1) LOBBYIST FEE EXPENDITURE AND COMPENSATION STATEMENT
Pursuant to Cily Code Section 2-485(d), the Ciry Clerk shall notify any lobbyist (or
principal)who faila to fle timely a Lobbyist Fee Expenditure and Compensation Statement
(Statement). In eddition to any other penalties that may be imposed as provided in
Section 2485.1, a fne of$50.00 a day shall be assessed for Statements filed after the
due date. The City Clerk shall notify the Mlami-Dade Counry Commission on Ethics and
Public Trust of the tailure of a lobbyist (or principal) to file the Sfatement or pay the
assessed flnes atter notifiption.
My lobbyist, who fails to file the required Statement by April 30'^ of each year, shall be
automatically suspended trom lobbying until all flnes are paid, unless the Me has been
appealed to the Miam!-Dade County Commission on Ethics and Public Trust.
I hereby acknowledge that on February 28 of each year, I shall submit to the City Clerk a
sipned Statement under oath listing all lobbyist expenditures, as well as compensation
receWed, for the preceding calendar year wkh regard to the specific issue on which I have
been engaged to lobby.
2) LOBBYIST WITHDRAWAL
Once an issue 1 have registered to lobby has conGuded or I am no longer lobbying on
that issue, I shall immediately file a Notice of Withdrewal with the Office oT the City Clerk.
3) ANNUAL IOBBYl3T REGISTRATION FEE
I understand annual registration of all lobbylsts shall be required for each 12-month period
commencing Odober 1 of each year, and the fee for such annual registration shall be
$500.00 pursu it to Section 2-482 of the Miami Beach Ciry Code.
