Dear Mr. Siaba,
The purpose of this letter is as a follow up to our prior correspondence of March 29th and
April 25th to inform you of additional services that CDM has performed during the course of
the Construction Services for the Pump Station Upgrades Project. We are requesting an
increase in CDM's upper limit budget to cover these additional services. In the meantime, we
have utilized the not-to-exceed portion of Amendment 13 A (Task 2.0), which was intended
for shop drawing reviews and other miscellaneous unidentified job issues. The four main
areas of additional services are associated with the following miscellaneous issues: DERM
assistance, structural inspections, pump monitoring and shop drawings.
DERM Assistance (Permitting/Accelerated Construction Coordination): In response to a
request form the City, CDM has prepared a set of permit applications and drawings for
various individual pump stations to meet the permitting requirements of DERM based on a
meeting CDM has had with the department. CDM has attended other meetings with DERM
and MDW ASD regarding the project with the intent of "fast tracking" the procurement of
new construction permits for the project. Other out of scope work included the increased
coordination between CIP, the Contractor, CDM, and Public Works for the accelerated
construction of Pump Station No.1, which was placed under moratorium by DERM. As you
will recall, there was an extensive amount of effort expanded by CDM during the last two
weeks of March and early April to get Pump Station No.1 on-line by April 6th. Our cost
projections in our earlier letter (dated April 25th) were in-line with the amount incurred.
Through the end of July, CDM had spent about $35,220 on the permitting and expediting
activities associated with resolving the City's issues with DERM.

Structural Inspections: During the course of recent construction, CDM noticed that the
structural columns that support the ground floor above the pump chambers at Pump Stations
No.1 and No. 28 showed evidence of fatigue due to corrosion. At the City's request, CDM
retained a structural engineering specialist (Dr. Bilgen Erel) to inspect the columns and
recommend a method to repair them and return their structural integrity. Additionally, the
City has expressed the desire to explore the possibility of converting an existing junction box
at the Pump Station No.1 site to a screen chamber. This improvement would help to protect
the new pumps from clogging with debris and rags. The chamber will require structural
repair from corrosion damage in addition to the structural modifications. Dr. Erel included an
evaluation of the structural damage to the junction box at the time he was on-site to inspect
the column damage. Dr. Erel inspected the structural component as described in this
paragraph and prepare structural contract drawings and specifications for the repairs. The
cost of these additional inspections and design is a lump sum $6,950 and is detailed in a letter
proposal from Dr. Erel attached to this letter. CDM is requesting a total of $8,000 including
our staff time.
Pump Monitoring: CDM assisted the City with evaluating an alternative to the SCADA
monitoring as described in the original contract documents. CDM assisted with the
negotiation of a change order to implement the selected alternative (Nextel radios) and has
had numerous correspondences between the City and the Contractor to establish the service
in time to be utilized by Pump Station No.1. CDM is requesting a total of $7,000 to cover the
costs of investigating and coordinating this issue with the various entities.
Shop Drawings: To date, CDM has received, logged-in and processed 366 shop drawings for
this project. Due to the unknown quantity of shop drawings the City and CDM agreed to
utilize a not-to-exceed contract billed at hourly rates to reduce the project costs. We are asking
for an increase of $15,000 to cover the cost of the anticipated remaining shop drawings.
We trust that this letter is sufficient to inform you of these additional services and the impact
on the current upper limit of $110,940 for the not-to-exceed work. At this time we are asking
for an increase in the upper limit cap on this particular task of the project of $65,000 to a total
of $175,940. This will take into account the additional items as described and provide for
additional unforeseen needs which may arise out of the project.

Pursuant to our recent conversation, Engineering Technologies, Inc. (err) is pleased to submit
this proposal to provide structural contract drawings and specifications for the repair of the
following structural items noted in Wastewater Pump Station No.1.
SCOPE OF SERVICES

Task 1: efT staff will conduct a site visit to examine the existing columns. Contract drawings
and specifications will be prepared to repair the cut in the two steel columns in the pump
station.
Task 2: e/T staff will prepare contract drawings and specifications to repair the lightly corroded
concrete wall of the flow splitter box. We propose to design a new wall inside the vault to
provide structural strength. The existing walls shall be abandoned in place. A site visit will be
conducted to assess the extent of corrosion.
COST AND SCHEDULE

elT proposes to undertake the work described above on a lump sum fixed fee basis at a price of
$6,950. Task I has a cost of52,100 and Task 2 has a cost of $4,850.

Dear Mr. Siaba:
As you are aware, the current construction contract for the above referenced project is
projected to extend past the contractual substantial completion date of September 18, 2005.
Based on the Contractots latest construction schedule dated July 28, 2005 work on the project
is projected to extend until April 12, 2006.
The contract between the City and CDM for general services and resident inspection services
was intended to run concurrently with the construction contract. Based on the additional time
the Contractor is expected to complete the project (approximately 177 calendar days), we
request an increase in our contract to extend our services to the new projected completion
date as follows:
. General Services-The current contact was a lump sum amount of $500,000 for a period of
555 days. This averages $900 per day and was based on an adequate staffing level to cover
the anticipated activity level of the Contractor to complete the work in the original time
period. Due to the extended period of work our daily activity level is not anticipated to be
as much. Therefore we propose a new daily rate of $600 per day for the additional time
period. This rate, projected out for the anticipated 177 additional construction days
increases our lump sum costs by $106,200. Combined this gives a new total lump sum
amount for general services of $606,200. CDM will bill these general services at a rate of
$18,310 per month of additional contract time.
. Resident Inspection Services-Similarly, the resident inspection services need to be
extended. The current contract is not-to-exceed amount of $313,700 for a construction

period of 510 days. This averages $615 per day and was based on staffing the project with
inspectors to cover the anticipated activity to finish the job in the original time period. By
stretching the project out over a longer period of time, the activity is not as much as
anticipated. Therefore, we propose a rate of $460 per day for the extended period of 177
days. This would raise the upper limit of the not-to-exceed contract to by $81,420 to a new
amount of $395,120. This would be billed at our standard billing rates against the new
upper limit.
The above is summarized on the attached calculation sheet.

As you are aware, the current construction contract for the above referenced project is
projected to extend past the contractual substantial completion date of August 18, 2005. Based
on the Contractor's latest construction schedule dated July 28, 2005 work on the project is
projected to extend from a final completion of October 17, 2005 until December 16, 2005.
The contract between the City and CDM for general services and resident inspection services
was intended to run concurrently with the 480 day construction contract time. Based on the
additional time the Contractor is expected to complete the project (approximately 44 calendar
days), we request an increase in our contract to extend our services to the new projected
completion date as follows:
. General Services - The current contact was a lump sum amount of $182,369 for a period of
480 days. This averages $380 per day and was based on an adequate staffing level to cover
the anticipated activity level of the Contractor to complete the work in the original time
period. Therefore we propose to maintain the rate of $380 per day for the additional time
period. This rate, projected out for the anticipated 44 additional construction days increases
our lump sum costs by $16,720. Combined this gives a new total lump sum amount for
general services of $199,089. We will bill for these services at monthly rate of $11,558 per
month.
. Resident Inspection Services - Similarly, the resident inspection services need to be
extended. The current contract is not-to-exceed amount of $95,250 for a construction period

of 480 days. This averages $198 per day and was based on staffing the project with
inspectors at approximately 20 hours a week. We propose to maintain the rate of $198 per
day for the extended period of 44 days. This would raise the upper limit of the not-to-
exceed contract to by $8,712 to a new amount of $103,962. Services will be billed at our
current billing rates up to the new not-to exceed ceiling.
. Reimbursables - We have reached the upper limit of the Reimbursables (Task 4.0). We
would request that an additional $610 be added to the reimbursables (Task 4.0). This would
raise the upper limit to $6,998 or 3.5 percent of the total GS labor.
The above is summarized on the attached calculation sheet.
