If you have nothing to report in a particular section , write "none" or "n/a." Any change to the information originally filed requires submission of an amended form. A IMPORTANT -Individual Registration Forms are Required for Each Issue: A separate registration form must be submitted for each issue on which you are appearing to express support or opposition. Neighborhood association means an organization of residential homeowners and tenants created to address quality- of-life issues in a defined neighborhood or community. Neighborhood association shall also mean a residential condominium or cooperative association. Pursuant to Section 2-481 of the City Code, the term "lobbyist" excludes any person who appears as a representative of a neighborhood or homeowner association without compensation or reimbursement, whether direct, indirect, or contingent, solely to express support of or opposition to an item. However, Section 2-483(b) requires that such individuals submit this disclosure form for each issue on which the person appears. By completing and signing this form, you affirm under penalty of perjury that you are not receiving compensation or reimbursement for this appearance and that you are excluded from the definition of "lobbyist." You also acknowledge that you may not give or receive any contingency fee in connection with this appearance, as prohibited by Section 2- 485.3 of the City Code. Forms filed with the Office of the City Clerk are public records under Florida law. If your home address is exempt from disclosure or if you prefer it not be made public, you may provide an office or alternate mailing address instead. Florida Statutes Section 119.071(4)(d) provides public records exemptions for certain individuals and their families. Even when you are speaking on behalf of a neighborhood association, you are not required to register as a lobbyist or complete this form when interacting with City government on many quality-of-life or public safety matters such as reporting a pothole or broken streetlight, requesting a code compliance inspection, submitting a noise or sanitation complaint, inquiring about trash pickup or recycling, discussing a permit or zoning matter, or seeking general information from City staff.
