1. Extensions of Time
a. DRB File No. 12890, 5875 Collins Avenue - Flatotel Sea Spa Palace.
The applicant, American Leisure Resorts, is requesting a one (1) year
Extension of Time to obtain a Building Permit for a previously issued
Design Review Approval for the construction of a 21 story, 336 unit
condominium/hotel building with accessory restaurant, bar, boutique and
meeting rooms.
APPROVED : One (1) Year
b. DRB File No. 14086, 103 63rd Street - 63rd Street Residences. The
applicant, La Gorce West, Inc., is requesting a one (1) year Extension of
Time to obtain a Building Permit for a previously issued Design Review
Approval for the construction of a 6 story, 28 unit condominium.

APPLICATIONS FOR DESIGN REVIEW APPROVAL
Previously Continued Matters
1. DRB File No. 15014, 5685 Alton Road - La Gorce Country Club. The applicant,
La Gorce Country Club, is requesting Design Review Approval for the
demolition of existing facilities in order to construct a new clubhouse, cart
storage building, pool and pool cabanas.

2. DRB File No. 14745, 1011 - 1015 Alton Road - Dorick Apartments. The
applicant, Jerry Wish, Trustee, is requesting Design Review Approval for the
demolition of existing residential structures in order to construct a four (4) story,
mixed-use facility consisting of retail, parking and residential units.

3. DRB File No. 14772, 1 Fisher Island Drive - Hazelwood Apartments. The
applicant, Fisher Islands Holdings, LLC, is requesting Design Review Approval
for the construction of four (4), ten (10) story apartment structures with a total of
140 units; this application is proposed to be approved as a Phased
Development Project.

4. DRB File No. 15384, 2001 Meridian Avenue - Lofts on the Green. The
applicant, FTP Investment Corporation, is requesting Design Review Approval
for the construction of a new eight (8) story condominium building on a vacant
lot facing Dade Boulevard and entered from Meridian Avenue.

Revisions to Previously Approved Plans
1. DRB File No. 11467, 1420-1428 West Avenue. The applicant, Alliage
Holdings, L.L.C., is requesting revisions to a previously issued Design Review
Approval for the demolition of an existing single family house and the
construction of a 12-unit, five (5) story residential building, inclusive of parking.

1. DRB File No. 15490, 1225, 1235, and 1245 11th Street, 1125 and 1141 West
Avenue, and 1230 and 1234 12th Street. The applicant, Charles E. Smith
Residential Realty Inc., is requesting Design Review Approval to demolish an
existing six (6) story apartment building and an existing one (1) story apartment

residential units fronting West Avenue.

2. DRB File No. 15491, 1661 and 1685 Michigan Avenue. The applicant, Colonial
Bank South Florida, Inc., is requesting Design Review Approval for the
construction of two (2) drive-through lanes and an automatic teller machine
(ATM).
