at 1603 NW 7th Avenue, in accordance with the federal definition as articulated in 24 CFR Part 91:Any facility with overnight sleeping accommodations,the primary purpose of which is to provide temporary shelter for the homeless in general or for specific populations of the homeless. As such, it follows the listed US HUD performance measures for shelter as per the HUD priority of systemic responses to homelessness: • Reduce length of time homeless • Reduce returns to homelessness • Reduce number of homeless • Increase employment and income • Increase successful placement and retention As evidenced by its listing as a Homeless Trust(HT) Provider(see Tab 1,Section C1),Camillus House participates in the Miami-Dade County Continuum of Care(CoC)to address the needs of homeless individuals and families,including those with a history of substance abuse,mental illness, HIV/AIDS,and other special needs utilizing a Care Coordination service model. This model involves provision of referral and follow-up to housing and supportive services, including but not limited to primary health care, benefits access, behavioral health treatment, legal services,immigration,and job development services. Camillus House adheres to the Standards of Care established by the Miami-Dade County Homeless Trust for provision of emergency shelter services in the CoC system.The CoC is designed to provide homeless individuals and families with the resources and opportunities by which to attain residential,financial and personal stability and self-sufficiency.Accordingly,all HT funded providers are mandated to provide services geared towards achieving household stability and retention in housing. The Homeless Trust provides these Standards of Care to establish the minimum scope of housing and services to be provided and the minimum level of quality of care when providing such housing and services directly to homeless individuals and families. Camillus is one of the largest providers in the Miami-Dade County Continuum of Care system. Management staff of Camillus serve on the Homeless Trust Board of Directors,as well as the Services Development Committee.Camillus has therefore incorporated the Trust's Standards of Care into its housing ethos and practice. Management staff sit on the CoC Subcommittee which reviews and approves the CoC's Homeless plan annually,as well as on other committees of the Continuum including, Finance,Service and Providers. Outreach,Assessment,and Placement Camillus House is an active provider in the Miami-Dade County Homeless Continuum of Care and is currently a CoC Specialized Outreach provider funded through the Homeless Trust with local Food and Beverage tax dollars. The Specialized Outreach team(comprised of Camillus House and sister agency 47 Camillus House, Inc. MIAMI BEACH RFP 2017-293-KB SCOPE OF SERVICES PROPOSED Camillus Health Concern staff)works closely with the Coordinated Outreach teams as part of the coordinated entry system of the CoC,to provide on-street care. The Camillus Specialized Outreach program,called Project Lazarus, has been operational since October 2015 and seeks to engage,assess, treat(medically and mentally)and secure supportive permanent housing placement for persons who are chronically homeless and vulnerable or both on the streets of Miami-Dade County. Through coordinated street outreach activities,the project engages persons experiencing homelessness, in their environment,and facilitates access to the healthcare and social services delivery system. With the respect for the dignity and rights of the client at the forefront of all of their engagements,staff address the individual's immediate health needs,and ultimately any long-term chronic conditions, provides mental health treatment,and navigation to benefits with the end goal of engaging the client into accepting services and come into shelter,treatment,or directly to housing.The project targets the chronically homeless, particularly those identified as the most service-resistant and having severe mental disorders,cognitive,and other disabilities,as well as chronic illness, including veterans on the streets and other areas not fit for human habitation.This outreach takes place 7 days a week in the early morning and late evening. For those individuals accepting of services,their immediate psychological and medical needs are addressed through assessment and diagnosis,medication reminders and access to treatment for chronic conditions. Coordinated assessment is provided through the standard continuum assessment document,the VI-SPDAT(Vulnerability Index-Service Prioritization Decision Assistance Tool),which helps to better identify the most appropriate program for the individual client and subsequent referral.The team works with CoC Outreach to abide by CoC rules on coordinated entry for referrals into shelter and to the permanent housing waitlist. Camillus will provide 12 emergency housing beds to single adults who are homeless. The beds will be in the"Florida Power&Light(FPL)Company Power to Care Facility"or"Building G"of our Norwegian Cruise Line(NCL)Campus. Linkages to services are provided both internally and externally and focus on stabilization, needs assessment,and the creation of a housing plan so that clients will eventually achieve transitional or permanent housing. Based on our extensive experience serving this population,we see no impediment to begin occupying these beds on October 1,2017. The program will benefit greatly from its location,which includes, but is not limited to,the following on-site amenities: • Dining hall providing 3 nutritious meals a day • Access to washers and dryers • Clothing • ID and Mail services • Computer Access • Life and social skill development activities • Wellness Center that offers stress management,fitness,and nutrition activities • Job development services(thru our partnership with the City of Miami and CareerSource South Florida) • Outpatient behavioral health treatment • Assistance with entitlement services(thru our on-staff and full time SSI/SSDI Outreach,Access, and Recovery(SOAR)case manager) • Care Coordination beyond Shelter Service Scope, including assistance in enrolling in GED classes or vocational training(via Lindsey Hopkins), primary health care services(thru our sister agency 48 Camillus House, Inc. MIAMI BEACH RFP 2017-293-KB SCOPE OF SERVICES PROPOSED Camillus Health Concern),and on-site hospital and institution services(NA/AA)for interested clients. 3.1.1 Proposer shall include detailed information on the extent to which Proposer plans support the goals of the local Continuum of Care Plan created by the Miami-Dade County Homeless Trust,and meet the priorities identified in this RFP. Camillus House fully supports the goals of the local Continuum of Care Homeless Plan (2017: Priority -)Home)created by the Miami-Dade County Homeless Trust and implements its services in accordance with the priorities and strategies of the plan. Camillus House is a voting member of the CoC Subcommittee that reviews and updates this plan annually and participated in the 2017 review and approval of the goals and strategies outlined in the Plan. Camillus operates its emergency shelter services in accordance with the Plan goal of ending homelessness and preventing chronic homelessness. This is accomplished through participating in a shelter system that reduces length of stay, reduces recidivism and increases housing placement and retention through strategies that make"homelessness rare, brief and non-recurring,offering shelter to all persons experiencing homelessness,and rapidly rehousing persons experiencing homelessness"(2017: Priority-Home, Priority 1). Camillus will also continue to work with the City of Miami Beach Homeless Program to meet the priorities identified in this solicitation.Camillus agrees to accept the initial care plan created by the City at the time of the client's placement.A standard goal for the local Continuum of Care Plan created by the Homeless Trust is for every client within their initial care plan to achieve the permanent affordable housing situation that is most appropriate to them,whether this be public housing funded housing vouchers,CoC or other permanent supported housing,independent living or reunification and living with family on a permanent basis. All clients will be made aware that transition to a permanent housing situation is their primary goal at intake,and case managers will work with each client to determine which permanent housing(or treatment if appropriate)solution is most suited to their needs and personal goals. In the formulization of the initial care plan,case managers will also conduct a complete needs assessment to develop individualized goals that will aid the client in the processes of stabilization and preparing for independent living. While in shelter, clients' progress toward their stated goals will be noted by their case manager via HMIS and case plans will be reviewed and updated with the client on a weekly basis. This review will allow for modifications/updates to reflect the progress and/or setback towards,completion,or addition of a goal, adjust timelines in order to reflect current realities,or to discuss if any additional attention is needed by either the client or the program. Camillus House will notify the City of Miami Beach of any client who is out of compliance with the program and not actively pursuing the goals of their plan and share all relevant case information as dictated by the contract with the City and the associate releases of information from the client. All clients sign releases of information,acknowledgement of the program format and rules as part of the intake process.There will be set rules that ensure the safety of all clients and staff at the facility.Clients are only discharged from the program when they move on to other stable housing(e.g.permanent supportive housing,their own unsubsidized home or move in with family)or for repeated non- compliance with program rules. Housing provided at NCL Campus is in a community setting and 49 Camillus House, Inc. MIAMI BEACH RFP 2017-293-KB SCOPE OF SERVICES PROPOSED therefore it is expected that persons conduct themselves in a manner that is respectful to others living and working around them. Once a client has been discharged from the program(be it a positive, negative,or unknown destination),the City of Miami Beach will be notified by Camillus House regarding the discharge,along with any pertinent information supporting the discharge. Camillus House will ensure that relevant program staff attend Project Safe training as per the requirements of this solicitation should the agency be awarded. 3.1.2 Proposer shall describe their participation in the Trust's Homeless Management Information System(HMIS)and compliance with established referrals. Camillus House utilizes the CoC's Homeless Management Information System (HMIS)for all its CoC funded and non-CoC funded programs.This allows for coordination with the Homeless Trust and other providers on client care and history,as well for housing referrals which are managed by the Housing Coordinator at the Homeless Trust. HMIS is used to set up ISP's(individual service plans/care plans)for clients. Once intake assessments are complete and a client profile established for the program in HMIS,the case manager then works with the client in building their care plan in order to set goals and timelines.These goals are based on an assessment of the client's needs and current behavioral health status,as well as the individual's personal objectives.While every service plan is unique to the individual,goals are typically related to accessing benefits, accessing healthcare,accessing and completing behavioral health treatment,finding employment,improving education,finding permanent housing,and reunification with family.This person-centered process seeks to identify the supports,activities and resources required for the client to achieve the set goals of their care plan. 3.1.3 Proposer shall describe their hours of placement and their ability to accept shelter placements 24 hours per day. The program will provide 24/7 staffing and will accommodate after hours intake. The staff-to-client ratio will be 1.5 FTE full-time staff during regular operating hours(9AM—5 PM)and one staff per 12 clients during evening and weekend hours.A total 2.8 FTE will provide direct services with an additional .45 FTE providing supportive services. Residential Assistants will provide 24/7 monitoring and assistance. This program will accommodate after hours intake for the City's outreach teams. Note that 24/7 wake coverage is also provided throughout the entire campus which means additional support staff is available to assist if needed.Also,24/7 campus security conducts periodic walkthroughs of all facilities. 50 Tab4 Value Added Services Camillus House,Inc. MIAMI BEACH RFP 2017-293-KB VALUE ADDED SERVICES Submit detailed information on any value added services that may differentiate Proposer from other Proposers,especially strategies for increasing the flexibility in the usage of funds to meet the needs of consumers of the relevant services in the local community. It has been estimated by the US Department of Housing and Urban Development that the annual cost to the taxpayer of an untreated homeless individual(in the form of police,court system and hospital emergency care)exceeds$40,000 per year. The homeless population of Miami-Dade County is complex and culturally diverse,with each individual and family facing a multitude of issues that have led to their homeless situations. Many have substance and abuse and mental health issues,most are unemployed or underemployed and all are well below the federal poverty line.Access to treatment and to benefits for which they may be entitled, navigation of legal and immigration issues,vocational supports and job development are critical services in the process of lifting these individuals and families out of their homeless circumstances. Homeless individuals are food insecure,a situation associated with chronic health problems in adults including diabetes, heart disease, high blood pressure,and mental health issues particularly major depression. Homeless individuals benefit from food assistance while seeking to stabilize their housing and economic situation. It is therefore essential to move homeless individuals off the streets and into stable,supportive environments as quickly as possible.The services available at Camillus House's state-of-the-art NCL campus serve as a stepping stone by providing resources to stabilize and assist its participants to achieve the highest level of self-sufficiency and housing readiness within their respective limits. With this in mind,Camillus provides participants guidance in obtaining benefits and/or employment. Individualized case plans outline each individual's steps to securing SSI/SSDI, Medicaid,or other benefits if eligible as well as employment. Due to the nature of the extreme mental illness with some of the clients,the best approach for increasing client income is often through utilizing the SOAR case management services in order to complete benefits applications for access to SSI and SSDI. Where possible Supportive Employment(SE) is utilized to assess and appropriately assist clients with job-seeking efforts. SE is an approach to vocational rehabilitation for people with serious behavioral health issues that emphasizes helping them obtain competitive work in the community and providing the supports necessary to ensure their success in the workplace. Camillus currently offers the Wellness Recovery Action Plan(WRAP)training to clients.WRAP is designed to assist clients with learning how to manage their mental illness more effectively to obtain employment and make permanent housing more sustainable. Camillus House also provides services to identify and connect program participants with business owners/employers eager to provide employment to persons who are or have been homeless through referrals to employment partners working with the Department of Vocational Rehabilitation as well as through CareerSource. Participants for this program will be referred to appropriate employment opportunities that can accommodate behavioral health disabilities. In addition,Camillus has begun using CDM* Internet an online career assessment tool that allows individuals to explore interests and possible career paths,and for persons re-entering the world of work or interested in changing jobs to learn about career options. This allows case managers to support client career planning by identifying training requirements,job outlooks,earning potential and current job openings. 52 Camillus House,Inc. MIAMI BEACH RFP 2017-293-KB VALUE ADDED SERVICES Furthermore,to support the transportation needs of clients in the program,Camillus provides bus passes or transportation for everyday activities and to access services.Clients will also be supported in obtaining free bus passes, if eligible.Camillus House is a transportation disadvantaged provider in the Florida Commission for the Transportation Disadvantaged(CTD)through a Coordinated Fare Agreement with the CTD Community Transportation Coordinator for the county,Miami-Dade Transit. Under this agreement,Camillus provides and documents trips for older adults,persons with disabilities, persons of low income and children at-risk as part of all Camillus House programs.
