1. DRB File No. 22764, 2767 Sunset Drive – Single Family Home.

II. EXTENSIONS OF TIME
a. DRB File No. 22734, 8143 Harding Avenue – Terrazas del Mar. The applicant,
Adan Beltran, is requesting a one (1) year Extension of Time to obtain a Full
Building Permit for a previously issued Design Review Approval for the
construction of a new 3-story multifamily building, which will replace an existing
single story building, to be demolished.

b. DRB File No. 22831, 7251 Collins Avenue – North Shore Bandshell Park. The
applicant, the City of Miami Beach, is requesting a one (1) year Extension of
Time to obtain a Full Building Permit for a previously issued Design Review
Approval for the renovation of the existing park, including new landscaping, site
furnishings, lighting, walkways, plazas and signage.

c. DRB File No. 22833, 7251 Collins Avenue – AIPP Project, ‘Beatles Mandala’.
The applicant, The City of Miami Beach, is requesting a one (1) year Extension
of Time to obtain a Full Building Permit for a previously issued Design Review
Approval for an Art in Public Places (AIPP) project located within Northshore
Bandshell Park.

a. DRB File No. 22878, 2125 N. Bay Rd. – Single Family Home. The
applicant, Marilyn Blanco-Reyes, is requesting Design Review Approval
for the construction of a new 2-story home, which will replace an existing
pre-1942 architecturally significant home, to be demolished. [Note: This
application was approved on February 7, 2012, with the exception of
the design for the front portion of the home.]

b. DRB File No. 22895, 401 West Rivo Alto Drive. The applicants, Mr. and
Mrs. Carmine Camera, are requesting Design Review Approval for the
construction of a new 2-story home, which will replace an existing pre -
1942 architecturally significant single story home, to be demolished.
[Note: Continued from April 3, 2012 meeting.]

c. DRB File No. 22896, 5146 Alton Road. The applicant, Pavel Pojidaev, is
requesting Design Review Approval for design modifications to an
existing 2-story pre-1942 architecturally significant home, including the
construction of a new roof -top addition on the existing single story portion
of the home. [Note: Approved on April 3, 2012, with the exception of
the design details.]

a. DRB File No. 22899, 6555 Allison Road. The applicants, Alan and Traci
Waserstein are requesting Design Review Approval for the construction
of a new 2-story home, which will replace an existing home, to be
demolished.

a. DRB File No. 22900, 2140 Calais Drive – Duplex. The applicant,
Fernando Maestre, is requesting Design Review Approval for the
construction of a new 2-story addition to an existing single story
multifamily building.

V. MODIFICATIONS TO PREVIOUSLY APPROVED PLANS
1. DRB File No. 18869, Block 51, 801-863 South Pointe Drive, 818-842
Commerce Street. – BLOCK 51. The applicant, TRG-Alaska III, L.L.C., is
requesting modifications to a previously issued Design Review Approval for the
construction of a new 7-story mixed-use project on a vacant lot. Specifically, the
applicant is requesting substantial design modifications to the previously
approved project, including the construction of a mechanical parking garage.
