II. APPLICATIONS FOR DESIGN REVIEW APPROVAL
1. Previously Continued Matters:
a. DRB File No. 14772, 1 Fisher Island Drive - Hazelwood Apartments. The
applicant, Fisher Islands Holdings, LLC, is requesting Design Review
Approval for the construction of four (4), ten (10) story apartment
structures with a total of 140 units; this application is proposed to be
approved as a Phased Development Project.

b. DRB File No. 15384, 2001 Meridian Avenue - Lofts on the Green. The
applicant, FTP Investment Corporation, is requesting Design Review
Approval for the construction of a new eight (8) story condominium
building on a vacant lot facing Dade Boulevard and entered from
Meridian Avenue.

c. DRB File No. 15490, 1225, 1235, and 1245 11th Street, 1125 and 1141
West Avenue, and 1230 and 1234 12th Street. The applicant, Charles
E. Smith Residential Realty Inc., is requesting Design Review Approval
to demolish an existing six (6) story apartment building and an existing
one (1) story apartment building in order to construct a five (5) story
parking garage with four (4) residential units fronting West Avenue.

2. New Projects:
a. DRB File No. 15560, 6985 Collins Avenue - Crystal Beach Club . The
applicant, First Leisure Corp., is requesting Design Review Approval for
the partial demolition, alteration and modification of the north and west
façades of an existing four (4) story hotel. Specifically the applicant is
proposing to remove an existing, non-original curtain wall and restore
and renovate the remaining façade.
