AMENDMENT NO. 1
TO
COMMUNITY DEVELOPMENT BLOCK GRANT AGREEMENT
BETWEEN
CITY OF MIAMI BEACH, FLORIDA
AND
UNIDAD OF MIAMI BEACH, INC.
This Amendment No. 1 (Amendment) to the Agreement, dated February 5, 2018
(Agreement), by and between the CITY OF MIAMI BEACH, FLORIDA, a municipal
corporation organized and existing under the laws of the State of Florida, having its
principal place of business at 1700 Convention Center Drive, Miami Beach, Florida
33139 (the City), and UNIDAD OF MIAMI BEACH, INC., a Florida not-for profit
corporation, having its principal place of business at 7251 Collins Ave. Miami Beach,
Florida 33141 (Sub-Recipient), is entered into this _ ?PO day of ddi , 2018

RECITALS
WHEREAS, on June 7, 2017, the Mayor and City Commission adopted Resolution
Number 2017-29876, approving the One-Year Action Plan for Federal Funds for FY
2017/2018, and providing CDBG funds in the amount of FIVE THOUSAND DOLLARS
($5,000) for Project LINK to be provided by the Sub-Recipient as a Public Service
(Program); and
WHEREAS, although the public service percentage remained the same, the
amount allocated to Public Services increased; and
WHEREAS, the increase was prorated among awarded public services as a
percentage of the original allocation; and
WHEREAS, the updated amount allocated to the Program was increased to
FIVE THOUSAND FIVE HUNDRED AND NINETY-TWO DOLLARS ($5,592); and
WHEREAS, on February 5, 2018, the City and Sub-Recipient executed the
Agreement (the Agreement), which services include information and referral services, as
set forth in Exhibits A through F of the Agreement; and
WHEREAS, the Agreement erroneously states that the Agreement was executed
on February 5, 2017; and
WHEREAS, the parties agreed to move the commencement date of the Term of
the Agreement to October 1, 2017; and
WHEREAS, according to 24 CFR 570.208 (a)(2), elderly persons are generally
presumed to be principally low- and moderate-income persons and do not require
income verification;
NOW THEREFORE, in consideration of the mutual promises and conditions
contained herein, and other good and valuable consideration, the sufficiency of which is
hereby acknowledged, the City and Sub-Recipient hereby agree to amend the
Agreement as follows:
1. ABOVE RECITALS.
The above recitals are true and correct and are incorporated as part of this
Amendment.
2. MODIFICATIONS.
The Agreement is hereby amended (deleted items &truck through and inserted
items underlined) as follows:
(a) Paragraph 1 of the Agreement is hereby amended to read as follows:
This Agreement made and entered into this 5`" day of February 2017
2018, by and between the CITY OF MIAMI BEACH, FLORIDA, a
Florida municipal corporation having its principal office at 1700
Convention Center Drive, Miami Beach, Florida, 33139, (hereinafter
referred to as "City"), and UNIDAD of Miami Beach Inc., a Florida not-
for profit corporation, having its principal office at 7251 Collins Ave.
Miami Beach, FL 33141 (hereinafter referred to as "Sub-Recipient").
(b) Section 11 of the Agreement is hereby amended to read as follows:
Term: This Agreement shall commence at contract execution be
deemed to have commenced retroactively on October 1, 2017 and shall
terminate on September 30, 2018 (the Term), with the understanding
that atT the end of the Term, the City Commission has the authority to
reappropriate any remaining unused Funds.
(c) Exhibit "B" to the Agreement is deleted in its entirety and replaced with
the attached Amended Exhibit "B".
3. RATIFICATION.
Except as amended herein, all other terms and conditions of the Agreement shall
remain unchanged and in full force and effect. In the event there is a conflict
between the provisions of this Amendment and the Agreement, the provisions of
this Amendment shall govern.
IN WITNESS WHEREOF, the parties hereto have caused this Amendment to be
executed by their appropriate officials, as of the date first entered above.

"Documentation"
The Sub-Recipient agrees to maintain the following documents as part of its record-
keeping requirements:
To document insurance coverage:
Copy of insurance policy or binder (with proof of payment) with limits and scope of
coverage adding the City of Miami Beach as additional insured
To document service:
• Project Link exclusively serves elderly residents living in the City of Miami Beach.
According to 24 CFR 570.208 (a)(2), elderly persons are generally presumed to
be principally low- and moderate-income persons and do not require income
verification.
• Keep beneficiary identification on file to verify age
• Intake & screening forms must include:
• Name
• Date of birth
• Beneficiary's address
• Demographic information as required in the CDBG Accomplishment Form
• Executed client consent Agreement to receive services
• Client authorization to release information
• Program enrolled
• Any additional documents as requested by the City
To document expenses incurred:
Supporting documents to be submitted with the Monthly Finance Report are as follows:
• Salaries related expenses:
• Cost allocation plan for each of the employees with split funding
• Timesheets reflecting the agreed budget percentage
• Electronic payroll ledgers with corresponding bank transactions
(statement)
• Cancelled checks
• Form 941 — Employer's Quarterly Federal Tax Return
• Unemployment payment
• Any additional documents as requested by the City
• Fringe Benefits related expenses:
• Bank statements
• Worker's Compensation invoice, policy and cancelled check
• Any additional documents as requested by the City
