I. REQUESTS FOR CONTINUANCES
1. DRB File No. 22935. The applicant, Clear Channel Outdoor, is requesting
Design Review Approval for modifications to existing bus shelters throughout the
City, including the installation of digital advertising. [The applicant is requesting
a continuance to February 5, 2013.]

2. DRB File No. 22912, 25 East Rivo Alto Dr. The applicant, Are Traasdahl, is
requesting Design Review Approval for the construction of a new 2 -story home,
which will replace an existing pre-1942 architecturally significant home, to be
demolished. [Note: Application approved on August 7, 2012, with th e
exception of the landscape plan.]

3. DRB File No. 22926, 980 West 48th Street. The applicant, MJS Family Trust, is
requesting Design Review Approval for the construction of a new 2 -story home,
to replace an existing pre-1942 architecturally significant single story home, to be
demolished. [Note: Application approved on November 6, 2012, with the
exception of the landscape plan.]

4. DRB File No. 22923, 440 West Dilido Drive. The applicants, Ricardo and M aria
Moraes, is requesting Design Review Approval for the construction of a new 2 -
story home, to replace an existing pre -1942 architecturally significant 2-story
home, to be demolished. [Note: Application approved on November 6, 2012,
with the exception of the landscape plan.]

5. DRB File No. 22941, 1691 Michigan Avenue – The Putting Club. The
applicant, City Middle, LLC., is requesting Design Review Approval for
modifications to an existing 6-story parking garage, including the construction of
several single story roof-top structures, which include an outdoor bar, kitchen,
and restrooms, as part of a new miniature golf course facility.

II. SINGLE FAMILY HOMES
1. Previously Continued Applications from November 6, 2012.
a. DRB File No. 22922, 6 Star Island Drive. The applicant, 6 Star Island
Residence Land Trust Agreement, is requesting Design Review Approval
for the construction of a new roof-top addition on an existing one (1) and
two (2) story home, along with the construction of a new 2-story addition
at the front of the property. [Note: Application approved on November
6, 2012, with the exception of the landscape plan.]

a. DRB File No. 22944, 5446 North Bay Road. The applicant, Dalia
Glottman, is requesting Design Review Approval for the construction of a
new 2-story home, to replace an existing pre-1942 architecturally
significant 2-story home, to be demolished.

b. DRB File No. 22946, 808 West Dilido Drive. The applicant, The Happy
Sunset Trust, is requesting Design Review Approval for the construction
of a new 2-story home, to replace an existing single story home, to be
demolished.

III. APPLICATIONS FOR DESIGN REVIEW APPROVAL
1. New Applications
a. DRB File No. 22943, 827 West 41st Street – Domino’s. The applicant,
Domino’s Pizza Inc., is requesting Design Review Approval for the
construction of a new single story commercial building, to replace an
existing single story structure and parking lot, to be demolished.
APPROVED, with the exception of the Landscape Plan, including outdoor seating plan,
which was Continued to March 5, 2013
b. DRB File No. 22942, 4701 North Meridian Avenue – Former Miami
Heart institute. The applicant, 4701 North Meridian, L LC., is requesting
a Design Review Approval for the partial demolition, renovation, and
adaptive re-use of the former hospital site as a new multifamily
residential development, retaining existing nonconforming height,
setbacks, floor area, and off-street parking.
APPROVED
c. DRB File No. 22945, 910 West Avenue – Southgate Towers. The
applicant, Southgate Towers, LLLP, is requesting Design Review
Approval for modifications to the exterior of two (2) existing 14 -story
apartment buildings, including design modifications to the existing

facades, and new roof -top terraces, and a new hardscape and landscape
plan for the site, including an outdoor dining area, and 10th Street street-
end improvements in the Public right-of-way, and modifications to
previously approved conditions for the construction of a public baywalk.
