97-22493 RESO RESOLUTION NO. 97-22493 A RESOLUTION OF THE MAYOR AND CITY COMMISSION OF THE CITY OF MIAMI BEACH, FLORIDA, URGING THE STATE OF FLORIDA DEPARTMENT OF HEALTH AND METROPOLITAN DADE COUNTY TO SUPPORT AND CONTINUE THE OPERATION OF THE PREVENTION, EDUCATION AND TREATMENT (PET) CENTER AT 615 COLLINS AVENUE, MIAMI BEACH, WHICH PROVIDES NECESSARY HEALTH SERVICES TO THE COMMUNITY. WHEREAS, the Prevention, Education and Treatment (PET) Center located at 615 Colli! s Avenue, Miami Beach, Florida provides health services to approximately 600 patients; and WHEREAS, the services of the PET Center include, among other things, treatment for1T I disease, tuberculosis, and sexually transmitted diseases, psychiatric counselling, anonymous testins for HIV, and childhood immunizations; and WHEREAS, the clinical services provided at the PET Center are consistently highly rIte j in terms of patient satisfaction; and WHEREAS, with regard to HIV, the State of Florida is the third most heavily impacted ntali e in the nation, Metropolitan Dade County is the most heavily impacted county in the State of Florid;, 1, and the City of Miami Beach is one of the most impacted areas in Dade County; and WHEREAS, the services provided by the PET Center are necessary and crucial for th ~ protection of the public health of the community. On July 7, Ira Clark, President of the Public Health Trust, announced to the-Health A.l. Committee o~e Metro-Dade Commissioners that the Public Health 'Trust would guararlt~e the continuation of services provided by the Prevention Education and Treatment (PET) Center at 615 Collins Avenue on South Beach. He further stated that the Trust and the Health Department would prepare a plan to maintain the long term stability of the PET Center services. Previously, the Dade County Health Department had announced that in order to cope with an $8 million budget shortfall, it would close the PET Center on South Beach. This clinic provides care and treatment for people living with mv / AIDS, treatment for TB and Sexually Transmitted Diseases (SID's), childhood immunizations, dental care, psychiatric care, counseling and other services. The PET center is the Health Department's only anonymous mv testing site and only HIV primary care clinic on South Beach. Community activists applaud Clark's pledge to maintain the PET Center services. Dr. BilI Skeen, Director of Public Policy for Florida AIDS Action Council (FLAAC), a statewide HIV advocacy organization, stated, "The community wants to be sure that a permanent solution for maintaining the clinic is put into place, not merely a 'quick fix' that will collapse again next year. It is imperative that the clinic be wen m~aged, weI1 funded and provide quality services in a caring environment." Gary Knight, a member of South Beach's AIDS Prevention Task Force added, "The Health Department stated at their Dade Health Summit in May that combating HIV/AIDS is their highest priority. MaintcUning the PET Center services on South Beach would seem to be the minimum commitment necessary toward achieving that goal." Community representatives are looking forward to a scheduled meeting with Annie Neasman, Chief Administrator of the Dade County Health Department, and a representative from the Public Health Trust. Neasrnan and the Trust representative will be asked to establish a public/private community advisory board for the PET Center. The land was donated to the City of Miami. Beach which in turn deeded it to Dade County to build a Public Health Clinic (see attached deed for stipulations on property). The Center started out as a clinic where the elderly received check-ups. Othe.. services, such as pediatrics, obstetrics, family planning, TB chest x-rays and VORL's (blood tests for syphilis) were added in 1965. No charges were levied fol' '> these services-both rich and poor enjoyed preventative care at this site. In 1990 the focus of the Miami Beach clinic changed in response to the nation- wide HIV/AIDS epidemic. Maternity and Pediatric services were discontinued as the Center began providing primary care for persons who tested positive for HIV infection. The "Early Intervention Project", as it was first called, aimed its services towards persons newly diagnosed with HIV. The Center, now known as the Miami Beach P.E.T. (Prevention, Education and Treatment) Center has treated more than 2000 HIV-infected persons. SERVICES - DEMOGRAPHICS - STATISTICS Primary Care Currently, the patient population is 562 patients who receive high quality compassionate, comprehensive, "one stop" care. This number of primary care patients represents close to the maximum number the P.E.T. Center is willing to absorb because of the complexity of HIV care. Expansion much beyond this number would seriously compromise the quality of care received by our patients. Medical services are provided by 3 physician FTE's and two FTE registered nurses all of whom are highly qualified and experienced in the treatment of HIV disease. This Center exercised an aggressive approach to support programs for protease inhibitors. The majority of its patients, for whom Center physicians prescribed protease inhibitors, were already receiving these medications before they became available through ADAP. Primary Care averages 450 visits per month including an average of 67 walk-ins per month. . petcenterrep. 07. 97page2 Dental Care Services have been provided since 1991, with a satellite at the Downtown Publi:: Health Unit (1350 N.W. 14th Street). The P.E.T. Center is the only provider cf Ryan White dental services on Miami Beach. Dental services provide an average of 186 visits per month: approximately 230 patients access thi:; program. Most dental patients also access primary care: 'eighty-eight patients receive dental services only. These services are available to any patient in Dade County. '''''-,'/1 Nutritional Services Nutritional services have always been a part of the Center's treatment plan and were provided before they were mandated by Ryan White. A registered dietician is provided one day a week through a contract with the Area Health Education Center (AHEC). This service is part of the P.E.T. Center's intake process for new patients since good nutrition is such an integral part of health maintenance for HIV patients. This service averages 24 visits per month. Case Management The P.ET. Center has 4.5 case management positions including a nurse case manager for high acuity and medically complex clients. Case managers average a minimum of 10 face to face contacts each per day. Collateral contacts for each patient seen average 3 per individual. PSYChiatry No support service was more needed for our patients than psychiatry. Moderate to severe psycho-pathology is frequent. Community Mental Health Centers are overburdened and clients with HIV Spectrum Disease have been placed on long waiting lists. Often a client can be medically improved but psychologically decompensated. In the last year other providers have collaborated with the PET Center in referring their clients for psychiatric care. This service averages 64 visits per month and experiences more demand than can be accomodated. HIV Counseling and Testing The P.E.T. Center is the only anonymous testing center on Miami Beach and currently averages 335 visits per month. This service is expected to experience an increase in demand with the implementation of HIV name reporting. DEMOGRAPHICS The demographics of the PET Center are 50% Hispanic, 30% Black-non Hispanic, 15% white non-Hispanic, and 5% other. The community of South Beach has changed over the years from a mainly elderly population to one which includes young, movie and modeling aspirants, immigrants from Central and South America, and seasonal snowbirds from all around the world. pe/cen/errep. 0 7. 97page3 PUBLIC HEALTH SERVICES The Miami Beach P.E.T. Center continues to offer traditional Department of Health services, namely: STD, Pap smears, Immunizations and TB for all who need them. These services are also part of the medical managment of HIV/AIDS clients. In addition, other HIV/AIDS related services are given: Anonymous Testing (identification by number only) and Confidential Testing (identification by name). Sexually Transmitted Diseases (STD'S) ......... The PET Center's STD clinic has a long and successful history of service. Clients from all over Dade utilize this clinic for treatment rather than being treated in their neighborhood. STD's treated at the PET Center include non-specific urethritis, syphilis, gonorrhea, herpes, and venereal warts. During STD examinations physicians have often detected symptoms suggesting that a particular client be tested for HIV. The conjunction of this service with HIV- primary care facilitates the provision of early intervention and treatment. The physician providing STD care at this Clinic has been providing these services for more than 20 years. She is an expert clinician who keeps up to date in this specialty. This service averages 129 visits per month. PAP Tests The American Cancer Society has referred women for PAP smears since 1972. This service is done without charge. Immunizations Immunizations have been given at the PET Center since its inception. Parents who were immunized here frequently return with their children for immunizations. Because of the diversity of the Miami Beach population, immunization records must be deciphered from 40 different countries. This service is available at no charge. Average visits per month are 385. Tuberculosis TB services, including chest X-rays, are very important to Miami Beach as a high percentage of residents from other countries carry high exposure rates. Prophylactic treatment is the primary T8 service provided. This requires a treatment of medication and monitoringed which may last from 6 months to a year. There is a minimal charge for these services, but as with all health care provided here, no patient is refused treatment because of inability to pay. pe~enre"e~O~97page4 TB services are provided on Monday and Thursday afternoons and average 1 DC visits per month. The conjunction of this service with HIV-primary care facilitates the provision of early intervention and treatment. The 1997-98 P.E.T. Center budget by program is as follows: ........
