A RESOLUTION OF THE MAYOR AND CITY COMMISSION
OF THE CITY OF MIAMI BEACH, FLORIDA, AUTHORIZING
THE ISSUANCE OF A CERTIFICATION OF CONSISTENCY
WITH THE CITY'S CONSOLIDATED PLAN TO DOUGLAS
GARDENS COMMUNITY MENTAL HEALTH CENTER OF
MIAMI BEACH TO CONTINUE OPERATING AN OUTREACH
TEAM PROGRAM PROVIDING MOBILE OUTREACH AND
CASE MANAGEMENT SERVICES FOR HOMELESS
PERSONS.
WHEREAS, on July 26, 1995, the Mayor and City Commission approved the City s
Consolidated Plan; and
WHEREAS, Douglas Gardens Community Mental Health Center of Miami Beac h
has proposed to continue operating an Outreach Team Program which provides outreach
and case management services for homeless persons; and
WHEREAS, the U.S. Department of Housing and Urban Development (HUD)
requires that applicants for Federal funding under the Supportive Housing Program obtain
and submit a Certification of Consistency with the Consolidated Plan from the entitlement
jurisdiction where the proposed activity is to be located; and
WHEREAS, in accordance with the City's adopted Consolidated Plan, each reque.;t
for a Certification of Consistency with the Consolidated Plan requires City Commis:;ic n
review and approval; and
WHEREAS, the Administration has reviewed the proposed activity and found it 110
be consistent with the Consolidated Plan.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE MAYOR AND CIT'(

Approve the Resolution.
BACKGROUND:
On June 2,1997, Douglas Gardens Community Mental Health Center (Douglas Gardens)
requested a Certification of Consistency with the Consolidated Plan in order to Sl~e(
Federal funding to continue operating an existing Outreach Team Program which provide;
outreach and case management services for homeless persons. The Outreach Tearl
Program operates from the main office of Douglas Gardens in Miami Beach, 701 Uncoil
Road, and it serves primarily homeless individuals in Miami Beach.
The outreach team travels in a van equipped with a cellular phone, a pager, and a COUI1t}-
issued two-way radio. The outreach team contacts homeless individuals and families livin~.J
on the street in order to assess their need for services. After assessments are completec ,
a case manager makes the appropriate referrals.
Since the Outreach Team Program works in conjunction with the Metro-Dade COLint l
Homeless Trust's "Continuum of Care", the majority of referrals are made to emerge:1c l
and transitional housing providers funded by the Trust. The Homeless Trust-funded
emergency shelter for persons from Miami Beach is the Salvation Army shelter, located at
1907 N.W. 38th Street, Miami. The Homeless Trust has pre-paid beds at this facility tl)
increase accessibility. Once a referral is made, the homeless client is provided with

transportation to the service provider. The Salvation Army case manager coordinates ,,,itl
Douglas Gardens' case manager to identify subsequent housing options for the indiviclucl
clients.
The Outreach Team Program has operated since July 1995 under a non-renewCilbl~
Initiatives Grant awarded by the U.S. Department of Housing and Urban Developmert
(U.S. HUD) to Metro-Dade County. In order to continue offering this service, Dou~.Jla s
Gardens is participating in a county-wide grant application being submitted by the Metrc-
Dade Homeless Trust to U.S. HUD under the Supportive Housing Program. Dou~lla s
Gardens has indicated that the deadline for submission of the application package to th .~
Homeless Trust is June 23, 1997. The Administration received this request on Junl~ ~ ,
1997, therefore allowing only one opportunity for the City Commission to consider it be":or3
the deadline.
ANALYSIS:
U.S. HUD requires that applicants for funding under the Supportive Housing Progl'afl
obtain and submit a signed Certification of Consistency with the Consolidated Plan fl'Ofl
the entitlement jurisdiction where the proposed activity is to be located. In accordance ",lIitl
the City's adopted Consolidated Plan, each request for a Certification of Consistency Witl
the Consolidated Plan requires City Commission review and approval.
The Administration has reviewed the proposed activity, and has concluded that thi ~s
proposal is consistent with the City's Consolidated Plan. The Consolidated Plan identifie s
long term objectives under Section 6, "Strategic Plan", part B, "Assignment of Prioriti'i3S',
on pages 6-1 to 6-3, including the following:
"To reduce the number of homeless and persons at risk of becoming homeles~.... "
CONCLUSION:
The Administration recommends approval of the attached Resolution of the Mayor anj
City Commission of the City of Miami Beach, Florida, authorizing the issuance of3
Certification of Consistency with the City's Consolidated Plan to Douglas Garden s
Community Health Center of Miami Beach to continue operating an Outreach TE!arn
Program providing mobile outreach and case management services for homeless persom,.
