SECTION FOUNDATION
Equipment Grant Agreement
The USIA Florida Section Foundation awards this Equipment Grant contingent upon
the following:
Purpose: This grant is made for the purpose outlined in the grant award letter and
may not be used for any other purpose without the Foundation's prior written
approval.
Accountability: All grantees are required to submit timely progress reports as
requested by the Foundation. If you fail to submit the required accountability report,
you may not be eligible for future grants. In addition, your program may be
monitored by site visits.
Equipment: The Foundation reserves the right to withdraw any equipment if you do
not fulfill the accountability requirements.
Publicity: You will allow the Foundation to review and approve the text of any
proposed publicity concerning this grant prior to its release. The Foundation may
include information regarding this grant, including the amount and purpose of the '�"
grant, any ou rovide, your logo or trademark, or other information or

materials about your organization and its activities, in its electronic and written public v
reports, newsletters, and news releases. N L+.

Your organization will agree to acknowledge the Foundation as a source of your . ) 1
program funding and agree to promote it to tennis program participants, donors,
sponsors and friends of the organization. In addition, use the following language as a 61 ni
guide to acknowledging your grant in print: This program/project was funded (or %
funded in part)by an Equipment Grant from The USTA Florida Section Foundation.
