located on the west side of Collins Avenue between 640 -700 Collins Avenue, Miami
Beach, Florida.
The results of analyses of all samples collected did not exhibit parameters above Dade
County regulatory guidelines. Allied Environmental Consultants, Inc. does not
recommend further investigation at this time.

1.0 INTRODUCTION
This report was prepared to summarize the findings of a Phase II Subsurface
Investigation performed at the property located on the west side of Collins Avenue,
between 640 - 700 ColJins Avenue, Miami Beach, Florida. The investigation was
undertaken by Allied Environmental Consultants, Inc. (AEC) at the request of Mr, Marlo
Courtney, of Ballet Valet, Inc., in response to the recommendations of a Phase I
Environmental Assessment performed for the property in March, 1994.

The scope of work included the installation of three (3) monitoring wells in the parking
lot associated with the subject property to determine the impact, if any, on the
subsurface. Potential impact may have resulted from the past use of the site by film
developing studios and print shops, as well as from the former service station on the
southeast comer of Collins Avenue and 7th Street, northeast of the subject property.
During installation of the monitoring wells, soil gases were monitored for the presence
of volatile hydrocarbons.

2.0 FIELD INVESTIGATION
Prior to proceeding with any subsurface investigation, underground utility clearance was
received by AEC from Sunshine State One Call Center and the City of Miami Beach
Public Works Department during the period March 24 - 28, 1994. Sunshine State One
Call Center is a liaison service provided by the local utility companies to notify and mark
the appropriate utilities to avoid damage to underground lines during subsurface
investigation.
2.1 MONITORING WELL INSTALLATION
Three (3) fifteen foot monitoring wells were installed on Thursday, April 7, 1994 within the
parking lot associated with the subject property (see monitoring well location map,
Appendix A, Figure 1). The well was installed by International Technologies, Inc. using
a truck mounted drill rig equipped with 7 7/S-inch outside diameter hollow stem augers.
The augers were decontaminated using high pressure steam prior to well installation.
The wells were constructed of 2-inch diameter schedule 40 PVC casing with ten (10) feet
of well screen (slot size .01- inch) and with five feet of riser to reach the surface. The
annular space between the boreholes and the screens was backfilled with clean well
graded (8/20) silica filter sand to a height of 6-inches above the well screens. A 6-inch
layer of bentonite was placed above the filter pack and wetted to seal the wells before
additional sand was added to the surface. The wells were finished with a water-tight
lOCking cap and developed until the water ran free of sediment using a portable gas
powered centrifugal pump.

2.2 GROUNDWATER SAMPLES
AEC personnel collected the groundwater samples on Tuesday, April 12, 1994, in
accordance with Florida Department of Environmental Protection (FDEP) sampling
protocol (CAP #930112G). The monitoring well depth and water table elevations were
measured to determine the volume of groundwater to be purged before collection of the

sample. The wells were purged of at least three volumes of water prior to sample
collection. The samples were collected using decontaminated tefton bailers which were
decontaminated prior to the collection of the samples. The samples were placed in an
iced cooler and transported to Precision Environmental Laboratory (QAP#92-Q323G) to
be analyzed for Total Petroleum Hydrocarbons, RCRA Metals, and EPA Methods 601
and 602 parameters.
2.3 SOIL SAMPLING
Composite soil samples were collected on Thursday, April 7, 1994 from 2 foot intervals
until the water table was reached. Each of the 2 foot interval soil samples were collected
from the borings for analysis with an Organic Vapor Analyzer equipped with a flame
ionization detector (OVAlFID). Each soil sample was placed in a 16-0unce soil jar, until
half full, sealed with foil, capped and placed in a water bath to equilibrate at
approximately 700F for five (5) minutes. After equilibration, the head space of each
sample was analyzed with the OVA/FID to provide the most representative method of
detecting the presence of volatile hydrocarbons in the head space of soil samples.

3.0 RESULTS
Copies of the laboratory data are included as Appendix B. Results of the laboratory
analysis revealed that the groundwater samples tested for EPA Methods 601 and 602
parameters exhibited no_compounds above detectable limits. Groundwater samples
tested for Total Petroleum Hydrocarbons and RCRA Metals exhibited se)leral compounds
above detection limits, but below regulated levels.

One soit-sample analyzed in the field with an OVNFID during the installation of the wells
revealed the presence of volatile hydrocarbons see Table 3.1. During the installation of
monitoring well MW3 (see Monitoring Well Location map, Appendix A, Figure 1), a layer
of loose clay with organic materials at a depth below the water table (11'-12.5') exhibited
840 parts per million (ppm). AEC does not believe this to be significant considering the
groundwater sample collected at the same location exhibited no contaminants above
detectable limits.

4.0 SUMMARY AND CONCLUSIONS
Three (3) monitoring wells were installed within the parking lot associated with the
subject property located between 640 - 700 Collins Avenue. The wells were located to
determine the impact, if any, to the soil and groundwater from the past use of the site
by film developing studios and print shops, as well as from the service station which was
previously located at the southeast comer of Collins Avenue and 7th Street.
Groundwater samples tested for Total Petroleum Hydrocarbons and RCRA Metals
exhibited several compounds above detection limits, but below regulated levels.
The results of this investigation indicate that the parking area of the subject property has
not been impacted significantly by EPA Methods 601/602, RCRA Metals, or Total
Petroleum Hydrocarbons. All samples collected exhibited either levels of contamination
below Dade County regulations or below detectable limits. Allied Environmental
Consultants, Inc. recommends no further investigation at this time.
