If you received a gift from an individual or a single entity during this period that exceeds $100, you are required to report it. However, if no such _gift was received, no reportinq is necessary. Who Must File Form 9? • Elected officials. • Anyone required to file Form 1 (Statement of Financial Interests). i Who Must File the Miami-Dade County Quarterly Gift Disclosure Form? • Any City employee who received a reportable gift (over $100 in value) during the reporting period and is not required to file Form 9. Where to File: • Form 9 must be submitted to the Commission on Ethics at P.O.
