The purpose of this Change Order is to: (highlight the key reasons for this Change Order)
y Add a fire suppression system to the two (2) VFP shelters purchased on
Contract 02-19333/V AS;
y Relocate the prime site equipment room;
y Relocate the Power-Pac UPS system at the Parkview Plaza
Condominium; and
y Add a two-week project extension due to Hurricanes Frances and
Jeanne.

In accordance with the terms and conditions of the contract identified above between
City of Miami Beach and Motorola, Inc., the following changes are approved:

Original Contract Value: $ 7,874,421.00
Previous Change Order amounts for Change Order
Numbers 1 and 2 $ 521,159.00
This Change Order: $ 98,353.69
New Contract Value: $ 8,493,933.69

~ Incorporate FMS200 Fire Suppression into the two VFP shelters purchased on
Contract 02-19333/vas per the attached Equipment List;
~ Revise and provide new prime site equipment plans for a new location; and
y Add construction material for relocation of the Power-Pac UPS (entry door,
latform, and adin).
~ Installation and field testing of the FMS200;
~ Engineering and documentation for the relocation of the prime site.
~ Engineering and construction services for the relocation of the Power-Pac UPS
A detailed description is provided in the attached Statement of Work.

The price increases in the amount of $98,353.69 for the
~ Purchase and installation of the FMS200 fire suppression system and
y The labor and materials to relocation the prime site equipment room and the
Power-Pac UPS.
The City of Miami Beach shall have the Parkview Point Condominium Association
approve construction plans for the relocation of the Power-Pac UPS and shall assist
Motorola in acquiring the building permits.

The additional amount of $98,353.69 is to be paid as per Section 5 "Payment Schedule"
of the contract 02-109333/vas.
The amount of $98,353.69 is to be added to the payment milestone titled "Final
Acceptance. "

Unless amended above, all other terms and conditions of the Contract shall remain in full
force. If there are any inconsistencies between the provisions of this Change Order and
the provisions of the Contract, the provisions of this Change Order will prevail.
