1) LOBBYI3T FEE EXPENDITURE AND COMPENSATION STATEMENT
Pursuant to City Code Section 2-085(d), fhe City Clerk shall notify any lobbyist (or
principal)who fails to File [imely a Lobbyist Fee Expenditure and Compensation Statement
(Statement). In addition to any other penalties that may be imposed as provided in
Section 2-485.1, a fine of $50.00 a tlay shall be assessed for Statements filetl after ihe
due date. The Ciry Clerk shall notify the Miami-0ade Counry Commission on Ethics and
Publfc Trust of the faiWre of a lobbyist (or principaq to file the Statement or pay the
assessed fines after notification.
My lobbyist, who fails to file the required Statement by April 30�^ of each year, shall be
automatically suspended from lobbying until all fines are paid, unless lhe fine has been
appealed lo ihe Miami-Dade County Commission on Ethics antl Publit TmSt.
I hereby acknowledge that on February 28 of each year, I shall submit to the City Glerk a
signed Sfatement under oath listing all lobbyist expenditures, as well as compensation
received,for the preceding calentlar year wtth regard to the spacific issue on which I have
been engaged to lobby.
2) LOBBYIST WITHDRAWAL
Once an issue I have registered to lobby has concluded or I am no longer lobbying on
that issue, I shall immediately file a No[ice of Withtlrawal wilh the ORice of the Ciry Clerk.
3) ANNUAL�OBBYIST REGISTRATION PEE
I understand annual rogistretion of all lobbyists shall be required for each 12-month period
commencing October 1 of each year, and ihe fee for such annual registration shall be
$500.00 pursuant to Section 2-082 of the Miami Beach Cily Code.

and write down Florida Waste Holdings, Inc., and the Chief Officer's name in the space

Good morning Jason,
I am reaching out to you to confirm that yesterday a courier came by City Hall and
applied the payment for your registration. However, I sent an email last week to Kurt
requesting an update that was brought to my attention.

The City requires that Principals that are Publicly Traded Companies, provide the name
of persons or entities who own 5% or more shares of the company. Once you have
received the required information, if the details do not fit in the space provided, a
separate word document with the breakdown is also acceptable. Simply write in the
provided space "Please see attached breakdown of shareholders owning 5% or more".

The City Clerk has also notified me that instead of N/A, to write "No Compensation".
These updates must be made to both registrations (Jason R Kurt). Once the updates
have been made, please email them to me so I can finalize the registrations. If you like,
you can fill in the details with a pen and scratch off the previously written information
that is no longer needed.
Lastly, The City Clerk has notified me that in light of Waste Connections not being a
non-for-profit organization and Kurt not being the direct owner of the company, he would
need to pay the required Lobbyist Fees ($500.00 Annual Fee, $350.00 New Issue Fee).
If it is easier to have someone stop by City Hall like they did for your payment, I can
assist them in getting the payment applied. If you have any questions, or if there is

reviewing your papervvork, We have been notified that a few more details are required,
in order to complete your registration.
i. On page 1, section II, Name of all persons holding, directly or indirectly, a 5% or
more ownership interest.
Although it is a publicly traded company, we would need to have a breakdown of any
