RESOLUTION OF THE MAYOR AND CITY COMMISSION OF
THE CITY OF MIAMI BEACH, FLORIDA, AUTHORIZING
THE CITY TO PREPARE AND SUBMIT AN APPLICATION
FOR NEW FUNDING OF A GRANT FROM THE STATE OF
FLORIDA, DEPARTMENT OF HEALTH AND
REHABILITATIVE SERVICES (HRS) , FOR SUPPORTED
INDEPENDENT LIVING OF DEVELOPMENTALLY DISABLED
(MENTALLY RETARDED) ADULTS, AND TO APPROPRIATE
FUNDS AND EXECUTE A CONTRACT FOR THE FINAL
GRANT AWARD IN THE FORM ATTACHED HERETO, IN
ACCORDANCE WITH THE GRANT PERIOD AND THE
AMOUNT NEGOTIATED WITH HRS.
WHEREAS, the State of Florida, Department of Health and
Rehabilitative Services (HRS) is desirous of having the City of
Miami Beach apply for new funding to provide supported independent
living for developmentally disabled (mentally retarded) adults; and
WHEREAS, the City is desirous of submitting an application for
funding, in order to provide essential support and services to
developmentally disabled (mentally retarded) adults in the
estimated amount of $35, 400 for the period of July 1, 1993 , through
June 30, 1994 ; and
WHEREAS, the City has been successfully operating training
programs since 1983 and wishes to provide supported living
services; and
WHEREAS, the City will prepare the budget in accordance with
the applicable grant period and the amount negotiated with HRS ; and
WHEREAS, it is necessary that such funds be appropriated for
the final grant award.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY
COMMISSION OF THE CITY OF MIAMI BEACH, FLORIDA, that the
Administration be authorized to prepare and submit an application
to the Department of Health & Rehabilitative Services (HRS) in
accordance with the grant period and the amount negotiated with
HRS, that the funds be appropriated for the final grant award, and
that the Mayor and the City Clerk be authorized and directed to
execute the contract in the form attached hereto, in order to
provide supported independent living for up to three (3)
developmentally disabled (mentally retarded) adults under the
auspices of the Miami Beach Activity Center.

ADMINISTRATION RECOMMENDATION
The Administration recommends adoption of the attached two (2)
Resolutions which authorize: the preparation and submission of two
(2) grant applications to HRS; the appropriation of funds for the
final grant awards; and authorization for the Mayor and City Clerk
to execute the two (2) corresponding contracts.
BACKGROUND
Since 1983, the City's Miami Beach Activity Center (MBAC) , located
at 8128 Collins Avenue, contiguous to the Log Cabin Nursery, has
operated an HRS program entitled Independent Living Skills Training
(development training) designed to benefit developmentally disabled
adults. This program is administered by the City's Department of
Development, Design & Historic Preservation Services. Clients
participating in this program are referred from either the State
Department of Health & Rehabilitative Services (HRS) or are
sponsored by their own families. Participants in this program are
limited to thirty-five (35) . The program strives to teach clients
basic living skills, which will allow them to become self-reliant
and no longer entirely dependent upon welfare and social service
agencies for their sustenance. Skills taught as part of the
curriculum include personal hygiene, housekeeping, cooking,
horticulture, interview techniques, etc. Not only does the
program teach and preparethe clients for these roles, but also
provides daily transportation to and from the site. The successful
graduate of the program is then able to maintain a household and
engage in meaningful employment.

In 1989, the City's MBAC become involved in another HRS program
for developmentally disabled adults entitled Supported Employment.
Participants in this program are limited to ten (10) . This program
focuses exclusively on preparing and training clients to assume
jobs utilizing the skills already learned under the Developmental
Training Program, detailed above. Both of these programs, while
funded separately, are intricately linked together. A counselor
makes the initial contact with a prospective employer to secure
potential positions for graduates of the program and then matches
a client's skills and abilities with the job duties and
responsibilities required by the position. Follow-up and progress
reviews are conducted by the counselor to ensure that the client is
able to perform on the job effectively.
In February 1993, the Department of Health and Rehabilitative
Services (HRS) requested that the City of Miami Beach submit an
application for funding for a new program entitled "Supported
Independent Living Services", beginning July 1, 1993. HRS has
assured the City of Miami Beach that funds are available for this
program and the City of Miami Beach does not have to provide a
direct match to qualify for and receive this grant. The purpose of
this program is to provide an independent living environment for
the clients currently being served through the City's Program(s) .
Presently certain clients are living in conditions which place them
at risk due to their family situation, i.e. elderly and
incapacitated parents or changes affecting their present living
status.
Through this grant component clients will be provided with
appropriate rental housing. Specific support services and
supervision, will be provided by present staff and a Sucpor}e�?
Living Coach, in order to ensure the clients health and safety in
their new environment.
ANALYSIS
Presently, the City provides services for up to fifty-two (52)
developmentally disabled adults of which forty-five (4 5) are funded
through these HRS Grants, the other seven (7) clients are privately
funded.
The City must apply annually to HRS for funding of these programs,
as the grant awards are not guaranteed from year to year, nor are
funds automatically allocated to the City. As such, two (2) grant
applications have been prepared which include narrative program
descriptions and proposed operating budgets for each program:
Independent Living Skills training, Supported Employment Training,
Transportation Services, and Supported Independent Living
Opportunities. Whereas, the City of Miami Beach Activity Center is
currently serving Medicaid eligible clients, and HRS now requires
a separate certification for the City of Miami Beach Activity
Center to become a Medicaid provider. Therefore, the Miami Beach
Activity Center Program Supervisor will be responsible for handling
this certification and submission of any applicable documents to
HRS.
Previously the City of Miami Beach had signed separatecontracts
for all these services, however, as of this year HRS now requires
that the following three (3) components be combined into one (1)
main contract: the Independent Living Skills Training Program, the
Supported Employment Training Program, and Transportation services.
There will be a separate contract for the Supported Independent
Living Program.

HRS has tentatively informed the City that it will consider grant
requests in the amount of $125, 050 under the Independent Living
Skills Training (Developmental Training) Program, $76, 679 under the
Supported Employment Training Program, $34, 300 for transportation
services, and $35,400 for the Supported Independent Living Program.
Combining these programs results in a total grant request of
$271,429 for Fiscal Year 1993-94 to fund the Miami Beach Activity
Center's Programs. While over one-half of the Center's operating
budget is provided by HRS, the balance is supported by a Community
Services Block Grant (CSBG) , the donations of private clients, and
the City. Funding at this level will allow the City to continue
providing services for up to fifty-two (52) developmentally
disabled (mentally retarded) adults. The program period begins on
July 1, 1993 and ends on June 30, 1994, in order to coincide with.
the State of Florida's fiscal year.
HRS requires that the City adopt Resolutions which demonstrate the
Commission's authorization to apply to the State of Florida -
Department of HRS, for funding of these essential programs.
CONCLUSION
After the City's two (2) grant applications have been negotiated
and approved by HRS, a final contract for each program, similar in
form to the contract attached hereto, will be executed by the Mayor
and City Clerk.

Introduction/History of the Organization
An activity center for sixteen mentally retarded adults was opened
in July 1975 offering a four hour per day program. The program was
operated in a facility that was provided by the City of Miami
Beach. Services were provided by a primarily volunteer staff with
financial support through grants from the Program Office of
Retardation and the City of Miami Beach Parks and Recreation
Department. As additional funding was obtained, the program was
expanded to six hours per day and comprehensive educational
component, as well as vocational and home living skills seminars
were added.
The City of Miami Beach's Miami Beach Activity Center (MBAC) in
conjunction with Log Cabin Enterprises, Inc. provides a therapeutic
learning environment for developmentally disabled adults living in
the City of Miami Beach. Vocational training is provided through
the Log Cabin Plant Nursery to prepare clients to secure and hold
jobs within the community. Classes in functional reading and
writing, speech, and daily living skills are offered through the
Miami Beach Activity Center. It is the intent of this program to
enable the clients to live independent, self-supporting lives. In
order to more fully accomplish this goal and address the needs of
clients who are at-risk, the organization is preparing to add a
residential component.
Through the support and response of the community, the program has
been able to provide services far beyond original expectations.
Today the program serves 52 clients and offers an eight-hour per
day training program. The clients operate a retail and wholesale
plant nursery. The success of the program is realized through the
placement of numerous clients in employment competition outside of
the nursery, within the past 24 months.
Proposed Program
Despite the program's success and growth, a very critical need
remains--that of providing the clients with a supported independent
living apartments. Training is geared toward equipping clients
with daily and community living skills which will enable them to
live independently. At present, housing opportunities for the
clients are not available within Miami Beach. Fifteen clients have
been identified as at-risk due to their family situation, i.e.
elderly and incapacitated parents or changes affecting their
present living status. It is the intent of the proposed activity to
provide supervised rental housing for three adults.
Goals and Objectives
GOAL:
To provide a supervised rental housing for male and female,
ambulatory, moderately and mildly developmentally disabled
adults from the Miami Beach community.
OBJECTIVES:
a. To provide a long-term supervised, supported living
program for developmentally disabled adults who have been
identified as being at-risk due to poor quality of life
in their current living environment.
b. To provide residential training and community living
skills which will enable higher functioning
developmentally disabled adults to live independently in
the community.
c. To expand the current program by adding a supported
living component to more fully address the goal of
enabling clients to become self-sufficient.
d. To identify suitable rental units for the clients.
Implementation
The supported independent living program is anticipated to be
operational within two months from the receipt of funding.
Rental properties have been identified near the existing Miami
Beach Activity Center (MBAC) Log Cabin facility.
Clients may be referred from the State Department of Health and
Rehabilitative Services, private social service agencies,
institutions, or they may be currently enrolled in the Miami Beach
Activity Center program and are at-risk. All applicants must be
residents of Miami and Miami Beach, medicaid eligible and have the
capacity to learn independent living skills. One or all of the
following instruments will be used to access their capabilities:
Functional Living Skills Assessment, Becoming Independent and Plant
Nursery Assessment. The final decision of admission will be made
jointly by the Program Supervisor, Social Worker and Independent
Living Job Coach, taking into account space availability and
urgency of client's situation. An individual Habilitation Plan
will be developed for each client and quarterly reviews of progress
will be conducted. A maximum of three clients will be admitted to
the residential program.
Two part-time supported living coaches will provide 24 hour
a ,
supervision and support services. Initial start-up costs will total
$13 , 200. 00. The on-going operational costs will be partially
financed by the State Department of Health and Rehabilitative
Services (HRS) . Private donations and additional grants will be
sought to off-set the remaining costs.
Results and Benefits
The creation of the Supported Independent Living Program will
enhance the City's efforts to realize the goal of maximizing the
independent living skills of each individual using the concept of
normalization as their framework. Fifteen developmentally disabled
Miami Beach residents have been identified as in needof supported
living arrangements. Initiating this activity will result in a
model program for supported independent rental housing and the
provider of service for the residents who are most at risk.
The initial investment will be rewarded by offering the opportunity
to live an independent, normal life to developmentally disabled
adults. It will give them a chance to become part of the community
and to make worthwhile contributions through their ability to live
productive, dignified lives.
