I. REQUESTS FOR EXTENSIONS OF TIME
1. DRB File No. 19233, 6115 Pinetree Drive – Garcia Residence. The applicant,
Jose Garcia, is requesting a one (1) year Extension of Time to obtain a Full
Building Permit for a previously issued Design Review Approval for the partial
demolition and renovation of an existing pre-1942, architecturally significant
single family home, including the construction of a new 3-story addition.

2. DRB File No. 19232, 4412 North Bay Road – Kodsi Residence. The applicant,
Isaac Kodsi, is requesting a one (1) year Extension of Time to obtain a Full
Building Permit for a previously issued Design Review Approval for the partial
demolition and renovation of an existing pre-1942, architecturally significant
single family home, including the construction of a new 2-story addition.

3. DRB File No. 19236, 1 Washington Avenue – South Pointe Park. The
applicant, the City of Miami Beach, is requesting a one (1) year Extension of
Time to obtain a Full Building Permit for a previously issued Design Review
Approval for an Art in Public Places (AIPP) project.

II. MODIFICATIONS TO PREVIOUSLY APPROVED PROJECTS
1. DRB File No. 3882, 4775-4779 Collins Avenue – Blue and Green Diamond.
The applicant, Blue and Green Diamond Master Association, Inc., is requesting
re-approval of the modification to the design of the pyramidal roof-top elements,
previously approved in 2003.

III. APPLICATIONS FOR DESIGN REVIEW APPROVAL
1. Single Family Homes
a. DRB File No. 20926, 4712 Alton Road – Single Family Home. The
applicant, Anthony Parks, is requesting Design Review Approval for
modifications to an existing pre-1942 architecturally significant single

construction of a new 2-story addition and a new roof-top addition.

b. DRB File No. 21269, 88 La Gorce Circle – Single Family Home. The
applicant, 88 La Gorce LLC, is requesting approval for revisions to a
previously approved two (2) story single family home, with an accessible
roof deck, which will replace an existing 2-story single family home, to be
demolished.

c. DRB File No. 21268, 280 South Hibiscus Drive – Single Family Home.
The applicant, Riad A. Kabakibi, is requesting approval for the
construction of a new two (2) story single family home, which will replace
an existing 2-story single family home, to be demolished.

Condominium. The applicant, The Grand Venetian Condominium
Association, is requesting Review Approval for the installation of new
landscaping and fencing.

Community Center. The applicant, Miami Beach Jewish Community
Center, is requesting Design Review Approval for the construction of a
new 3-story educational, cultural and recreational facility.
